Background The novel coronavirus, SARS-CoV-2, has been responsible for the devastation of hundreds of thousands of lives directly and has caused disruptions globally. Vulnerable populations, specifically those suffering from serious mental illness and homelessness, are at higher risk of contracting COVID-19 infection resulting in medical complications and psychiatric destabilization. In addition, mental health has become increasingly relevant throughout the country given the psychological distress people have been facing due to the spread of COVID-19 and the toll of a more restricted way of living. Although the healthcare industry has quickly integrated novel ways of treating patients with mental illness with technological advances, these technologies are not applicable to different populations equally. There is a clear disparity that is represented within the public county health systems, which leads to a widening gap between those who receive adequate treatment for mental illness and those who do not. Aims ates, serve as a rationale for the need of innovative solutions for the integration of these services in not only emergency situations such as the COVID-19 pandemic, but also in daily non-emergent operations to sufficiently address the needs for those needing mental healthcare.Background The end plate plays an important role in intervertebral disc degeneration progression. The aim of the study was to examine the compositional and structural changes of the end plate with age and to investigate the correlation between end plate and disc degeneration by T1ρ and T2 map magnetic resonance imaging. Methods There were 12 young monkeys (6-7 years old), 20 aged monkeys (14-17 years old) and 12 human participants (30-50 years old) in this study. T1ρ or T2 map values of the nucleus pulposus and end plate cartilage were analyzed according to Pfirrmann grades and age. Afterwards, micro computed tomography and histological analysis were used to confirm the end plate changes in monkeys. Pearson's correlation was performed to investigate the relationship between end plate and disc degeneration. Results In monkeys, T1ρ (r=-0.794, P less then 0.001) and T2 map values (r=-0.8, P less then 0.001) of the nucleus pulposus were negatively associated with Pfirrmann grades. Moreover, the T2 map was more suitable than T1ρ for the evaluation of end plate degeneration. Age was an important influence factor of end plate and disc degeneration, which was confirmed by microcomputed tomography, Safranin O/fast green staining, and collagen II staining. The T2 map value of lower end plate degeneration positively correlated with that of the intervertebral discs in monkeys (R2=0.3133, P less then 0.001) and humans (R2=0.2092, P less then 0.001). Conclusion This study suggests that the compositional and structural changes of the end plate can be quantitatively evaluated by T2 map. Furthermore, cartilage end plate degeneration is associated with disc degeneration during ageing. The translational potential of this article A better understanding of how the cartilage end plate affects disc degeneration is needed, which may propose a new clinical application using T2 map to evaluate end plate degeneration.The Interstate Highway System (IHS) used to be a symbol of American growth and its economic machine, but the IHS has not been expanded much since its inception. Recently there are both executive and legislative desires to restore and upgrade the IHS to its role as a premier system that can meet the growing and changing demands of the twenty-first century. Demographic forecasting is a precursor to rational transportation planning and decision activities. This paper provides population projections into 2060 at the county level for the entire US using cohort-component methods. We found that the US is projected to experience population growth across all age groups over the next 50 years. The projected growth, however, varies across the entire US. Population growth areas are in the west/south/east border states and the Atlanta-NC-Nashville triangle. Population decline areas include many counties from the northeast corner to the Appalachian region, counties bordering the five Great Lakes, counties along the Mississippi River, the Deep South states, and Alaska. We also identified the counties that may need additional or less transport capacity based on projected population change and proximities to IHS. We further identified the counties with high population density but without the IHS network that are projected to experience rapid population growth. IHS demands will be affected by the aging population, the young population, baby boomers, millennials, immigrants, telecommuting, and autonomous vehicles. This study provides the necessary demographic forecasting for decision and policy makers in better deciding whether and where to expand or invest in the IHS when the resources become available.Introduction In late December 2019, reports emerged from Wuhan, China of a novel corovonavirus SARS-CoV-2, which caused severe acute respiratory distress syndrome referred to as COVID-19. As the virus spread, reports of severe perioperative complications, including fatalities, began to emerge in the literature. We present a case of a previously healthy patient who developed classic symptoms of appendicitis. The patient was also found to be positive for COVID-19. MSDC-0160 cost Given the risks to both the patient and surgical team, we elected to pursue a non-operative management strategy for this patient with appendicitis. Materials and methods A 13 year old female with COVID-19 presented with a day of right lower quadrant abdominal pain. A computerized tomography (CT) scan diagnosed uncomplicated appendicitis. The patient was successfully treated non-operatively with antibiotics and discharged home. Conclusion To our knowledge, this case illustrates the first report of a pediatric patient with concomitant appendicitis and COVID-19 infection. We have been able to utilize a non-operative management strategy to effectively treat the patient's acute appendicitis, while protecting her from the risks of undergoing a general anesthetic as well as the operative team. We hope this report can provide others with a potential management strategy for similar patients.
