Introduction Acute emotional stress triggers autonomic responses that affect sympathovagal balance. However, the temporal pattern of changes in each autonomic arm during stress and recovery remains unclear. Therefore, we analyzed separately sympathetic and vagal activity, elicited by acute unpredictable stress in a rat model. Methods Continuous electrocardiographic recording was performed during (32 minutes) and after (two hours) successive use of restraint and air-jet stress in 10 rats, whereas five rats served as controls. Sympathetic and vagal indices were calculated non-invasively after heart rate variability analysis. Voluntary motion was quantified during recovery, as an index of continuing anxiety. Results The sympathetic nervous system index increased during stress and remained elevated during the initial stage of recovery. The parasympathetic nervous system index decreased immediately after the onset of stress and remained low throughout the observational period. During recovery, voluntary activity was more pronounced in the stress group than in the controls. Conclusion Successive restraint and air-jet stress in rats increased sympathetic activity and decreased vagal activity. These changes displayed only partial recovery post-stress and were accompanied by enhanced voluntary motion. Our findings may be important in the evaluation of the cardiac electrophysiologic implications of autonomic changes elicited by acute emotional stress.Cardiac Implantable Electronic Devices (CIED)-associated infective endocarditis complicated by septic emboli and acute on chronic pulmonary hypertension is rare. We present a case where pulmonary thromboendarterectomy was required for treatment. A 55 year-old man with a history of myocardial infarction and ischemic cardiomyopathy status-post ICD placement 8 years prior presented with bacteremia, infected ICD, and tricuspid valve vegetation. He underwent CIED extraction along with the use of the AngioVac suction device to remove right ventricular and atrial vegetations. However the patient had persistent valvular vegetation and bilateral sub-massive pulmonary emboli. Pulmonary angiography showed filling defects in the lobar and segmental arteries. Percutaneous attempts at embolectomy were unsuccessful and he therefore underwent a pulmonary endarterectomy surgery (PTE). This case of CIED- associated endocarditis demonstrates the importance of early aggressive treatment of such infections. Guidelines recommend compete CIED system removal when there is associated infection. The AngioVac is a novel system for removal of right-sided vegetations and thrombi; however, complications such as distal embolization can occur. PTE surgery for septic emboli is rare. Skeletal myoblasts and bone marrow-derived stem cells are two types of stem cells being tested. The objective is to evaluate the efficacy of these two types of stem cells for heart failure therapy. Data were searched in PubMed using both regular and Medical Subject Heading (MeSH) keywords (stem cells, therapy, heart failure) and then filtered using inclusion/exclusion criteria (language, species, publication date, and age). In total, 31 research articles were reviewed (14 clinical trials, four randomized control trials, nine review articles, one case report, one comparative study, one systematic review, and one categorized as a systematic review and meta-analysis). Both skeletal myoblasts and bone marrow-derived stem cells showed mixed results in improving left ventricular ejection fraction in heart failure patients in the majority of studies. Forty years ago, methyl ethyl ketone was used in the Navy to degrease and remove paint from planes and naval equipment. A 57-year-old Navy veteran presented with an absence of all fingernails and thumbnails as well as pterygium formation on his digits; however, his toenails were normal. Additional history revealed that his unprotected hands were regularly exposed to methyl ethyl ketone for three years. His nails shed and stopped growing after one year of this work; subsequently, pterygiums developed. We postulate that exposure to methyl ethyl ketone may result in chemical destruction of the nail matrix in a similar manner to phenol or sodium hydroxide. We introduce the following acronym that describes not only the etiology but also the manifestations of Methyl Ethyl Ketone-related Loss of Matrix with Nail Onycholysis and Pterygium (ME KLMNOP).
