METHODS Using Danish nationwide registries, patients undergoing left-sided heart valve surgery in the course of an IE hospitalization (1996-2017) were identified and matched with controls undergoing left-sided heart valve surgery due to another cause than IE in a 11 ratio. Patients were stratified according to type of surgical valve intervention and affected valve. The comparative risk of recurrent vs. first-time IE was assessed by cumulative incidence curves and multivariable Cox regression analyses. RESULTS The study population comprised 971 patients with a first-time admission for IE requiring heart valve surgery matched with 971 controls undergoing heart valve surgery due to other causes than IE. The risk of recurrent IE was significantly higher than the risk of first-time IE following heart valve surgery (5.5% and 3.0% by 10 years, hazard ratio (HR) 1.66, 95% confidence interval (CI) 1.02-2.70). The risk of IE recurrence was not significantly different comparing valve replacement and valve repair (5.5% and 5.3%, respectively, HR 1.60, 95% CI 0.71-3.60). Yet, the risk of IE recurrence was significantly higher among patients with biological versus mechanical prostheses (6.3% and 4.6%, respectively, HR 2.00, 95% CI 1.02-3.70). CONCLUSIONS Following heart valve surgery, the risk of recurrent IE was significantly higher than the risk of first-time IE.Atherosclerotic cardiovascular disease (ASCVD) is a prime example of a systems disease. In this chapter, we review the current state of knowledge of NAFLD and underline the striking similarities with atherosclerosis. We first describe current epidemiological data showing the staggering increase of NAFLD numbers and its related clinical and economic costs. We then provide an overview of pathophysiological hepatic processes in NAFLD and highlight the systemic aspects of NAFLD that point toward metabolic crosstalk between organs as an important cause of metabolic disease. Finally, we end by highlighting the currently investigated therapeutic approaches for NAFLD, which also show strong similarities with a range of treatment options for atherosclerosis.PURPOSE We explored the association of vestibular drop attacks (VDA) with or without migraine in patients with Ménière's disease (MD) and compared with patients classified as vestibular migraine (VM). METHOD The study involved a cross-sectional survey design. Data were collected from 401 members of Finnish Ménière Federation who experienced VDA. RESULTS In the sample of 401 patients with VDA who did not experience headache, VM was diagnosed among 16.4% participants and migraine not associated with vertigo was experienced among 25.2% of the participants. Severity of postural instability, ability to move, and ability to stand up from chair differed among VDA baseline, VDA migraine and VM patient groups. Syncope associated with VDA was noticed more frequently in VM group. In terms of neurological complaints, except for visual problems, all other symptoms differed significantly between three patient groups. The health-related quality of life also differed between groups with VM group having the worse scores. CONCLUSIONS The current study suggests that migraine is common in MD patients who experience VDA.
