The community must also continue to address the reality that Black patients receive less respectful care compared with White patients.The gold(I)-dithiocarbamate (dtc) complex [Au(N,N-diethyl)dtc]2 was identified as the active cytotoxic agent in the combination treatment of sodium aurothiomalate and disulfiram on a panel of cancer cell lines. In addition to demonstrating pronounced differential cytotoxicity to these cell lines, the gold complex showed no cross-resistance in therapy-surviving cancer cells. In the course of a medicinal chemistry campaign on this class of poorly soluble gold(I)-dtc complexes, >35 derivatives were synthesized and X-ray crystallography was used to examine structural aspects of the dtc moiety. A group of hydroxy-substituted complexes has an improved solubility profile, and it was found that these complexes form 2  1 host-guest inclusion complexes with β-cyclodextrin (CD), exhibiting a rarely observed "tail-to-tail" arrangement of the CD cones. Formulation of a hydroxy-substituted gold(I)-dtc complex with excess sulfobutylether-β-CD prevents the induction of mitochondrial reactive oxygen species, which is a major burden in the development of metallodrugs.The dialkyl malonate derived 1,3-diphosphines R2 C(CH2 PPh2 )2 (R=a, Me; b, Et; c, n-Bu; d, n-Dec; e, Bn; f, p-tolCH2 ) are combined with (p-tol3 P)2 PtCl2 or trans-(p-tol3 P)2 Pt((C≡C)2 H)2 to give the chelates cis-(R2 C(CH2 PPh2 )2 )PtCl2 (2 a-f, 94-69 %) or cis-(R2 C(CH2 PPh2 )2 )Pt((C≡C)2 H)2 (3 a-f, 97-54 %). Complexes 3 a-d are also available from 2 a-d and excess 1,3-butadiyne in the presence of CuI (cat.) and excess HNEt2 (87-65 %). Under similar conditions, 2 and 3 react to give the title compounds [(R2 C(CH2 PPh2 )2 )[Pt(C≡C)2 ]4 (4 a-f; 89-14 % (64 % avg)), from which ammonium salts such as the co-product [H2 NEt2 ]+ Cl- are challenging to remove. Crystal structures of 4 a,b show skew rhombus as opposed to square Pt4 geometries. The NMR and IR properties of 4 a-f are similar to those of mono- or diplatinum model compounds. However, cyclic voltammetry gives only irreversible oxidations. As compared to mono-platinum or Pt(C≡C)2 Pt species, the UV-visible spectra show much more intense and red-shifted bands. Time dependent DFT calculations define the transitions and principal orbitals involved. Electrostatic potential surface maps reveal strongly negative Pt4 C16 cores that likely facilitate ammonium cation binding. Analogous electronic properties of Pt3 C12 and Pt5 C20 homologs and selected equilibria are explored computationally.
Desoxycorticosterone pivalate (DOCP) is a commonly used mineralocorticoid replacement for dogs with primary hypoadrenocorticism (HA), but manufacturer-recommended dosing protocols can be cost-prohibitive. Recent reports also have raised concerns that label dose protocols could be excessive.

To investigate the relative efficacy and adverse effects of 2 DOCP dosages in dogs with primary glucocorticoid and mineralocorticoid deficient HA.

Thirty-seven dogs, including 19 test population dogs and 18 controls.

Randomized controlled double-blinded clinical trial. Dogs with newly diagnosed primary HA were assigned to standard (2.2mg/kg q30d, control population) or low-dose (1.1mg/kg q30d, test population) DOCP treatment. Clinical and laboratory variables were assessed 10 to 14 days and approximately 30 days after each DOCP treatment for 90 days.

Mean serum sodium to potassium ratios at reevaluations were ≥32 in both populations throughout the study. No dog developed electrolyte abnormalities warranting medical treatment, although hypokalemia occurred on at least 1 occasion in 9 controls and 6 test population dogs. Urine specific gravities (median, interquartile range) were lower in control dogs (1.022, 1.016-1.029) as compared to test population dogs (1.033, 1.023-1.039; P = .006). Plasma renin activity was overly suppressed on 84 of 104 (80.8%) assessments in control dogs whereas increased renin activity occurred on 23 of 112 (20.5%) assessments in test population dogs.

Low-dose DOCP protocols appear to be safe and effective for treatment of HA in most dogs. Standard-dose protocols are more likely to result in biochemical evidence of overtreatment.

We report a case of IE secondary to thrombophlebitis in the left vertebral vein following pharyngitis (LS variant) with distinctively severe manifestations, including metastatic infection and severe neurological impairment with multiple cerebral infarctions. A pedunculated abscess was noted on the left ventricular free wall. Despite the patient's highly impaired consciousness level (i.e., comatose state), we performed early surgery to remove the abscess after excluding LS-related brain complications. Preoperative antibiotics included clindamycin to cover LS-related anaerobic bacteria, and thrombophlebitis required postoperative anticoagulation. By managing LS as well as IE, the infection was controlled, and the neurological status normalized. This report provides insights into the perioperative management of IE secondary to LS.
