The diagnosis of malnutrition in patients with independent hepatocellular carcinoma (HCC) varies from 20% to 50%, is related to important complications and has a direct impact on the prognosis. Determination of the resting energy expenditure (REE) has become an important parameter in this population, as it allows therapeutic adjustments to recover their nutritional status. The REE in cirrhosis, with and without HCC, is not clearly defined, and requires the identification and definition of the best nutritional approach.
To evaluate the REE of patients with cirrhosis, with and without HCC.
This is a prospective observational study evaluating the REE of 118 patients, 33 with cirrhosis and hepatocellular carcinoma and a control group of 85 patients with cirrhosis without HCC, using indirect calorimetry (IC), bioimpedance, and predictive formulas.
The REE determined by IC in cirrhotic patients with HCC was 1643 ± 364 and in those without HCC was 1526 ± 277 (

Although this lesion is likewise rare in veterinary medicine and there were sparse reports in several species such as dogs, cats, cows, and calves, we incidentally encountered ectopic hepatic tissue in a guinea pig. In the case of guinea pigs, we report for the first time the occurrence of ectopic hepatic tissue implanted in the gallbladder. The healthy guinea pig remained asymptomatic, and its macroscopic findings also showed no abnormalities. Hematological examinations exhibited moderate decreases in white cell counts, hemoglobin concentrations, and packed cell volume ratio. Serum biochemical examinations showed decreases in total protein and albumin and increases in glucose levels, alkaline phosphatase, lactate dehydrogenase, creatine kinase, and γ-glutamyl transpeptidase. There were slight changes in electrolytes (Cl, Ca, and Mg) and inorganic phosphorus, indicating minor deviations from physiologic ranges. Histopathological examination revealed the presence of normal hepatic structures (hepatocytes and hepatic cords) within the wall of the normal gallbladder. The vascular and bile duct structures and portal triads were not observed in the ectopic hepatic tissue. A 13-year-old crossbreed female dog was submitted to surgery to remove an inguinal mammary nodule. The tumour was composed of mammary epithelium and mesenchymal populations, not only of cartilage and bone but also of myoid cells. PTAH demonstrated cross striation of striated muscle, and immunohistochemistry highlighted striated muscle expressing desmin and calponin, and smooth muscle expressing desmin, SMA, and calponin. The tumour was diagnosed as a benign mixed tumour with leio- and rhabdomyoid differentiation. There was no tumour recurrence after one year of clinical follow-up. In conclusion, the well-differentiated features of leiomyocytes and rhabdomyocytes and the growth pattern define the benign origin of the mesenchymal component, which has been confirmed by a benign outcome; therefore, the knowledge of this kind of differentiation is helpful to avoid misdiagnoses.Coronary artery anomalies (CAAs) are congenital vascular defects which can remain hidden and asymptomatic over the complete life course of an individual. They are defined as deviations from the normal coronary anatomy regarding the arterial origin, course, or both. Their incidence varies from 1.3% to 5.64% in coronary angiography cohorts, and they can be detected as incidental findings. In certain cases, CAAs can be hemodynamically significant and unfortunately can be proven lethal. Their link with sudden cardiac death, especially in otherwise healthy competitive athletes, is well established, but their prognostic significance, range of symptoms, and pathophysiology remain to be further elucidated. Here, along with a brief review of related literature, we present a series of three cases one case of an anomalous origin of the right coronary artery (RCA) from the left coronary sinus, one case of a split RCA originating from the left coronary sinus, and one case of a dual left anterior descending (LAD) artery system.
Chronic megacolon is a rare condition which primarily occurs in patients with autonomic dysfunction of a variety of causes. Its management is often challenging and people with chronic megacolon often suffer from abdominal distension, pain, and malabsorption. Given the struggles clinicians experience in managing these patients long term, this case study provides an example of an alternate strategy for the symptomatic management of chronic megacolon.
. An 80-year-old male with early Parkinson's disease developed megacolon following a basal ganglia stroke. He had a protracted hospital stay over 6 months due to malabsorption requiring total parenteral nutrition and electrolyte disturbances. A trial of subcutaneous neostigmine was unsuccessful, so patient underwent a trial of intermittent rectal tube decompression which improved his symptoms and malabsorption. This technique was then taught to the patient's wife until she was confident performing this herself. With continuation of decompression approximately every three days, the patient was able to return to oral nutrition and no longer required ongoing electrolyte replacement. He was able to be discharged into the community with significant improvement in his quality of life.
This is the first report to suggest the benefit of intermittent rectal tube decompression in the community for the long-term management of chronic megacolon. Further prospective studies should evaluate the potential for this strategy to be implemented in a wider cohort of patients who are not responsive to existing treatments for chronic megacolon.
