Information on the long-term outcome of MIS-C is limited. This study was conducted to describe the long-term outcome of MIS-C from a tertiary care center in North India. Children admitted with MIS-C from September 2020 to January 2021 were followed up after discharge until June 2021. The details during the acute phase (clinical features, investigations, treatment, and outcome) and follow-up (symptoms, echocardiographic findings, ongoing treatment, and outcome) were collected retrospectively. During the acute phase, 40 children presented at median (interquartile range [IQR]) age of 7 (5-10) years with fever, mucocutaneous, gastrointestinal, and respiratory symptoms. The majority (66.7%) of the children had positive SARS-CoV-2 serology and elevated inflammatory markers (C-reactive protein, procalcitonin, ferritin, D-dimer, and fibrinogen), lymphopein the majority of children during follow-up.
Molecular tests predicting the risk of distant recurrence (DR) can be used to assist therapy decision-making in oestrogen receptor-positive (ER+) and human epidermal growth factor receptor 2-negative (HER2-) breast cancer patients after considerations of standard clinical markers. The Oncotype DX Recurrence Score (RS) is a widespread tool used for this purpose. Here, we compared the RS with the StemPrintER Risk Score (SPRS), a novel genomic predictor with a unique biological basis in its ability to measure the expression of cancer stemness genes.

We benchmarked the SPRS vs. RS, alone or in combination with clinicopathological variables expressed by the Clinical Treatment Score (CTS), for the prognostication of DR in a retrospective cohort of 776 postmenopausal patients with ER+/HER2-breast cancer enrolled in the translational arm of the randomised Arimidex, Tamoxifen, Alone or in Combination (ATAC) trial. Likelihood ratio (LR) with χ
test and C-index were used to assess prognostic performance for the en, SPRS provided more prognostic information than RS for DR when used alone or in combination with the CTS. The SPRS could therefore potentially identify high-risk patients, who might benefit from aggressive treatments, from low-risk patients who might safely avoid adjuvant chemotherapy or prolongation of endocrine therapy.
Biomarkers for cyclin-dependent kinase 4/6 inhibitors, such as palbociclib, for patients with hormone receptor-positive/HER2-negative metastatic breast cancerare lacking. Thymidine kinase is a proliferation marker downstream of the cyclin-dependent kinase 4/6 pathway. We prospectively investigated the prognostic role of serum thymidine kinase activity (sTKa), in patients treated with Palbociclib+fulvestrant.

PYTHIA was a phase II, single-arm, multicentre, trial that enrolled 124 post-menopausal women with endocrine-resistant hormone receptor-positive/HER2-negative metastatic breast cancer. Serum samples were collected pre-treatment (pre-trt; n=122), at day 15 of cycle 1 (D15; n=108), during the one week-off palbociclib before initiating cycle 2 (D28; n=108) and at end of treatment (n=76). sTKa was determined centrally using Divitum®, a refined ELISA-based assay with a limit of detectionof 20 Divitum Units (Du)/L. The primary study endpoint was progression-free survival, assessed for its association with pre- and on-treatment sTKa.

Data from 122 women were analysed. Pre-treatment sTKa was not associated with clinical characteristicsand moderately correlated with tissue Ki-67. Palbociclib+fulvestrant markedly suppressed sTKa levels at D15, with 83% of patients recording levels below limit of detection. At D28, sTKa showed a rebound in 60% of patients. At each timepoint, higher sTKa was associated with shorter progression-free survival (each p<0.001), with the strongest effect at D15.

STKa is an independent prognostic biomarker in patients treated with palbociclib. High pre-treatment sTKa and its incomplete suppression during treatment may identify patients with poorer prognosis and primary resistance. This warrants validation in prospective comparative trials. CLINICALTRIALS.

