All rights reserved.Post-operative right coronary artery occlusion is a serious complication that demands acute coronary revascularization to prevent myocardial infarction. We present two cases with acute right coronary artery obstruction caused by (1) transfemoral aortic valve implantation and (2) acute type A aortic dissection. Although coronary artery bypass grafting was performed intraoperatively, right heart failure was observed in both cases. The Impella RP® device offers temporary right ventricular mechanical support; wherefore, we decided to deploy it in both patients. The devices were uneventfully and successfully implanted to bridge for recovery of the right heart. We report the perioperative course of the patients as well as their condition at 1 year follow-up. © 2020 The Authors. Since extrinsic probes are in many cases cell-impermeable, different biochemical, and physical approaches have been used to break the cell membrane barrier for direct delivery into the cytoplasm. In this Review, these intracellular delivery strategies are discussed, briefly explaining the mechanisms and how they are used for live-cell labeling applications. Methods that are discussed include three biochemical agents that are used for this purpose-purpose-different nanocarriers, cell penetrating peptides and the pore-foraming bacterial toxin streptolysin O. Most successful intracellular label delivery methods are, however, based on physical principles to permeabilize the membrane and include electroporation, laser-induced photoporation, micro- and nanoinjection, nanoneedles or nanostraws, microfluidics, and nanomachines. The strengths and weaknesses of each strategy are discussed with a systematic comparison provided. Finally, the extrinsic probes that are reported for intracellular labeling so-far are summarized, together with the delivery strategies that are used and their performance. This combined information should provide for a useful guide for choosing the most suitable delivery method for the desired probes. © 2020 WILEY-VCH Verlag GmbH & Co. However, the relationship between the incidence of diabetes mellitus (DM) and U-pH has not yet been fully addressed. MATERIALS AND METHODS We investigated the relationship between U-pH and the development of DM during a 10-year period in a general population of subjects who received annual health examinations in 2006 (n=28,990). After exclusion of subjects with missing data and those with DM and/or chronic kidney disease at baseline, a total of 12,476 subjects (male/female8,027/4,449) who received health examinations at least once during the period from 2007 to 2016 were recruited. The recruited subjects were divided into 4 groups according to their U-pH levels groups of U-pH ≤5.0, 5.5, 6.0 and ≥6.5. RESULTS During a 10-year period, 521 male subjects (6.5%) and 132 female subjects (3.0%) had new onset of DM. The cumulative incidence of DM was 7.5% (male/female9.3%/4.4%) per 100 person-years. The hazard ratios (HRs) in the groups of U-pH≤5.0 (HR1.93) and U-pH=5.5 (HR1.46) were significantly higher than that in the group of U-pH≥6.5 as a reference in males but not in females. After adjustment of age, obesity, fasting glucose, habits of smoking and alcohol drinking, family history of DM and use of drugs for hypertension and dyslipidemia, HR in the group of U-pH≤5.0 (HR1.39) was significantly higher than that in the group of U-pH≥6.5 in males but not in females. Recently, our institution added a phosphate dialysis solution, Phoxillum®, to our formulary as an option for CVVH fluid in addition to the bicarbonate-based Prismasol® products available. We sought to evaluate the impact of Phoxillum in patients who required CVVH when compared to Prismasol with regard to phosphate and glucose management. METHODS This was a single-center, retrospective, observational cohort analysis approved by Partners Health Care System Institutional Review Board that included patients who received a minimum of 24 hours of either Prismasol 4/2.5 or Phoxillum for CVVH from February 2017 to November 2017. Phosphate and glucose levels were monitored daily while on CVVH. Prevalence of hypoglycemia (glucose 4.3 mg/dL) were collected in terms of days of occurrence while on