Case reports of adverse outcomes create suspicion about water birth safety, which restricts the availability of water birth in the United States. The objective of this study was to synthesize the information from case reports of adverse water birth events to identify practices associated with these outcomes, and to identify patterns of negative outcomes. The research team conducted a systematic search for cases reports of poor neonatal outcomes with water immersion. Eligible manuscripts reported any adverse neonatal outcome with immersion during labor or birth; or excluded if no adverse outcome was reported or the birth reported was unattended. A qualitative narrative synthesis was conducted to identify patterns in the reports. There were 47 cases of adverse outcomes from 35 articles included in the analysis. There was a pattern of cases of Pseudomonas and Legionella, but other infections were uncommon. There were cases of unexplained neonatal hyponatremia following water birth that need further investigation to determine the mechanism that contributes to this complication. The synthesis was limited by reporting information of interest to pediatricians with little information about water birth immersion practices. These data did not support concerns of water aspiration or cord rupture, but did identify other potential risks. Water immersion guidelines need to address infection risk, optimal management of compromised water-born infants, and the potential association between immersion practice and hyponatremia.The aim of this study was to evaluate the effects of a behavioral infant calming technique to support mothers' parenting self-efficacy and parenting satisfaction. Exposure to CT might lead to clinical differences eventually observed in these patients. We present a cross-sectional study with 54 patients with schizophrenia spectrum disorder (schizophrenia and schizoaffective disorder). We obtained sociodemographic data, as well as data on CT, dissociation, suicide history, and intensity of positive and negative psychotic symptoms. More than 75% of the patients reported a history of CT. We observed a link between CT and suicidal behavior. Patients showed high rates of dissociation. Dissociative experiences were related to CT, both in terms of intensity of trauma and number of traumas experienced. All CT forms except emotional neglect showed direct correlations with dissociative experiences. We found no correlation between intensity of CT and intensity of positive psychotic symptoms, yet we observed a moderate inverse correlation with negative psychotic symptoms.
To compare clinical outcomes between pars plana vitrectomy (PPV), scleral buckling (SB) and PPV+SB for Rhegmatogenous Retinal Detachment (RRD) in the Japan-RD Registry.
This is a nation-wide, multi-center, observational study based on the registry data between 2016 and 2017. The failure levels were defined as level 1 (a failure of RD repair), level 2 (remaining silicone oil), and level 3 (multiple surgeries to achieve reattachment). We compared cases treated by SB or PPV in the subgroup of simple RRD using multivariate Cox proportional hazard models.
A total of 2,775 cases were included. Overall, 6-months any levels of failure in total, SB, PPV, and PPV+SB were 9.2% (n=256), 6.9% (n=48), 8.2% (n=157), and 21.3% (n=51), respectively. Poor visual acuity at baseline in SB and Inferior RRD and larger retinal tear in PPV were associated with higher risk of failure. PPV was associated with higher chance of achieving primary success in cases with simple RDD especially for cases with superior RDD (adjusted hazard ratio 3.61, 95%CI 2.22 to 5.94, P<0.001).
