Results Changes in climatic factors like temperature, humidity, and precipitation support the growth, breeding, and fecundity of arthropod vectors transmitting the arboviral diseases. Increased human migration and urbanization due to socio-economic factors play an important role in population increase leading to the rapid geographical distribution of arthropod vectors and transmission of arboviral diseases. Medical factors like misdiagnosis and misclassification also contribute to the spread of arboviruses. Conclusion This review highlights two important findings First, climatic, environmental, socio-economic, and medical factors influence the constant distributions of arthropod vectors. Second, either of the three holistic approaches or a combination of any two can be adopted on arboviral disease control. Our findings underline the need for holistic approaches as the best strategy to mitigating and controlling the emerging and reemerging arboviruses.Ehlers-Danlos syndrome is a rare genetic disorder that presents with a variety of pathologies depending on the disease type. Among them, vascular Ehlers-Danlos syndrome requires extremely careful management as there have been many reports of fatal perinatal complications such as uterine rupture. Although hypermobile Ehlers-Danlos syndrome is less likely to cause fatal complications, symptoms such as arthralgia, hip dislocation, and depression may be seen throughout pregnancy. We report here a case of twin pregnancy in which Ehlers-Danlos syndrome was first suspected at 19 weeks of gestation. Vascular Ehlers-Danlos syndrome could not be ruled out based on family medical history, making it difficult to determine the perinatal management strategy. Prompt genetic testing did however rule out the vascular type and the patient was diagnosed with hypermobile Ehlers-Danlos syndrome from the clinical symptoms, enabling us to manage the pregnancy safely until 34 weeks of gestation.Corona virus 2019 (COVID-19) erupted toward the end of 2019, and it has continued to be a source of concern for a large number of people and organizations well into 2020. Wearing a face cover has been shown in studies to reduce the risk of viral transmission while also providing a sense of security. Be that as it may, it isn't attainable to physically follow the execution of this strategy. This proposed system is built by pretrained deep learning model, Vgg16. The proposed scheme is easy to implement and use all the layers in vgg16 model and train only the last layer called fully connected layer, which reduce the training time and effort. The proposed scheme is trained and evaluated using two Face mask datasets, one having 1484 pictures and the other with 7200. For a smaller dataset, augmented pictures were utilized to enhance accuracy. The suggested model is tested on unknown pictures, and it correctly predicts whether the image is wearing a mask or not. The proposed scheme gives accuracy 96.50% during testing in small dataset. The model gives accuracy in medium dataset is 91% during testing. By using vgg16 pretrained model and image augmentation in the dataset improves performance and gives a high accuracy.Today there is a disjuncture between migration flows that are complex, mixed and constantly evolving and the emerging global migration governance paradigm that seeks to impose clarity, certainty, regularity and order. Addressing the gap between policies and realities, this article explores lessons for migration policy and governance from mobile pastoralists' experience. Using examples from human migration flows within and between Europe and Africa and insights from pastoral systems from India, Italy and Kenya, the article identifies important similarities between international migration and pastoral mobility. We focus on four interconnections both international migration and pastoral mobility show multi-directional and fragmented patterns; both involve multiple, intersecting socio-economic, political, cultural and environmental drivers; both must respond to non-linear systems, where critical junctures and tipping points undermine clear prediction and forecasts, making social navigation and reliability management more useful concepts than risk-based prediction and control and finally for both uncertainty is not conceived of as a state of crisis but an inherent feature, pregnant with possibility and hope. Building on these four points, and drawing from pastoralists' experiences, we propose some methodological, practical and policy reflections for bridging the disjuncture between migration realities on the ground and global migration governance policies and discourses.Lactobacillus is a facultative anaerobic Gram-positive rod usually found in the normal microbiota of the gastrointestinal and genitourinary tract. Frequently dismissed as a contaminant, it is implicated in several diseases. We describe a rare case of endocarditis caused by Lactobacillus jensenii in an immunocompetent 40 year-old male patient, with a history of mitral valve repair. He presented complaining of asthenia and his laboratory results showed a moderate increase in inflammatory markers. A trans-thoracic echocardiography confirmed a vegetation on the posterior leaflet of the mitral valve, with associated severe mitral insufficiency. Blood cultures revealed the significant growth of L. jensenii. The patient developed an acute abdomen with intestinal ischemia and occlusion of the superior mesenteric artery, requiring urgent surgical laparotomy. A cerebral MRI showed multiple minor emboli in the frontal and left parietal cortex. The patient consequently underwent surgery to have his mitral valve replaced with a mechanical valve. L. jensenii was isolated in culture from the mitral valve and from a mesenteric artery thrombus. After one week of combined amoxicillin and gentamicin therapy, ampicillin alone was continued for a total of six weeks and the patient could be discharged in a good general condition. Only five cases of L. jensenii are described in literature, and they mainly affect immunocompromised hosts. In our case, a long delay between the start of symptoms and the full onset of the disease was observed. tolerance of Lactobacilli to penicillin is a key determinant of therapy choice.Vancomycin is used for the treatment of gram-positive infections. The increasing prevalence of methicillin-resistant Staphylococcus aureus since the 1980 s has led to a significant increase in the clinical utility of vancomycin in the United States. The diagnosis of vancomycin induced thrombocytopenia (VIT) poses a challenge since it requires the exclusion of other etiologies of thrombocytopenia, such as infectious or immunologic diseases and alternative drugs. Physicians should be aware that VIT is a rare but important complication of IV vancomycin therapy and failure to recognize this diagnosis may lead to a delay in discontinuation of vancomycin and serious complications.Moraxella canis was first identified in 1993 as normal flora of the oral cavities of dogs and cats. The species has been reported to cause localized infections in immunocompromised humans only three times. We report the first description of severe disseminated infection attributed to M. canis.