CVVH. Oral and intravenous phosphate repletion requirements were collected al Society for Hemodialysis.Treacher Collins syndrome (TCS OMIM 154500) is an autosomal dominant craniofacial disorder belonging to the heterogeneous group of mandibulofacial dysostoses. OBJECTIVE To investigate four Treacher Collins syndrome patients of the Sgaw Karen family living in Thailand. METHOD Clinical examination, hearing tests, lateral cephalometric analyses, Computed tomography, whole exome sequencing, and Sanger direct sequencing were performed. RESULTS All of the patients affected with Treacher Collins syndrome carried a novel TCOF1 mutation (c.4138_4142del; p.Lys1380GlufsTer12), but clinically they did not have the typical facial gestalt of Treacher Collins syndrome, which includes downward-slanting palpebral fissures, colobomas of the lower eyelids, absence of eyelashes medial to the colobomas, malformed pinnae, hypoplastic zygomatic bones, and mandibular hypoplasia. Lateral cephalometric analyses identified short anterior and posterior cranial bases, and hypoplastic maxilla and mandible. Computed tomography showed fusion of malleus and incus, sclerotic mastoid, hypoplastic middle ear space with a soft tissue remnant, dehiscence of facial nerve, and monopodial stapes. CONCLUSION Treacher Collins syndrome in Sgaw Karen patients has not been previously documented. This is the first report of monopodial stapes in a TCS patient who had aTCOF1 mutation. The absence of a common facial phenotype and/or the presence of monopodial stapes may be the effects of this novel TCOF1 mutation. This article is protected by copyright. Airway obstruction due to occluded cuffless tracheostomy tubes themselves remains unknown, although capping trials are commonly used before decannulation. The aim of this study is to evaluate the extent to which airway obstruction can be caused by occluded cuffless tubes in patients who underwent head and neck surgery. DESIGN Prospective Research Outcome. SETTINGS University teaching hospital. PARTICIPANTS Fiftypatients requiring transient tracheostomy after head and neck surgery. MAIN OUTCOME MEASURES A flow volume loop (FVL) through the mouth using a portable spirometer, with the occluded fenestrated cuffless tube, was measured before and immediately after decannulation, by obstructing the orifice of tracheostomy tube. The measurement of FVL recordedthe forced vital capacity (FVC), forced expiratory volume in 1 second(FEV1 ), peak expiratory flow (PEF), forced expiratory flow at 50%of FVC, peak inspiratory flow (PIF), forced inspiratory flow at 50% of FVC. RESULTS A statistically significant difference between all spirometric parameters was found. Mean PEF and PIF respectively increased from 2.8 to 4.5 L/s (P less then 0.0001), and 2.3 to 2.7 L/s (P less then 0.01) before and after decannulation, with a strong positive correlation (r =0.7; P less then 0.05). A mean expiratory (34%) and inspiratory (9%) airflow reductionwas observed due to cannula. CONCLUSIONS Occluded cufflesstracheostomy tubes cause a dramatic airflow obstruction, mainly in the expiratory phase of FVL. This should be taken into account during capping trials. All rights reserved.Childhood malnutrition, associated with poor diet, is a clear public health threat in Sri Lanka, with high rates of under-nutrition and micronutrient deficiencies coupled with the growing risk of overweight/obesity in urban locations. This study explored the dietary diversity and food intake of urban living Sri Lankan preschool children. A cross-sectional analysis of the baseline data from a cohort study was conducted with parents/caregivers of children aged 2-6 years, from 21 preschool centres in Kurunegala District, Sri Lanka. Demographic and socio-economic factors, dietary diversity score (DDS) (n = 597) and food intake (n = 458) (using a food frequency questionnaire) were assessed. Children had a mean DDS of 4.56 ± 0·85 out of 9, with most (91.1%) in the medium DDS category (DDS of 3.1-6.0), consuming rice as most common food. Lentils were consumed more than any meat or alternative food groups at all DDS levels. Child DDS differs with parent/caregiver age and ethnicity. Mean daily intakes of fruit (1.02) and vegetables (0.
