The median cost of attending medical school is rising annually, and with it, student debt. Neurology residents have stepped up during the pandemic to answer the call of a health system at its breaking point. In this article, we outline how this escalating problem of student debt affects the neurology pipeline, the wellbeing and career decisions of current neurology trainees and practicing neurologists and through it, and the gap in healthcare. We describe currently available options for loan repayment and call for advocacy and legislation to address this mounting burden as a means to improve neurological care in the United States.
Herbal medicines are not regulated by regulatory authorities and are often not of proven safety and efficacy. Anecdotal reports suggest widespread use of traditional herbal medicine (THM) for treatment of stroke in Africa, but verifiable data are limited in published literature.
To assess the frequency and reasons for THM use among Ghanaian stroke survivors.
We conducted a cross-sectional survey of 106 consecutive stroke survivors presenting for care at a tertiary medical center in Kumasi, Ghana between June and October 2020. Information on demographic, clinical, vascular risk factors and use of THM were collected in a non-judgmental manner and analyzed.
Among the cohort, 46 (43.4%) reported use of any THM for their stroke management. There was a trend towards fewer women who reported use of THM 34.8% vs 53.3%, p=0.06. Of those reporting use of THM, 78% had blood pressure>140/90mmHg vs. 56% among those not exposed to THM (p=0.02). Top reasons proffered for use of THM were expecting them to aid faster recovery from stroke (n=30), expecting them to cure stroke (n=18), and expecting them to be of superior effectiveness compared to approved conventional medications of proven efficacy/safety (n=1).
Almost half the individuals in this contemporary sample of Ghanaian stroke survivors reported use of THM with expectations for improved outcomes. More counseling is required to inform patients about potential safety issues with THM use, and more research is needed to explore risk/benefit of promising THMs to improve stroke outcomes.

Signs and symptoms of Systemic Mastocytosis (SM) are due to pathological infiltration of MCs and to the release of chemical mediators, mainly histamine. Patients with SM may also present with neurological symptoms or complications. The pathophysiology of these neurological disorders remains uncertain to this day, but it can be associated with the infiltration of tissue mastocytes, release of mastocytes' mediators or both. Moreover, there is a lot to understand about the role of neurological symptoms in SM and knowing, for example, what is the real frequency of neurological disorders in SM and if is present a relation between other SM subtypes, because it has been noted that the alteration of the histamine expression may be an initiating factor for susceptibility, gravity and progression of the epigenetic disease. In this review we explain the possible pathophysiological mechanism about neurological symptomatology found in some patients affected by SM, describing the role of histamine and its receptors in the nervous system and, in light of the results, what the future prospects may be for a more specific course of treatment.
With declining numbers of obstetrician-gynecologists operating in rural areas across the United States, primary care providers have stepped in to fill contraceptive service gaps. Many of these providers operate in Rural Health Clinics; however, little is known about the provision of contraception in these clinics.
This exploratory qualitative descriptive study used a purposive sampling strategy to recruit South Carolina Rural Health Clinic providers from across regions and income levels. Contraceptive care practices were identified using a combination of inductive and deductive coding.
Participants described their typical contraceptive patient as a low-income woman under 22 years. While providers were open to providing contraception, their on-site services were limited. Each included clinic offered the oral contraceptive pill and the shot, but only one offered the implant, and none offered the intrauterine device.
Rural Health Clinic providers have limited capacity to offer a full range of contraception due to financial, training, and staffing constraints. Despite these limitations, Rural Health Clinics remain a contact point that helps meet national recommendations for increasing access to reproductive health services for rural women. Efforts to increase access to contraceptive care for rural women must include resources for these providers.

