This critical literature review presents a theoretical framework for how community-based youth service organizations may enhance youth-adult partnership quality through organizational learning. The main premise is that Y-AP implementation challenges are best understood as challenges of collective learning within an organization. As such, the review integrates theory and research in organizational learning with present scholarship on Y-APs to delineate how two learning processes-intra-group and inter-group knowledge transfer-influence Y-AP quality. These learning pathways exist in dynamic interaction at different levels of the organizational hierarchy, at the point of service and beyond. The theoretical framework provides a road map for effective functioning of Y-APs in practice and an interpretive lens for descriptive and intervention research to understand and address Y-AP challenges.
Pediatric brain tumor survivors (PBTS) are at risk for both neurocognitive impairments and psychological difficulties, yet these two domains have historically been discretely examined, with assessment of psychosocial outcomes rarely included in studies of cognitive outcomes. Taking a person-centered approach, the current study aimed to more comprehensively evaluate PBTS late effect profiles, including both neurocognitive and psychological sequelae, and predictors of these profiles.
PBTS (N=89) were assessed in a pediatric neuropsychological clinic between May 2009 and May 2018, diagnosed at least 1year prior, and off-treatment for at least 3months (M
=6.57years, SD=4.53; 46.1% female). Parent- and teacher-report of psychological symptoms, and performance-based measures of neurocognitive functioning were examined using latent profile analysis. The R3STEP procedure identified predictors of class membership.
The optimal model identified four classes characterized by (1) average functioning across all meah unique patterns of relations among neurocognitive and psychological domains. These findings are a vital first step toward identifying those at highest risk for poor outcomes and informing interventions that effectively address interrelated treatment targets for specific groups.
National Comprehensive Cancer Network (NCCN) guidelines recommend biomarker testing as the first step in the management of patients with advanced non-small cell lung cancer (aNSCLC). We assessed anaplastic lymphoma kinase (ALK) testing rates and factors related to underuse in community medical systems between 2012 and 2019 to understand guideline adoption.
A retrospective observational study using a nationwide electronic health record (EHR)-derived deidentified database was conducted. Patients with aNSCLC diagnosed in community medical centers from January 2012 to May 2019 were included to describe the ALK testing trend. This cohort was further restricted to patients diagnosed after 2015 to understand factors associated with testing underuse using mixed-effects multivariable logistic regression models.
Trends for increased ALK testing rates by year were observed in both NCCN guideline-eligible patients (59.5% in 2012 to 84.1% in 2019) and -ineligible patients (15.6% to 50.8%) in a cohort of 41,728 patiessed adoption of testing recommendations for anaplastic lymphoma kinase rearrangements in a national database. Although test use increased over the time period studied (2012-2019), there is still room for improvement. Efforts are needed to increase test use in undertested groups, thus enabling eligible patients to benefit from novel lung cancer therapies.
