Several multiparametric magnetic resonance imaging (mpMRI)-based models have been developed with significant improvements in diagnostic accuracy for clinically significant prostate cancer (csCaP), but lack proper external validation. We therefore sought to externally validate and compare all published mpMRI-based csCaP risk prediction models in an independent Asian population.

A total of 449 men undergoing combined transperineal fusion-targeted/systematic prostate biopsy at our specialist center between 2015 to 2019 were retrospectively analyzed. csCaP was defined as lesions with ISUP (International Society of Urological Pathology) grade group ≥2. The performance of 6 mpMRI-based risk models (MRI-ERSPC-3/4, Distler, Radtke, Mehralivand, van Leeuwen and He) were evaluated in terms of discrimination, calibration and clinical utility, using area under the receiver operating characteristic curve (AUC), calibration curves and decision curve analyses.

A total of 202 (45%) subjects were diagnosed with csCaP. AI-based risk models demonstrate excellent discrimination and clinical utility and are easy to apply in practice, suggesting that individualized risk-based approaches can be considered over mpMRI alone to avoid unnecessary biopsies.

Recreational athletes (n = 18 (9 females), aged = 26.2 ± 6.2 years) performed 12 maximal 30-m sprints on a non-motorised treadmill. Three-dimensional kinematics and ground reaction forces were subsequently recorded during a 10-m maximal overground sprint before and immediately after the repeated running protocol, from which we calculated work done by sagittal plane hip, knee, and ankle moments. Relative work (J/kg) was reported as a percentage of positive and negative work done by the sum of joint moments.

Following the repeated running protocol, maximal sprint speed decreased by 19% and was accompanied by reductions in total positive (-1.47 J/kg) and negative (-0.92 J/kg) work, in addition to work done by hip (-0.43 to -0.82 J/kg) and knee (-0.28 J/kg) moments during swing. Rate of torque development (RTD) and normalised EMG were measured during rapid voluntary isometric contractions from rest.

All neuromuscular variables were unaffected by time in CON. In HOT, MVT, normalised EMG at MVT and VA were lower at 39.5°C compared to 37°C (p<0.05). Early- (0-50ms) and middle- (50-100ms) phase voluntary RTD were unaffected by increased T
(p>0.05), despite lower normalised EMG at T
39.5°C (p<0.05) in rapid contractions. In contrast, late-phase (100-150ms) voluntary RTD was lower at 38.5°C and 39.5°C compared to 37°C (p<0.05) in HOT. Evoked twitch and octet RTD increased with increased T
(p<0.05).

Hyperthermia reduced late-phase voluntary RTD, likely due to reduced neural drive and the reduction in MVT. In contrast, early- and middle-phase voluntary RTD were unaffected by hyperthermia, likely due to the conflicting effects of reduced neural drive but faster intrinsic contractile properties.