Neurotuberculosis comprises around 6% of systemic tuberculosis. It targets a younger population, and it often leads to severe neurological complications or death.
We report a young gentleman with a clinically defined tuberculous meningitis (TBM) and multiple neurological complication associated with TBM occurring simultaneously. This includes hydrocephalus requiring a ventriculoperitoneal shunt, vasculitic infarcts, cranial nerve palsies, TB granuloma and cerebral venous thrombosis. The cerebrospinal fluid polymerase chain reaction for tuberculosis as well as cultures remained negative repeatedly. The patient was treated with anti-tuberculous medication in addition to steroids based on validated scoring systems suggestive of TBM and made a good recovery.
This report highlights the different complication seen with TBM and the importance of using clinical criteria to guide management plan particularly when cultures are negative.
This report highlights the different complication seen with TBM and the importance of using clinical criteria to guide management plan particularly when cultures are negative.Infections with intravascular digenean trematodes of the Spirorchiidae family (spirorchiidoses) are of great conservation concern both in marine and freshwater turtles due to their pathogenic potential. Between 2014 and 2021, Spirorchis sp. infections associated with granulomatous inflammation and sudden death were detected in European pond turtles (Emys orbicularis) from three conservation breeding facilities in Switzerland. Blood fluke eggs associated with lesions were found in the intestine, spleen, testis, skeletal musculature, heart, kidneys, stomach, pancreas, liver, lung, and meninges from nine pond turtles submitted for necropsy and in the intestinal content from five of these animals. Two novel polymerase chain reactions (PCRs) targeting the 28S ribosomal RNA gene and the ITS2 region and subsequent sequencing revealed 100% nucleotide identity with a Spirorchis sp. previously isolated from an Escambia map turtle (Graptemys ernsti) in the USA. Our findings suggest a spill-over event secondary to direct or indirect contact with invasive North American turtle species in Switzerland. We describe the clinical, haematological, ultrasonographical, endoscopical, parasitological, pathological, and molecular findings associated with spirorchiid blood fluke infections of the Spirorchis genus in E. orbicularis, as well as the biosecurity measures that were developed to prevent the spread of this parasite among breeding and highly endangered free-ranging E. orbicularis populations in Switzerland.Sixteen species of parasitic mites of the family Demodecidae have been described in ungulates (Ungulatomorpha), particularly among domestic animals and livestock. Specific synhospital species have been found in seven host species. Floxuridine Demodex bisonianus, occurring in the Meibomian glands of the eyelids, was described in the European bison Bison bonasus. Together with Trypanosoma wrublewskii and Bisonicola sedecimdecembrii, it is one of the three known specific parasite species of European bison. The European bison is not only a rare and endangered mammal, but its biology has been shaped by its recent restitution from extinction from a small pool of individuals kept in breeding facilities. This could have been the reason for the extinction of the majority of specific parasites. The present study reports the finding of a new specific parasite species, Demodex bialoviensis sp. nov., associated with the nasal skin region, in European bison from the Białowieża Forest (Poland). It is the second species of Demodex reported in this host species, however occurring in a different location.Acute leukemias are often of myeloid or lymphoid origin. However, some acute leukemias revealed an undefined differentiation into a single lineage. Mixed phenotype acute leukemia (MPAL) is an uncommon diagnosis were blasts can share B/T/myeloid phenotype. Here, we report a rare case of a 17-year-old Moroccan female diagnosed with B/T mixed phenotype acute leukemia and a high hyperdiploid karyotype who relapsed after one year of complete remission with a lineage switch to B-cell acute lymphoblastic leukemia. This case report corroborates the disclosed findings about the high occurence of abnormal karyotypes and poor prognosis of MPAL.
