Vital statistics are the primary source of data used to understand the mortality burden of dementia in the US, despite evidence that dementia is underreported on death certificates. Alternative estimates, drawing on population-based samples, are needed.

To estimate the percentage of deaths attributable to dementia in the US.

A prospective cohort study of the Health and Retirement Study of noninstitutionalized US individuals with baseline exposure assessment in 2000 and follow-up through 2009 was conducted. Data were analyzed from November 2018 to May 2020. The sample was drawn from 7489 adults aged 70 to 99 years interviewed directly or by proxy. Ninety participants with missing covariates or sample weights and 57 participants lost to follow-up were excluded. The final analytic sample included 7342 adults.

Dementia and cognitive impairment without dementia (CIND) were identified at baseline using Health and Retirement Study self- or proxy-reported cognitive measures and the validated Langa-Weir score 95% CI, 7.0%-12.5%). Underlying cause of death recorded on death certificates (5.0%; 95% CI, 4.3%-5.8%) underestimated the contribution of dementia to US mortality by a factor of 2.7. Incorporating deaths attributable to CIND revealed an even greater underestimation.

The findings of this study suggest that the mortality burden associated with dementia is underestimated using vital statistics, especially when considering CIND in addition to dementia.

To beta test a secure teleophthalmology mobile app at military treatment facilities in Afghanistan.

This prospective case series included 16 military treatment facilities at diverse roles of care including forward operating bases in Afghanistan and 1 location outside of Afghanistan. Thirty point-of-care medics and medical professionals were included from September to November 2019.

Users placed teleophthalmology consults on their mobile phone using the mobile eye care app, and an expeditionary ophthalmologist stationed at a military hospital in Afghanistan responded. Users graded the mobile app using a rating scale from 1 to 5, with 1 being very dissatisfied and 5 being very satisfied.

Mean initial response time, agreement between the teleophthalmology diagnosis and final diagnosis, treatment and management following recommendations outlined in the Joint Trauma System clinical practice guidelines, preven 5 (minimum, 3; maximum, 5). All 28 consults (100%) were secure and compliant with the Health Insurance Portability and Accountability Act.

The temperature-dependent resistivity behaviors were investigated for the films with changing core-occupation ratio of clusters. Much interestingly and surprisingly, a switchable metal-insulator transition can be observed, featuring the rapid switching from the metal state to the insulation state and then back to the metal state, for films within a specific range of core-occupation ratio. Further, the resistivity change rate used to characterize the metal-insulator transition can reach as high as two orders of magnitude over a very narrow temperature region. The design of Fe/Fe3O4 core-shell clusters plays a decisive role in the mechanism of the switchable metal-insulator transition in these films. The assembled core-shell clusters in the films form current conduction channels that are switchable between the cores and shells of clusters as the temperature changes. The switching of the current conduction channels can be regulated by controlling the core-occupation ratios of clusters, which induce a switchable metal-insulator transition and can be verified by the effective medium theory over a specific core-occupation ratio range.A protection method for cycloalkynes by the formation of (hexafluoroacetylacetonato)copper(i)-cycloalkyne complexes is disclosed. Here, we report on the observation of compact ferrimagnetic skyrmions for the class of amorphous alloys consisting of 4f rare-earth and 3d transition-metal elements with perpendicular magnetic anisotropy, using a DyCo3 film, that are identified by combining X-ray magnetic scattering, scanning transmission X-ray microscopy, and Hall transport technique. These skyrmions, with antiparallel aligned Dy and Co magnetic moments and a characteristic core radius of about 40 nm, are formed during the nucleation and annihilation of the magnetic maze-like domain pattern exhibiting a topological Hall effect contribution. Our findings provide a promising route for fundamental research in the field of ferrimagnetic/antiferromagnetic spintronics towards practical applications.
