Authors assessed APP prevalence and APP association with beneficiary race/ethnicity and payer among publicly-insured adults regardless of diagnosis. Retrospective repeated panel study of fee-for-service (FFS) Medicare, Medicaid, and dually-eligible white, black, and Latino adults residing in California, Georgia, Iowa, Mississippi, Oklahoma, South Dakota, or West Virginia, filling antipsychotic prescriptions between July 2008 and June 2013. Primary outcome was any monthly APP utilization. Across states and payers, 11% to 21% of 397,533 antipsychotic users and 12% to 19% of 9,396,741 person-months had some APP utilization. Less than 50% of person-months had a schizophrenia diagnosis and up to 19% had no diagnosed mental illness. Payer modified race/ethnicity effects on APP utilization only in CA; however, the odds of APP utilization remained lower for minorities than for whites. Elsewhere, the odds varied by race/ethnicity only in OK, with Latinos having lower odds than whites (odds ratio 0.76; 95% confidence interval 0.60-0.96). The odds of APP utilization varied by payer in several study states, with odds generally higher for Dual eligibles, although the differences were generally small; the odds also varied by year (lower at study end). APP was frequently utilized but mostly declined over time. APP utilization patterns varied across states, with no consistent association with race/ethnicity and small payer effects. Greater use of APP-reducing strategies are needed, particularly among non-schizophrenia populations.The phenomenon of a precursor spreading in front of an advancing droplet is still not fully understood. We recently used a driven lattice-gas model to study the microscopic dynamics of thin film spreading. We found that the scaling exponents describing the dynamics of both the precursor and the bulk layers are not universal, and strongly depend on the parameters describing the various interactions in the system. In this paper we show yet another nontrivial and rich behavior of the system. This is the non-monotonicity of the exponents, which reflects the competition between the various mechanisms that drive the system. In particular, we study the dependence of the scaling exponents on the ratio of the external substrate potential and the internal short-range interactions that drive the spreading in both layers.Management of vestibular dysfunction may include treatment with medications that are thought to act to suppress vestibular function and reduce or eliminate abnormal sensitivity to head motions. The extent to which vestibular medications act centrally or peripherally is still debated. In this study, two commonly prescribed medications, meclizine and diazepam, and a candidate for future clinical use, JNJ7777120, were evaluated for their effects on short latency compound action potentials generated by the peripheral vestibular system and corresponding central neural relays (i.e., vestibular sensory-evoked potentials, VsEPs). The effects of the selected drugs developed slowly over the course of two hours in the mouse. Findings indicate that meclizine (600 mg/kg) and diazepam (> 60 mg/kg) can act on peripheral elements of the vestibular maculae whereas diazepam also acts most effectively on central gravity receptor circuits to exert its suppressive effects. The novel pharmacological agent JNJ7777120 (160 mg/kg) acts in the vestibular periphery to enhance macular responses to transient stimuli (VsEPs) while, hypothetically, suppressing macular responses to sustained or slowly changing stimuli.The UV cross-linking technique applied to the cornea is a popular and effective therapy for eye diseases such as keratoconus and ectatic disorders. The treatment strengthens the cornea by forming new cross-links via photochemical reactions and, in turn, prevents the disease from further developing. To better understand and capture the underlying mechanisms, we develop a multi-physics model that considers the migration of the riboflavin (i.e., the photo-initializer), UV light absorption, the photochemical reaction that forms the cross-links, and biomechanical changes caused by changes to the microstructure. Our model is calibrated to a set of nanoindentation tests on UV cross-linked corneas from the literature. Additionally, we implement our multi-physics model numerically into a commercial finite element software. We also compare our simulation against a set of inflation tests from the literature. The simulation capability allows us to make quantitative predictions of a therapy's outcomes in full 3-D, based on the actual corneal geometry; it also helps medical practitioners with surgical planning.

Testicular ascent is a well-reported outcome of the surgery and a problem that could be fixed with a repeat procedure. The purpose of this review is to determine the rate of subsequent testicular ascent and repeat surgery after a two-stage Fowler-Stephens using best available evidence.
