Acetaminophen (paracetamol, APAP) overdose is the principal cause of acute liver injury (ALI) that leads to liver failure typified with oxidative stress, mitochondrial and lysosomal dysfunction and with few antidotes for this condition. Therefore, more effective therapeutics are urgently required. Sinapic acid is a phenolic phytochemical with significant antioxidant, anti-inflammatory and hepatoprotective potential.

This study was conducted to evaluate hepatoprotective effect of this phytochemical in acetaminophen-induced model of ALI.

Male C57BL/6 mice were treated p.o. with sinapic acid (10 or 50mg/kg) 3 times at 72, 24, and 1h before APAP (300mg/kg; i.p.) challenge. Functional factors of liver dysfunction were determined along with hepatic assessment of oxidative stress and inflammatory indexes and histopathological analysis was also conducted. Sinapic acid (50mg/kg) properly decreased serum levels of ALT, ALP, and AST besides reducing liver level of ROS, MDA, IL-6, TNF-α, NF-kB, and MPO and improved sirtuin 1, HO-1, Nrf2, SOD activity, and MMP with no significant effect on IL-1β and catalase activity in addition to decreasing activity of lysosomal enzymes including cathepsin B and β-galactosidase. Also, sinapic acid at the higher dose ameliorated liver histopathological changes due to APAP and properly reversed NF-kB and Nrf2 immunoreactivity.

These findings show that sinapic acid pretreatment effectively protects liver against adverse and hepatotoxic effect of APAP through its antioxidant- and anti-inflammatory potential linked to NF-kB/Nrf2/HO-1 signaling and also via regulation of sirtuin 1, mitochondrial integrity, and lysosomal stabilization.

Blood is a common source of RNA for gene expression studies. However, it is known to be vulnerable to pre-analytical variables. Although RNA stabilization systems have been shown to reduce such influence, traditional EDTA tubes are still widely used since they are less expensive and enable to study specific leukocyte populations. This study aimed to assess the influence of storage time and temperature between blood sampling and handling on RNA from peripheral blood mononuclear cells (PBMCs).

Nine blood samples were collected in EDTA tubes from 10 healthy donors. One tube from each donor was immediately processed for PBMC isolation, while the others were first incubated at either 4 degrees Celsius (°C) or room temperature for 2, 4, 6 and 24h. RNA yield and quality and the expression level of fourt housekeeping (B2M, CASC3, GAPDH, HPRT1) and 8 target genes (CD14, CD19, CD20, IL10, MxA, TNF, TNFAIP3, NR4A2) were compared between samples. RNA yield, quality and integrity did not vary significantly with time and temperature. B2M was the most stable housekeeping gene, while the others were increasingly influenced by storing time, especially at 4°C. Even when normalized to B2M, the expression level of some target genes, particularly TNFAIP3 and NR4A2, was highly affected by delays in blood processing at either temperature, already from 2h.

Abdominal surgery and chemotherapy are well-established risk factors for venous thromboembolism (VTE) in patients with cancer, but their specific contribution in patients with esophageal and gastric cancer is unclear. We aim to quantify the risk of VTE, identify risk factors associated with VTE, and determine the association between VTE and survival in patients undergoing surgery for esophageal or gastric cancer.

A retrospective, population-based cohort study was conducted using linked administrative healthcare databases. We used the Ontario Cancer Registry to identify patients with esophageal or gastric cancer between January 1, 2007 and December 31, 2016 who underwent surgical resection. Incidence of first VTE event was identified using International Classification of Diseases 9 and 10 codes. VTE incidence was calculated at clinically relevant time points 180 days before and after surgery. Logistic regression was used to identify factors associated with VTE with odds ratios (OR) and 95% confidence interated with inferior OS (HR 1.36, 95% CI 1.13, 1.63) and CSS (HR 1.42, 95% CI 1.16, 1.75).

The purpose of this study was to (1) assess the effect of preoperative echocardiogram on time to surgery and (2) assess the outcomes of patients with a previous percutaneous coronary intervention (PCI).

Demographic, clinical, quality and cost data were obtained and a validated risk predictive tool (STTGMA) was calculated for each of a consecutive series of hip fracture patients. Comparative analyses of patients who had an echocardiogram prior to surgery or a PCI prior to hospitalization were performed.

Between 2014 and 2020, 2625 patients presented to our institution with a hip fracture. From this cohort 471 patients underwent a preoperative transthoracic echocardiogram (TTE), 30 who had a history of a PCI, and an additional 26 who had a history of PCI but did not undergo a preoperative TTE. A total of 76 families and their children (boys aged 6-12 years), who previously participated in a randomized controlled trial comparing THAV with an active control group, took part in the 10-month FU assessment. Outcome measures were rated by parents and included the evaluation of child aggressive behavior, prosocial behavior, problem-maintaining and problem-moderating factors, and comorbid symptoms. Linear mixed models for repeated measures (MMRM) were conducted. The results revealed that THAV effects remained stable (problem-maintaining and problem-moderating factors; comorbid symptoms) and even partially improved (aggressive behavior; ADHD symptoms) over the FU period. Additionally, the differences between the THAV intervention group and the control group, which were apparent at the end of the treatment (post), mainly also remained at the FU assessment.
