We connect LAPONITE®-nanoclay platelets with each other using carboxymethyl cellulose, a cellulose derivative, thus creating thin, flexible films with a local brick-and-mortar architecture. The dried films show a pronounced resistance against tensile forces allowing for stronger thin films than nacre. In terms of functionalities, we report excellent glass-like transparency along with exceptional shape-persistent flame shielding. We also demonstrate that through metal ion-coordination we can further strengthen the interactions between the polymers and the nanoclays, and thus enhanced mechanical, and thermal properties as well as resistance against swelling and dissolution in aqueous environments. The following natural step is the development of a tool to accurately assess the morphology of the bronchial system in all its aspects to help physicians better diagnosis and prognosis complex pulmonary diseases such as COPD, chronic bronchitis and bronchiectasis. Traditional methods for the assessment of airway morphology usually focus on lumen and wall thickness and are often limited due to resolution and artifacts of the CT image. Airway wall cartilage is an important characteristic related to airway integrity that has shown to be deteriorated during the airway disease process. In this paper, we propose the development of a Model-Based GAN Regressor (MBGR) that, thanks to a model-based GAN generator, generate synthetic airway samples with the morphological components necessary to resemble the appearance of real airways on CT at will and that simultaneously measures lumen, wall thickness, and amount of cartilage on pulmonary CT images. The method is evaluated by first computing the relative error on generated images to show that simulating the cartilage helps improve the morphological quantification of the airway structure. We then propose a cartilage index that summarizes the degree of cartilage of bronchial trees structures and perform an indirect validation with subjects with COPD. As shown by the results, the proposed approach paves the way for the use of CNNs to precisely and accurately measure small lung airways morphology, with the final goal to improve the diagnosis and prognosis of pulmonary diseases.Background and hypothesis Reverse shoulder arthroplasty (RSA) is an increasingly popular treatment modality for glenohumeral joint arthritis in association with rotator cuff arthropathy. A prolonged hospital stay following joint arthroplasty risks increased complications for patients plus financial implications for institutions. We hypothesized that RSA could be safely and effectively carried out as an outpatient procedure with reduced risks to patients and institutional costs. Methods Patients attending our institution for RSA during March 2015 to August 2018 were reviewed preoperatively for consideration for RSA as an outpatient procedure. The inclusion criteria were arthritis of the shoulder having failed conservative management, age older than 50 years, and intact deltoid muscle function. Patients were excluded if they underwent RSA for trauma or for revision following previous total shoulder replacement or hemiarthroplasty. Overall health, social circumstances, and individual wishes were considered. Results A total of 21 patients underwent RSA as an outpatient procedure. The mean age was 74 years (range, 59-84 years). There were 8 male and 13 female patients. No overnight stays were required in patients in whom outpatient surgery was planned. The Oxford Shoulder Score increased from a mean of 16 (range, 4-30) preoperatively to a mean of 31 (range, 7-35) at 6 months postoperatively; it was a mean of 36 (range, 7-48) at 12 months postoperatively. Of the patients, 88% were "very satisfied" or "satisfied" with the service and 81% would undergo the surgical procedure again as a day-case procedure. Conclusion RSA as an outpatient procedure can be carried out effectively with high patient satisfaction rates in carefully selected patients.Background A linear relationship between baseplate insertion torque and compression force in reverse shoulder arthroplasty (RSA) baseplates with central screw design has been recently established. In this study, we evaluated 3 different baseplate designs and their influence on the torque-compression relationship. Selleck GW4064 Methods Three different RSA baseplate designs were evaluated through biomechanical testing using a glenoid vault, bone surrogate model. A digital torque gauge was used to measure insertion torque applied to the baseplate, whereas compression data were collected continuously from a load cell. Additionally, 2 predictive models were developed to predict the compression forces of each baseplate design at varying levels of torque. Results A linear relationship was found between baseplate compression and insertion torque for all 3 baseplate designs. Both the monoblock and 2-piece locking designs achieved the goal torque of 6.8 Nm, whereas the 2-piece nonlocking design did not due to material strip-out. No significant difference in maximum compression was found between the monoblock and 2-piece locking designs.