NCT02536742; EudraCT 2014-005387-15.Waste sorting is highly recommended for municipal solid waste (MSW) management. Increasingly, computer vision (CV), robotics, and other smart technologies are used for MSW sorting. Particularly, the field of CV-enabled waste sorting is experiencing an unprecedented explosion of academic research. However, little attention has been paid to understanding its evolvement path, status quo, and prospects and challenges ahead. To address the knowledge gap, this paper provides a critical review of academic research that focuses on CV-enabled MSW sorting. Prevalent CV algorithms, in particular their technical rationales and prediction performance, are introduced and compared. The distribution of academic research outputs is also examined from the aspects of waste sources, task objectives, application domains, and dataset accessibility. The review discovers a trend of shifting from traditional machine learning to deep learning algorithms. The robustness of CV for waste sorting is increasingly enhanced owing to the improved computation powers and algorithms. Academic studies were unevenly distributed in different sectors such as household, commerce and institution, and construction. Too often, researchers reported some preliminary studies using simplified environments and artificially collected data. Future research efforts are encouraged to consider the complexities of real-world scenarios and implement CV in industrial waste sorting practice. This paper also calls for open sharing of waste image datasets for interested researchers to train and evaluate their CV algorithms.
Diabetic retinopathy (DR) is a leading cause of blindness in the working-age population, which is characterized by retinal neurodegeneration and vascular dysfunction. Long non-coding RNAs (LncRNAs) have emerged as critical regulators in several biological processes and disease progression. Here we investigated the role of lncRNA AQP4-AS1 in retinal neurovascular dysfunction induced by diabetes.

Quantitative RT-PCR was used to detect the AQP4-AS1 expression pattern upon diabetes mellitus-related stresses. Visual electrophysiology examination, TUNEL staining, Evans blue staining, retinal trypsin digestion and immunofluorescent staining were conducted to detect the role of AQP4-AS1 in retinal neurovascular dysfunction in vivo. MTT assays, TUNEL staining, PI/Calcein-AM staining, EdU incorporation assay transwell assay and tube formation were conducted to detect the role of AQP4-AS1 in retinal cells function in vitro. qRT-PCR, western blot and in vivo studies were conducted to reveal the mechanism of AQP4-AS1-ect Fund of Nanjing (Grant No ZKX17053 and YKK19158), grants from Innovation Team Project Fund of Jiangsu Province (No. CXTDB2017010), and the Science and Technology Development Plan Project Fund of Nanjing (Grant No 201716007, 201805007 and 201803058).

Also, in India, with its rampant increase in crime rates, the greatest promise of forensic nursing is the emergence of a new holistic approach to legal issues surrounding patient care in the hospital and community setting. The Indian Nursing Council has introduced forensic nursing as a subject in the graduate nursing program in the year 2021. The inclusion of forensic nursing as a subject in the nursing curricula introduces the students to a wider role in contemporary nursing practice that contributes to our own and to public health safety. find protocol The implementation of graduate and postgraduate programs in forensic nursing further reduces the gap between health care systems and judiciary systems. The knowledge and practice-related realms of forensic nursing are still in the infancy stage among nursing aspirants of India. The integration of forensic nursing in the existing forensic medical science might be more gainful to improve professionalism. Though forensic nursing has a broad scope, there are certain issues related to the feasibility, ethical and legal implications regarding the mode of service delivery of forensic nursing practice in the Indian setting which needs central attention. The article highlights and discusses the evolution, scope, promises, caveats, and future directions of forensic nursing in the Indian setting.
Parenting support policies increase the period that the mother and child spend together after childbirth, which is expected to facilitate breastfeeding. Additionally, the mother's attitudes towards parenting may influence her willingness to breastfeed. The objective of this study was to identify the association of breastfeeding duration with parenting support policies and mothers' attitudes towards parenting.

This study employed a cross-sectional design.

The study was conducted using data from South Korea's 2018 National Survey on Fertility and Family Health and Welfare.

Data of women with children born on or after January 2016 to September 2018 (N=1,861) were analysed using Cox's proportional hazard regression model with the duration of breastfeeding as the outcome variable.

The main predictors comprised social policy factors (hospital environment and parenting support) and individual factors (mothers' attitudes towards parenting). The median duration of breastfeeding was 5 months. Not initiating bf breastfeeding. Therefore, the midwives and nurses in hospitals should encourage mothers to initiate breastfeeding within 1 h of birth. In addition, the government should improve positive attitudes towards parenthood amongst young adults.
The management of the active second stage in labor and perineal protection varies between countries and is rarely described regarding waterbirths. The objective of this study was to describe how midwives manage the active second stage of labor in waterbirths compared to conventional births. A secondary aim was to compare clinical outcomes between the two groups.

A prospective cohort study, based on 323 women who gave birth at three clinics in Sweden, between Dec 2015-May 2019. The women were both primiparous and multiparous; 153 gave birth in water and 170 had a conventional uncomplicated birth. A protocol was completed by the attending midwife after birth, describing characteristics and management of the active second stage of labor as well as perineal protection.

The active second stage of labor differed in several aspects between waterbirths and conventional births. Maternal pushing was spontaneous to a higher extent among women giving birth in water and the use of manual perineal protection was lower.
