Compared to the fresh egg group, the bacterial produced SCFAs of preserved egg group were increased in abundance (p less then 0.05), which may have potential anti-obesity and anti-inflammatory effects. The results provide a novel insight into the relationship between preserved egg intake, gut microbiota and potential positive effects on host health. Macrolactins (MLNs) have attracted considerable attention due to their antibacterial and antiviral properties. Here, the MLN production of Bacillus sp. strain IMDGX0108 was improved using a breeding strategy of atmospheric room temperature plasma (ARTP) technique. Combining with a selection procedure based on the colony morphology and specific growth rate index (SGRI), two genetically stable mutants A29 and A72 were identified. The MLN production of A29 and A72 was 35.2% and 52.8% greater than that of the parent strain, respectively. The best-performing mutant A72 was subjected to RNA-sequence analysis. Five pathways were significantly enriched, and fatty acid bioprocesses might play an important role in improving the production of MLNs. The combined strategy developed herein (i.e., ARTP mutation plus an efficient screening procedure) might be an appropriate method by which to obtain strains overproducing MLNs. Fractures of the mandibular angle have been well-described and, in most societies, their incidence is decreasing. In this study we analysed the stabilisation of fractures using a single plate (standard or optimised model). The finite element model was developed based on a mandibular computed tomographic scan, together with a miniplate from DePuy Synthes and an optimised plate. Using the finite element model we looked in turn at the four screws for fixation of the standard plate, and the six screws for the optimised plate, in a complete and an incomplete favourable fracture of the mandibular angle, using two screw diameters, 1.5 and 2 mm. The results indicated that a complete fracture is critical, with 10% more strain at the bone holes. The maximum microstrain was found for the 1.5mm diameter, in screws number 2 and 4, with 7270με and 6872με in the complete fractures, respectively. There were similar microstrains in screws number 1 and 2 of the optimised plate with six screws showing similar strains. Micromovements in the fracture line achieved 60με. The position of the screws influences the microstrains along the fracture line, suggesting that the surgeon places the screws along that line at a distance of 2.5 times the diameter of the screw. The optimised geometry with more screws does not prevent screws from loosening. BACKGROUND S. pneumoniae carriage by children is a major source of pneumococcal transmission, and the initial step prior to infection. Pilus type 1, reported in ~30% of pneumococcal strains in the pre-vaccine era, contributes to pneumococcal colonization and virulence. In this study, we report the impact of the pneumococcal conjugate vaccine (PCV), PCV7/PCV13 sequential implementation on serotype distribution, and on the prevalence of piliated strains among carried pneumococci during the pre- and post-vaccine eras. METHODS During 2002-2016, 12 repeated cross-sectional surveillances of nasopharyngeal S. pneumoniae carriage were conducted among 8,473 children less then 5.5 years old visiting primary care physicians in Central Israel. Seven biannual surveillances in the pre-PCV period, 2 surveillances after PCV7 was licensed but before implementation in the National Immunization Plan, and 3 additional surveillances in the post-PCV period. S. pneumoniae serotype distribution and prevalence of piliated strains weains. This suggests that the pilus confers an advantage in colonization. OBJECTIVE Voice is an essential working tool in the teaching profession and thus needs to be acknowledged and cared for by student teachers. This study aimed to investigate the presence of vocal symptoms among fourth-year students of teacher education programs and whether students had received voice care advice during their education. METHODS We conducted a descriptive cross-sectional study of 70 students in their final year of 9 teacher education programs at Regional Jataí of Universidade Federal de Goiás, Midwestern Brazil, from January to April 2017. Participants completed a questionnaire on sociodemographic, academic, and work-related characteristics, and were asked to answer an open-ended question on whether they had received any voice care advice during their education (and what type of advice if any). The Screening Index for Voice Disorder was then administered to all participants for self-reported vocal symptoms. RESULTS Of 70 students included in the study, 81.4% were women with a mean age of 30 years. Overall, 62.9% were employed 47.7% in the field of education and 52.3% in other activities. As for vocal symptoms, 35.7% reported ≥ 5 symptoms, including strained speech (51.4%), dry throat (50.0%), dry cough (48.6%), hoarseness (37.1%), and phlegm (35.7%). Most students (87.14%) did not receive voice care advice during their education. Among those who received advice (12.86%), voice care was limited to lay advice, such as drinking plenty of water, speaking in a lower voice, avoiding straining the voice by shouting, avoiding cold beverages, and drinking ginger tea. CONCLUSION Our findings highlight the need for the introduction of voice training during teacher education in order to promote efficient voice production and prevent