Most of the techniques or programs used in automated testing platforms are customized to identify the common variants seen in that particular region. At times, variant Hbs may be identified which are not commonly seen in the local population. This may cause diagnostic dilemma and may require further studies for definitive characterization. We present a patient with Hb Rush (HBB c.304G>C), a rare unstable Hb variant eluting in the Hb S (HBB c.20A>T) window on HPLC.Introduction Around 10% of melanoma patients have a positive family history of melanoma and/or related cancers. Although a germline pathogenic variant in a high-risk gene can be identified in up to 40% of these patients, the remaining part of melanoma heritability remains largely unexplained.Areas covered The aim of this review is to provide an overview of the impact that new technologies and new research approaches had and are having on finding more efficient ways to unravel the missing heritability in melanoma.Expert opinion High-throughput sequencing technologies have been crucial in increasing the number of genes/loci that might be implicated in melanoma predisposition. However, results from these approaches may have been inferior to the expectations, due to an increase in quantitative information which hasn't been followed at the same speed by an improvement of the methods to correctly interpret these data. Optimal approaches for improving our knowledge on melanoma heritability are currently based on segregation analysis coupled with functional assessment of candidate genes. An improvement of computational methods to infer genotype-phenotype correlations could help address the issue of missing heritability.OBJECTIVES To estimate differences in spending and utilization between hospice users and non-users with lung cancer by length of hospice enrollment. STUDY DESIGN Retrospective analysis using 2009-2013 Medicare claims. METHODS The study sample was a 10% random sample of Medicare fee-for-service beneficiaries with lung cancer who died between 2010 and 2013. We identified different categories of hospice users (hospice enrollment for 1-7 days, 8-14 days, 15-30 days, 31-60 days, 61 days - 6 months) and non-users. We used propensity score matching to match users in each enrollment category with non-users. The outcomes were a) total Medicare spending, b) number of hospitalizations, c) number of emergency department (ED) visits, d) number of physician-administered chemotherapy claims, and e) number of radiation therapy sessions. Regression analysis was used to compare outcomes between users and non-users by enrollment period. RESULTS Hospice users had significantly lower spending, fewer hospitalizations, and fewer ED visits than non-users across all categories of hospice enrollment. Large savings occurred when patients stayed in hospice for at least one month (US$16,566 for those enrolled 61 days - 6 months; US$16,409 for those enrolled 31-60 days). Significant reduction in use of outpatient services including chemotherapy and radiation therapy was observed among patients using hospice for at least 1 month. CONCLUSIONS Hospice led to cost savings by reducing utilization of aggressive care towards end-of-life among lung cancer patients. While cost savings were realized even when hospice is utilized for a short duration, large savings occurred when hospice is used for at least 1 month.Carl Ferdinand von Arlt, Ritter von Bergschmidt (1812-1887) was a pioneer in ophthalmology. The purpose of our paper is to highlight his scientific work. He did not only introduce new surgical techniques and invent new instruments in ocular surgery, but also influenced the development of ophthalmology in the 19th century. After a brief review of the oldest forms of embalming, the author, Michele Martone, presents petrification as the only way to obtain the perfect conservation of the corpse. CONCLUSION This scientific note presents some considerations regarding the constant search of humanity to arrest, if not the death of a person, the decomposition of their body. Copyright © 2019 by Academy of Sciences and Arts of Bosnia and Herzegovina.A biographical note on Teodora Krajewska (1854-1935) reveals the details of her life and professional activities as an Austro-Hungarian and Yugoslav health officer (Amtsärztin) in Tuzla (1893-1899) and in Sarajevo (1899-1923). Teodora Krajewska, née Kosmowska was the third of nine official female doctors employed by the Austro-Hungarian administration in occupied Bosnia and Herzegovina (BH 1878-1918) and charged with the special task of popularising public health and hygiene, particularly among Muslim women. A Polish intellectual and fervent patriot from Warsaw, Krajewska had left Congress Poland as a young widow in 1883 to study medicine in Geneva, Switzerland. In 1890, she became the first woman in Europe to be employed as an assistant professor at the medical faculty of the University of Geneva but was forced to resign in 1892. In the same year, she was both awarded her doctorate and appointed to the position of an Austro-Hungarian female health officer in Tuzla. After being nationalised in Austria, she reported for duty in Tuzla in March 1893. selleckchem In 1899, she accepted her transfer to a newly created position in Sarajevo where she was active as an official physician until 1922/23. She contributed to contemporary medical science through her research on leprosy and osteomalacia in Bosnia. She returned to Warsaw in 1928 and devoted herself to the translation of Serbo-Croatian literature and writing her memoirs on her life and activities in BH.
