stroke education at all levels of care.Our expanding knowledge of the pathophysiology of inflammatory bowel disease (IBD) has led to the development of a multitude of new therapies, including parenterally administrated biologic agents and new oral small molecules. Tofacitinib is the first compound of a promising class of new small molecules approved for the treatment of IBD. This pan-Janus kinase (JAK) inhibitor (JAKi) targets the four isoforms of cytokine associated JAKs (JAK1, JAK2, JAK3 and TYK2). Next generations JAKi with marked selectivity for specific JAK isoforms or gut-restricted effect are in development, with promising results in phase I and II clinical trials. Whether increased JAK selectivity will translate into more favorable clinical efficacy and safety profiles remains to be demonstrated in larger clinical trials. Here we provide an overview of the clinical and pharmacological aspects of these drugs and discuss how they may be incorporated in the current treatment paradigm for Crohn's disease and ulcerative colitis.
To assess the impact of intrauterine balloon tamponade as a second-line technique before resorting to emergency peripartum hysterectomy in cases with severe postpartum hemorrhage and uterotonic treatment failure following vaginal delivery.
A retrospective, monocentric study evaluating cases of uterotonic treatment failure with severe postpartum hemorrhage following vaginal delivery between January 2005 and June 2018. Cases of cesarean section, amniotic embolism, uterine rupture, placenta accreta, and postpartum hemorrhage by vaginal laceration were excluded. The primary study outcome was incidence of emergency peripartum hysterectomy before and after use of intrauterine balloon tamponade was initiated.
Among the 55,776 women who gave birth vaginally at our site during the study period, 161 (0.3 %) had severe postpartum hemorrhage with medical treatment failure (78 during the period prior to intrauterine balloon tamponade use, and 83 after intrauterine balloon tamponade use was initiated). The intrauterien conservative surgical treatment was followed by intrauterine balloon tamponade.
Coronavirus disease 2019 (COVID-19) is a public health emergency of international concern and has caused traumatic experience for nurses worldwide. However, the prevalence of depression and anxiety symptoms in nurses, and how psychosocial factors influence nurses in this public crisis are unknown.
To determine the effect of COVID-19 on the mental health of nurses and the prevalence of anxiety and depression symptoms among nurses in China during the outbreak.
A cross-sectional study.
A total of 3,228 nurses in Sichuan Province and Wuhan City were selected by convenience sampling. All participants were invited to complete the questionnaire through WeChat from January 27 to February 3, 2020.
A self-reported questionnaire combining depression and anxiety scale was used to collect data anonymously. Binary and multivariate logistic regression was applied to measure the odds of psychosocial factors of anxiety and depression and perceived health, respectively.
The total incidence of depression (34.3%) andevealed the high prevalence of depression and anxiety among nurses during the outbreak of COVID-19. COVID-19 factors and psychosocial factors were associated with mental health of nurses. The results suggest that hospitals should implement effective mental health promotion programs focused on occupational safety and family support to improve the well-being of nurses.Borderline personality disorder is marked by high levels of comorbidity in both adolescent and adult populations. However, the mechanisms involved in the development of comorbidity in BPD remain unclear. To address this issue, the current paper proposes the use of dynamic mutualism theory as a valuable and underexplored framework for investigating comorbidity in BPD from a developmental perspective. Specifically, we discuss how predictions of dynamic mutualism can be extended to better understand the onset, maintenance, and interplay of BPD symptoms with other forms of psychopathology over time. Moreover, we suggest that mutualistic interactions among internalizing and externalizing features throughout early development may foster the emergence of BPD symptoms in adolescence and beyond. Next, we discuss methodological approaches for testing mutualism and review indirect evidence that supports the role of mutualistic processes in the emergence and maintenance of BPD and its comorbidities. We conclude with methodological cautions and recommendations for future studies.We report the effect of thermal annealing on structure, composition, optical transmittance and thickness of a novel fluorozirconate glass (ZLANI) containing Zr, La, Al, Na and In fluorides. In this work, pulsed laser deposition was used to grow thin films of ZLANI, and thermal annealing at different temperatures was performed on the films. Annealing did not change the composition, but a clear structural evolution of the ZLANI glass was observed by transmission electron microscopy (TEM), showing that we can control microstructure independent of composition. An increase in transmittance after the film was subject to a 100 °C thermal anneal was