occupational voice disorders among future teachers by raising awareness of the importance of voice care and the risk factors that may affect the voice directly or indirectly. BACKGROUND Various designs of total knee arthroplasty (TKA) have provided satisfactory outcomes for the treatment of knee osteoarthritis for many years. The aim of the study is to evaluate the success and failure rate of the medial stabilized (MS) TKA design through national joint registries and the current literature. MATERIALS AND METHODS A comprehensive literature review using the Preferred Reporting Items for Systematic Reviews and Meta-Analyses was carried out through PubMed and MEDLINE database. The international registries analyzed included the National Joint Registry, the Australian Orthopedic Association National Joint Replacement Registry, the Dutch Arthroplasty Register, the New Zealand Orthopedic Association Joint Registry, and the Michigan Arthroplasty Registry Collaborative Quality Initiative. We are reporting descriptive data which include means and percentages of survival rates and revision rates and number of years after the primary procedure. The revision rates and the cumulative revision rasion rate of 2.28% at 3 years. CONCLUSION The MS TKA design has comparable results to traditional TKA designs across several joint registries and 25 studies in the literature. INTRODUCTION AND AIMS Helicobacter pylori (H. pylori) is associated with a higher risk of peptic ulcer and gastric cancer. The sole presence of the bacterium is not a determinant of clinical outcome, but rather the interaction of strain type and host factors determines the risk of disease. Our aim was to study the association between bacterial load, strain type, and gastric symptoms in H. pylori-positive subjects. MATERIALS AND METHODS In a community survey, a diagnostic 13C-urease breath test for H. pylori was performed on 302 volunteers that were not taking antibiotics, antacids, or proton pump inhibitors one month prior to the test. The breath test produced 25 H. pylori-positive subjects, between 25-74 years of age, who then took a gastric symptoms survey and were tested for the presence of the cagA genotype in gastric juice, using the Entero-test®. Bacterial load was determined as a measure of urease activity, utilizing the delta over baseline value, obtained in the 13C-urea breath test. RESULTS A total of 48% of the H. pylori-positive subjects were cagA+. A positive association was found between cagA status and high gastric urease activity (P less then .0001) and the latter was significantly associated with the presence of symptoms (P less then .0001). CONCLUSION Gastric urease activity was strongly associated with dyspeptic symptoms and cagA+ H. pylori. Elevated 13C-delta over baseline values could be used as indicators of a higher risk for gastric disease. Isolated traumatic maxillofacial injury without concomitant brain injury may cause delayed post-concussive symptoms. Early identification allows optimal diagnosis, prognostication, and therapeutic intervention. The aim of this prospective observational study was to investigate longitudinal microstructural changes of the white matter (WM) tracts based on diffusion tensor imaging (DTI) indices in patients with isolated maxillofacial injuries, immediately and 6 months post-trauma, and to correlate these DTI indices with neuropsychological changes observed. Twenty-one patients with isolated maxillofacial injuries and 21 age-matched controls were recruited. DTI was performed and indices were calculated for 50 WM tracts. The neuropsychological evaluation was done using the screening module of the Neuropsychological Assessment Battery. PURPOSE To establish an appropriate clinical protocol for early photoscreening in 12-to-24-month-old children at pediatric well-baby clinics. METHODS This prospective study included a total of 277 children aged 12-24 months who visited a pediatric well-baby clinic. All participants underwent 1) inquiry of medical history; 2) photoscreening with PlusoptiX A12; and 3) comprehensive ophthalmologic examinations. The optimal referral cut-off point for PlusoptiX was determined by receiver operating characteristic analyses. A high-risk subgroup was defined as having a birth weight less then 3000 g or a history of major systemic diseases, based on the results of multivariable risk factor analysis from children's medical history. A strategy of selective photoscreening focusing on the high-risk subgroup was evaluated. The main outcome measures included sensitivity, specificity, and positive and negative predictive values. RESULTS The prevalence of amblyopia risk factors in our study population was 12.3%. A total of 172 children (62.1%) were assigned to the high-risk subgroup. Compared with the nonselective photoscreening of all 277 children, selective photoscreening of the high-risk children yielded a higher positive predictive value (59.6% vs. 46.7%, p = 0.001) while showing no difference in sensitivity (82.3% vs. 85.3%, p = 0.32), specificity (92.2% vs, 86.4%, p = 0.05), and negative predictive value (97.4% vs. 97.6%, p = 0.50). CONCLUSION At pediatric well-baby clinics, selective photoscreening among 12-to-24-month-old children with a birth weight less then 3000 g or a history of major systemic diseases helped reducing the number of children need to be screened and conserving medical resources yet identifying children at risk for timely eye care.