Efforts to increase access to contraceptive care for rural women must include resources for these providers.The addition of optical fibers to improve light penetration into microalgae-immobilized poly (vinyl) alcohol (PVA) gel beads is proposed herein. Live/dead staining in combination with confocal laser scanning microscopy analysis revealed greater light penetration in Chlorella sorokiniana-immobilized PVA gel beads with optical fibers (PVA-OF) than in C. sorokiniana-immobilized PVA gel beads (PVA-NOF). The improved light penetration had positive effects on the initial nutrient removal efficiency, which resulted in a 1.2-fold higher initial nutrient removal efficiency in PVA-OF than in PVA-NOF. The use of polymeric PVA materials is effective for maintaining structural integrity. The findings demonstrate a methodology for mitigating self-shading effects in microalgae immobilized on opaque polymeric materials.This research evaluated the effect of biochar amendment rate on nitrogen species and organic carbon removals and hydraulic performance in biofilter columns treating dairy farm runoff. Initial studies compared the performance of sand columns amended with two types of biochar with different specific surface area (SA) and cation exchange capacity (CEC) with an un-amended sand column. The results showed that biochar enhanced N-species removal due to its unique physicochemical properties. In subsequent tests, two biofilter columns with different biochar fractions (20% and 50% by volume) were operated at varying hydraulic loading rates and antecedent dry conditions. Total nitrogen, ammonia, organic nitrogen and organic carbon removals were significantly higher in the column with the higher biochar fraction. The high CEC of biochar increased ammonium retention during the application period, allowing for nitrification during the antecedent dry periods (ADPs) when aerobic conditions developed in the media pores. High biochar SA also resulted in greater retention of DON and DOC by adsorption. A variable saturation flow model of biochar amended biofiltration was developed using HYDRUS-1D software. The model was calibrated using data from conservative tracer and moisture content studies. Model results showed that the high microporous structure of the biochar increases the time needed to reach full saturation, lowers the saturated conductivity and increases the hydraulic retention time in the medium. This calibrated model can be used to design field scale biofilter systems for managing agricultural runoff.River sediments are considered as sinks of microplastics (MPs). Although numerous studies have been conducted on MPs pollution in river sediments, the impact of MPs on the environmental behavior of Cd (II) in river sediments is still unknown. In this work, the effects of six MPs (polyethylene, polypropylene, polystyrene, polyvinyl chloride, polymethyl methacrylate and polylactic acid) on the adsorption of Cd (II) by Xiangjiang River sediment and the transport of Cd (II) in sediment were studied. The results showed that the adsorption ability of sediment to Cd (II) decreased with the increase of the content of MPs in sediment. When the content of MPs in sediment increased to 10%, polypropylene had the greatest effect on the adsorption affinity of sediments to Cd (II). Moreover, the addition of MPs accelerated transport of Cd (II) in sediment, and the transport of Cd (II) in sediment increased with the increase of the content of MPs. The reason may be that after adding MPs, the adsorption capacity of sediment to Cd (II) decreases, and the mass transfer resistance of Cd (II) to sediment reduces, which leads to faster transport of Cd (II) in sediment. Especially, when the content of MPs in sediment increased to 10% (w/w), the saturation point of the breakthrough curve decreased by about 70 pore volumes. This work hopes to provide helpful views on the environmental behavior and risk assessment of Cd (II) in the presence of MPs.In the present work, multi-walled carbon nanotubes (MWCNTs) were used as support material for the impregnation of metallic nanoparticles (MNPs) produced by green synthesis. The influences of the plant extracts (pomegranate (Punica Granatum), Eucalyptus, and pecan (Carya illinoinensis, leaves), metal species (copper and iron), metallic concentrations, and type of functionalization (OH and COOH) on the characteristics of the obtained materials were studied. The precursor and impregnated MWCNTs were characterized through X-ray diffraction, Fourier transformed infrared spectroscopy, scanning electron microscopy, energy-dispersive X-ray spectroscopy, point of charge, N2 adsorption/desorption isotherms and, X-ray photoelectron spectroscopy. All the synthesized materials were tested as adsorbents to remove glyphosate (GLY) in an aqueous medium. The MWCNTs were resistant to withstand the synthesis process, preserving its structure and morphological characteristics. The copper and iron on the surface of MWCNTS confirm the successful synthesis and impregnation of the MNPs. The MWCNTs impregnated with high metallic concentrations showed favorable adsorption of GLY. The adsorption capacity and percentage of removal were 21.17 mg g-1 and 84.08%, respectively, for the MWCNTs impregnated with iron MNPs using the pecan leaves as a reducing agent. The results indicated that an advanced adsorbent for GLY could be obtained by green synthesis, using MWCNTs as precursors and pecan leaves as a reducing agent.Difenoconazole is one of the most typical triazole fungicides. Difenoconazole is widely used in the field of agricultural production, and its health and safety problems need to be further studied. The main purpose of this paper is to verify the neurotoxicity of Difenoconazole at the cellular level. In this study, SH-SY5Y cell line of human neuroblastoma was used to evaluate its potentially toxic effects and molecular mechanism in vitro. The research indicated that Difenoconazole could reduce cell viability and inhibit cell proliferation, induce DNA damage and accelerate programmed cell death. Further studies showed that Difenoconazole induced DNA double-strand breaks, intracellular generation of ROS, cleaved PARP, mitochondrial membrane potential collapse, induced Cyt c release, and Bax/Bcl-2 ratio increase in SH-SY5Y cells. In conclusion, the cytotoxicity of Difenoconazole revealed its toxic effect on SH-SY5Y cells, and the IC50 value was 55.41 μM after 24 h exposure. Meanwhile, the genetic toxicity of Difenoconazole has revealed that it can induce DNA damage and apoptosis of SH-SY5Y cells.