ascribed to the removal of defects and structural relaxation in the amorphous state. Following an anneal of 200 °C, the transmittance decreases due to heterogeneous formation of crystalline nuclei and changes in the local bonding. After the final annealing at 300 °C, a wider-scale crystallization occurred, with some major crystal phases formed as Zr2F8(H2O)6 and ZrO2, which alters the shape of the transmittance curve. The crystalline content of the crystal phases that form in the annealed films was quantified using hollow cone dark field TEM imaging. The 100 °C or 200 °C annealing decreases the film thickness by inducing structural relaxation and densification of the amorphous films, while the thickness increase of the 300 °C annealed film resulted from the formed large crystals. These results provide insights for the design of multilayer nanocomposites with a ZLANI glass matrix, which have potential applications as up-/down-conversion luminescent materials and X-ray storage phosphors.The mineralocorticoid receptor (MR) plays a central role in cardiac physiological function and disease and is thus an attractive therapeutic target for patients with heart failure. However, the incidence of significant side effects from mineralocorticoid receptor antagonist (MRA) treatment has led to investigation of new mechanisms that may enhance MR targeted therapies. Recent studies have identified the circadian clock as a novel, reciprocal interacting partner of the MR in the heart. While the closely related glucocorticoid receptor (GR) and its ligand, cortisol (corticosterone in rodents), are established regulators of the circadian clock, new data suggest that the MR can also regulate circadian clock gene expression and timing. This review will discuss the role of the MR and its ligands in the regulation of the circadian clock in the heart and the implications of dysregulation of these systems for cardiac disease progression, and for MR activation.
The brain is a common site of metastatic disease for patients with advanced melanoma. Brain metastasis portends a poor prognosis, often causing deterioration in neurological function and quality of life, and leading to neurological death. Treatment approaches including surgery, radiotherapy and systemic therapy can lead to better control of this problem. Therefore, appropriate guidelines for the management of melanoma brain metastases need to be established, with regular updating when new treatment options become available.
A multidisciplinary working party established by Cancer Council Australia has produced up-to-date, evidence-based clinical practice guidelines for the management of melanoma. Comparison of the chromatographic properties of the prepared stationary phases in suppressed ion chromatography (IC) mode using hydroxide eluent allowed us to evaluate the effect of the number of substituents at nitrogen atom in alkanolamine on selectivity of anion exchangers toward organic acids. Obtained anion exchangers were also examined together with previously described hyperbranched stationary phases with different mono- and diamines in the external part of the layer for evaluating the influence of various parameters on their selectivity. Effects of hydrophilicity, functionality of amine, and the number of substituents at nitrogen atom of amine used in the last modification cycle were established independently from each other, which provided the possibility to tailor selectivity toward organic acids when preparing anion exchanger for solving particular analytical task. Predominance of hydrophilicity as a key factor affecting the separation of weakly retained organic acids over other studied parameters was demonstrated.Botanical drugs have unique advantages in the treatment of complex diseases. In order to ensure the efficacy and safety of botanical drugs, ascertaining the effective and risk compounds is quite necessary. However, the conventional identification method is laborious, time-consuming, and inefficient. In this work, a 3-steps strategy was presented to rapidly identify the effective and risk compounds of botanical drugs, and a Ginkgo biloba preparation, Shu-Xue-Ning injection (SXNI), was taken as a case study. Firstly, mass spectral molecular networking was used to rapidly identify the compounds of SXNI. Secondly, three networks (i.e. the compound-target network, the indication-related biomolecule network, and the adverse drug reaction-related biomolecule network) are constructed. Finally, a novel network analysis algorithm was used to predict the effective and risk compounds in SXNI. By this strategy, a total of 138 compounds were identified including the firstly reported terpenoid glycosides and lignan glycosides. Among them 71 compounds were predicted as effective ones, and 42 compounds as risk ones. Especially, 31 compounds relevant to both efficacy and safety should be scientifically controlled during manufacturing. In addition, ten pathways were enriched to preliminarily explain the action mechanism of SXNI. This strategy for MS data analysis can be applied to provide important basis for the manufacturing and quality control, as well as valuable points for research on the pharmacological mechanisms of botanical drugs.
