How to Diagnose ADHD
Clinicians use clinical interviews and behavior ratings scales, checklists for symptoms filled out by the patient and other people in their lives along with medical history and psychiatric histories to diagnose ADHD. Psychotherapy may also be used to manage symptoms.
To be diagnosed with ADHD you must exhibit at least six symptoms that started before age 12 and affect your daily activities in two environments (home and school, and social situations). It is crucial to receive the correct diagnosis prior to choosing a treatment plan.
History of Symptoms
When a patient decides that they'd like to consult a specialist for ADHD, the doctor will examine their symptoms and determine if they meet the diagnostic criteria. These are listed in Diagnostic and Statistical Manual of Mental Disorders Fifth Edition (DSM-5) published by the American Psychiatric Association. To be diagnosed, the symptoms must be present for at minimum six months. The person must also have at least five continuous ADHD symptoms and the symptoms should hinder functioning well in two or more settings--for instance, at school and at work. In addition, the person's ADHD symptoms can't be better explained by another mental health condition.
The first step is for the doctor to interview the person about their symptoms and how they impact their daily life. They'll also inquire about family history and a person's overall medical and psychiatric history.
People with symptoms of ADHD frequently have difficulty managing or completing tasks. They might forget appointments or make careless mistakes at work. They are easily distracted during lectures or conversations by phone calls or television. They might be restless or fidgety or talk too much and disrupt others, or have difficulty waiting for their turn at games or other activities. Adults with ADHD are more likely to struggle with making decisions and executing on them, as well as being unable to keep their hands off of things they should not be doing.
If ADHD symptoms aren't treated they could result in serious consequences such as traffic violations and accidents. They can also impact relationships, and can be difficult to live with. A diagnosis can help a person to take control of their life and manage their symptoms with medication, lifestyle changes or therapy.
However the stigma that surrounds ADHD can prevent people from seeking treatment or an evaluation. People suffering from ADHD are often accused of being lazy or unwilling to try. A lack of child psychiatrists as well as adolescents psychosis puts them at risk of being omitted by employers. They also face challenges in obtaining psychiatric drugs, including stigma, fear of side effects, and barriers due to the absence of psychiatrists.
Physical Examination
It is essential to find a mental health professional or a doctor who has experience in diagnosing ADHD. The person you choose could be a psychologist, psychiatrist or a family doctor. A thorough evaluation should include an assessment of the patient's clinical condition as well as diagnostic tests and questionnaires. The examiner could also inquire about the patient's medical history, his or her behavior and development from the time of his or her childhood. The examiner can often benefit from speaking to people who know the patient, like siblings, spouses or parents for adults; coaches, teachers religious leaders, Scout leaders in the cases of children, in order to obtain information on their observations.
A physical examination is necessary to rule out health issues which could trigger symptoms that appear similar to ADHD, such as hearing and vision problems, thyroid disease or sleep disorders. A X-ray and other tests in the laboratory may be needed as well.
The most valuable information is derived from what the patient says about their symptoms and how these symptoms have affected them. It is important that the doctor listens to the words of the patient. Rushing through an ADHD evaluation could result in a wrong diagnosis and leaving out crucial details.
Most experts agree that a diagnosis of ADHD can't be made for an adult unless the signs are present in childhood as well. Adults don't "grow out" of ADHD however, they can be affected by an alteration in lifestyle or a new job. A person may have difficulty meeting the expectations of an employment that requires frequent meetings.
It is a good idea for a child to see a specialist as soon as problems begin to develop. This will increase the likelihood that a problem will be identified, treated quickly and reduce the risk of a misdiagnosis that could have devastating long-term effects. Parents can ask their school's principal or counselor to send them to a health care professional who is able to assess ADHD. Adults may consult their family and friends for recommendations or contact a local group to get names of professionals who are qualified. You can also consult your primary care physician or the psychiatric facility for an inventory.
Psychological Tests
Psychological testing is a vital element of diagnosing ADHD. It can confirm the diagnosis and provide an extensive analysis of the symptoms and their impact on a person's daily activities. Tests can also reveal other mental health issues that may be a part of ADHD, such as anxiety or depression. It can also help inform the need for interventions and accommodations.
For children, there are a variety of psychological tests that can be used to test for ADHD. These include rating scales, direct behavioral observations, and neuropsychological tests. Interviews with teachers and parents are also conducted to obtain an overall picture of the child.
Finding out if you have ADHD for adults is more difficult. The DSM-V symptoms guide is actually intended to help diagnose children, and the best way to make an accurate diagnosis for adults is to conduct an extensive interview conducted by an expert in ADHD who is able to assess the patient's history. Some healthcare practitioners also use computers to assess the level of impulsivity and attention, and brain scans are often used as well.
It can take a variety of time to finish a comprehensive evaluation, but you can anticipate spending a full day in the clinic or hospital to receive a neuropsychological evaluation. This is a small amount of time for clear, well-informed answers and a path forward that can improve the quality of life. Patients are often overwhelmed by their results, especially if they've been living with undiagnosed ADHD for a long period of time or for a long time.
There are online ADHD self-assessments that claim to provide a precise scientifically-validated diagnosis of your symptoms. These assessments are not endorsed by a majority of mental health experts because they do not take into consideration the complexity of psychiatric disorders and how their symptoms affect a person's functioning in their daily lives. A licensed healthcare professional who adheres to the DSM guidelines can only give a formal ADHD diagnosis. You can locate a qualified healthcare professional through your family physician, or request referrals from friends and colleagues who have had experience with ADHD.
Family History
Currently, under DSM-5 guidelines, a child must demonstrate symptoms of inattention and/or hyperactivity/impulsivity in two or more settings, including home and school, before a diagnosis can be made. Adults, however, can be diagnosed when they meet the criteria in a single setting.
Interviewing the patient is done by a health care professional who is conducting the examination. She will ask questions regarding the symptoms and their impact on the daily activities. She will ask about the person's medical history and background. She may utilize a checklist, or a standard rating system, to determine if the signs are ADHD. She will also ask for information from various sources, such as teachers, coaches and Scout and religious leaders, as well as family members and friends who spend a significant time with the child being assessed.
A complete psychiatric history should include the patient's account of symptoms, the completion of questionnaires and rating scales by him and his family and a review on the person's upbringing, education and environment. A medical professional or psychiatrist may also test to determine whether a patient's symptoms are caused by depression or anxiety.
The DSM-5 symptom manual is targeted towards children, making it difficult to diagnose ADHD in adults. A diagnosis will often emerge only through a thorough clinical interview, conducted by a health care specialist who is specialized in ADHD. adhd in adults undiagnosed will inquire with the patient and those who know him well -- such as the spouse or a close friend -- about the existence of ADHD-related problems. She will also ask about the history of other conditions that could affect concentration, such as depression or drug abuse.
To locate a health care professional who is qualified to perform an ADHD evaluation, ask your primary care physician for an appointment. You can also inquire with the local ADHD support group or a mental health organization to get recommendations. A lot of insurance plans have specialists listed by specialization, and you can check with your insurer to find out whether there are ADHD specialists in your region.
