Multiple sclerosis (MS) is a chronic demyelinating inflammatory disorder with variable clinical and pathologic characteristics reflecting multiple underlying pathophysiologic mechanisms. Repositioning of existing drugs for the new indications offers several advantages including significant reduction in the cost and time of drug development and exemption from early phase clinical trials. Minocycline has been reported to exhibit immunomodulation in several pre-clinical and clinical studies through suppression of migratory inflammatory cells, modulation of peripheral immune response, and inhibition of microglial activation within the CNS.

Here, the authors review the repositioning potential of minocycline for the treatment of MS along with appraisal of the evidence obtained from preclinical and clinical research. The authors also discuss the advantages and potential safety concerns related to the use of minocycline for the management of MS.

To evaluate the effectiveness and safety of allogeneic tendons for functional reconstruction of severe hand injuries.

From August 2007 to July 2014, we performed functional reconstruction with tendon allografts for severe hand injuries affecting two or more tendons. At the final follow-up, we assessed total active motion (TAM); pincer pinch strength; grip strength; Disabilities of the Arm, Shoulder, and Hand (DASH) score; degree of satisfaction; and adhesion. We measured the white blood cell count, C-reactive protein concentration, erythrocyte sedimentation rate, total T-cell count, and CD4
T/CD8
T ratio to evaluate the immune response and check for infection.

Ten patients received 26 allogeneic tendons to reconstruct hand function. The average follow-up period was 50.0 months (range, 24-82 months). The TAM was 126.4° (12°-253°), pincer pinch strength was 0.83 kg (0-4.5 kg), and grip strength was 13.69 kg (4-41.5 kg). The DASH score was 14.25 (3.3-30.8), and seven and three patients were satisfied and partially satisfied, respectively. One patient developed tendon adhesion. All immune and infectious parameters were within the reference range.

Detection of carriers and premarital counseling play an important role in preventing the birth of a thalassemic child. The present study aimed to detect large numbers of asymptomatic carriers in rural areas of West Bengal, India. The present cross-sectional study was conducted over a period of 10 years. Thalassemia awareness programs and detection camps were organized at the community level. After signed written consent was obtained, the collected blood samples were subjected to a complete blood count (CBC) in an automated blood cell counter and then analyzed by high performance liquid chromatography (HPLC); in difficult cases, samples were sent to the reference laboratory for molecular characterization. Out of 287,258 samples collected, 32,921 (11.46%) cases revealed abnormal hemoglobins (Hbs); of these, 31,782 (11.06%) carried heterozygous states (carriers/traits), and the remainder were either homozygous or compound heterozygous for different hemoglobinopathies. Two common variants were revealed in the study, namely β-thalassemia (β-thal) (7.23%) and Hb E [β26(B8)Glu→Lys, HBB c.79G>A] (2.77%) traits. Among homozygous or compound heterozygous states, Hb E/β-thal (0.14%) and β-thal major (β-TM) (0.12%) were predominant. In rural areas of West Bengal, the most common Hb variants detected were β-thal and Hb E traits. In view of the high prevalence of hemoglobinopathies in this region, routine premarital screening and genetic counseling should be emphasized and encouraged to prevent the birth of a thalassemic child, and thus curtailing the burden on families and the health economy.This study was performed to investigate the user perception of a cordless, motorized electronically controlled delivery system for botulinum toxin type A. Forty-six post-graduate students of varying experience levels of botulinum toxin injections and four members of the faculty from the MSc programme in Esthetic Medicine at Queen Mary University London participated in a demonstration of a motorized injection device. Thereafter, they performed a number of injections on a high fidelity silicone simulation model before completing a nine-item questionnaire. The more experienced injectors tended to appreciate the accuracy of the device more than the less experienced participants. Seventy-eight percent of participants said the device improved accuracy, particularly when administering small doses. Eighty-four percent leaned toward a favorable general view of the device. Forty-seven percent would possibly consider purchasing the device. Sixty-one percent would consider recommending the device to a colleague. The main advantage of the motorized injection device was the improved accuracy enabling delivery of small and precise doses.
