Molecular detection of JAK2 mutation (V617F or exon 12) is included as a major diagnostic criterion for polycythemia vera (PV) by the WHO 2016 guidelines. JAK2 exon 12 mutations are seen in about 2-5% of JAK2V617F-negative cases of PV. Mutations in JAK2 cause constitutive activation of JAK-STAT pathway which results in variable phenotypes. PV patients with exon 12 mutations in JAK2 present characteristically with erythrocytosis. There are limited reports describing the spectrum of JAK2 exon12 mutations in myeloproliferative neoplasms (MPNs). Here, we describe the characteristics of a series of MPN patients with mutations in exon 12 of JAK2 of which two were novel variants associated with polycythemia. Interestingly, we noted two patients presenting as myelofibrosis having JAK2 exon 12 mutations.This is a single-center report on coordination of staff and handling of patients during the outbreak of the COVID-19 (coronavirus disease 2019) in a region with high incidence and prevalence of disease. The selection of procedures for interventional radiology (IR), preparation of staff and interventional suite before the arrival of patients, the facility ventilation systems and intra- and post-procedural workflow optimization are described. The control measures described may increase the cost of the equipment, prolong procedural times and increase technical difficulties. However, these precautions may help control the spread of COVID-19 within the healthcare facility.PURPOSE There is conflicting advice about the inclusion of dairy foods in a lower greenhouse gas (GHG) emission dietary pattern. Our purpose was to assess the prevalence of dairy food intake among higher diet quality and lower GHG emission diets in Australia and within these diets assess the association between level of dairy food intake and adequate intake of a broad range of nutrients. METHODS Dietary intake data collected using a 24-h recall process were sourced from the most recent Australian Health Survey. Diet quality was assessed by level of compliance with the food group-based Australian Dietary Guidelines. A subgroup of 1732 adult (19 years and above) daily diets was identified having higher diet quality score and lower GHG emissions (HQLE). Intake of core dairy foods (milk, cheese, yoghurt) was assessed and nutrient profiling was undertaken for 42 macro- and micronutrients. RESULTS The HQLE subgroup had 37% higher diet quality score and 43% lower GHG emissions than the average Australian adult diet (P less then 0.05). Intake of dairy foods was very common (90% of HQLE diets) and greatly exceeded the intake of non-dairy alternatives (1.53 serves compared to 0.04 serves). HQLE daily diets in the highest tertile of dairy food intake were more likely to achieve the recommended intake of a wide range of nutrients, including calcium, protein, riboflavin, vitamin B12, folate, phosphorous, magnesium, iodine and potassium compared to other HQLE daily diets. CONCLUSION Core dairy foods have an important role for achieving adequate nutrient intakes in a healthy and lower GHG emission dietary pattern in Australia.PURPOSE Reducing postprandial hyperglycemia has beneficial effects on diabetes-related risk factors, but the magnitude of the reduction needed to achieve such an effect is unknown. The purpose of the study was to quantify the relationship of acute glucose and insulin postprandial responses with longer-term effects on diabetes-related risk factors by performing a systematic review and meta-analysis of dietary intervention studies. METHODS We systematically searched EMBASE and MEDLINE. Dietary intervention studies among any human population aiming to reduce postprandial glycemia, with actual measures of postprandial glucose (PPG) and/or insulin (PPI) as acute exposures (incremental area under the curve, iAUC) as well as markers of glucose metabolism (fasting glucose, HbA1c) and insulin sensitivity (fasting insulin, HOMA-IR) after at least 4 weeks of diet intervention as outcomes were included. For the diagnosis of LAM trauma ultrasound evaluation is advisable. As ultrasound equipment and trained personnel are not available everywhere at all times, we aimed to evaluate whether specific overt birth tears are an indicator for LAM trauma. METHODS In a prospective cohort study at our center from March 2017-April 2019, we evaluated vaginal births of nulliparous women with singletons in vertex presentation ≥ 36 + 0 gestational weeks for LAM trauma by translabial ultrasound and for overt birth tears by inspection. EGFR inhibitor We then calculated the association of overt birth tears with complete LAM avulsion. RESULTS Of 213 women, 23.9% had any kind of LAM trauma, with 14.1% being complete avulsions. In univariate analysis, solely high-grade perineal tears (OASIS) were significantly associated with complete LAM avulsions. CONCLUSIONS Fourteen percent of women suffered a complete LAM avulsion after vaginal birth, with OASIS being the only associated parameter of significance. The occurrence of such trauma might be an indicator for a mismatch between the size of the fetus and the structures of the birth canal, leading to birth trauma. EGFR inhibitor Assessing for LAM trauma by translabial ultrasound in women with OASIS might be worthwhile.INTRODUCTION AND HYPOTHESIS Pelvic organ mobility is defined as the displacement of pelvic organs between rest and maximal straining. We hypothesized that pelvic organ mobility after vaginal sacrospinous hysteropexy (SSHP) might be increased compared with other surgeries for uterine descent, which may contribute to the high occurrence of postoperative cystocele after this surgery. Pelvic organ mobility and the vaginal axes after SSHP are compared with other surgical procedures for uterine descent vaginal hysterectomy with uterosacral suspension (VH) and laparoscopic sacrohysteropexy (LSH). METHODS In this prospective pilot study, 15 women were included (5 for each procedure). Six months postoperatively, POP-Q examination and dynamic MRI were performed and questionnaires were filled out regarding prolapse complaints. Pelvic organ mobility on MRI was defined as vertical displacement of pelvic organs at rest and maximal straining. The displacements and angles were measured using an image registration method. Furthermore, the angle of displacement of cervix/vaginal vault and vaginal axes were assessed.
