The particular lengthy shadow of years as a child drawback upon lifelong well being: acquiring even worse over time?
The article discusses perspectives of medical care in elderly and senile patients with cerebrovascular pathology with portable telemedical complex use. Analysis of telemedical cooperation physician-consultant-patient and discussion of medical equipment criteria for telemedical complex selection and organizational together with legal issues of medical data transfer, storage and access were performed. The important clinical features of cerebrovascular pathology in elderly patients were presented, which determined necessity of highly qualified medical specialist involvement in management of such patients. The opportunities of portable medical complex usage were discussed. The recommendations of the European Society of Cardiology (ESC), released in 2015, provide limited data on the management of elderly patients with pericarditis. Elderly people are more likely to have pericarditis risk factors such as malignant neoplasms or renal failure. Difficulties in diagnosing pericardial diseases may occur in a group of elderly patients with concomitant coronary heart disease and the development of acute myocardial infarction. In the treatment of pericarditis in elderly patients, non-steroidal anti-inflammatory drugs, preferably aspirin, are primarily used. Elderly people are not recommended taking indomethacin. Difficulties of drug treatment may be associated with polypharmacy and cognitive impairment in elderly patients with comorbid diseases. The prognosis in the group of patients older than 60 who have undergone pericarditis is less favorable in comparison with younger persons. The study aimed to assess a negative impact of an acquired single-sided deafness on quality of life of the elderly. Prospective analysis of outpatient records was carried out to identify elderly patients with single-sided deafness using pure tone audiometry. The main inclusion criteria were age over 60, ≥90 dB threshold of an affected ear, ≤30 dB threshold of an intact ear, an acquired single-sided hearing loss with sudden onset and deafness duration less than 5 years. Taking inclusion criteria into account two groups were formed the main group with single-sided deaf patients (n=25) and the control group of normal hearing patients (n=25). All participants were surveyed with PSQ, HHIE, THI questionnaires. There were revealed some changes in psychological status in the group of patients with single-sided deafness in comparison with the group of normal hearing participants. The scores of the questionnaires showed increased stress and anxiety levels and deterioration in quality of life. Patients with single-sided deafness complained about severe tinnitus in an affected ear, worse speech intelligibility in noisy environment and a constant need to adapt to the different acoustic situations that in turn badly affected psychoemotional homeostasis, intensifying stress severity, and quality of life.We examined 408 patients (151 male, 257 female) in an age range from 54 to 85 years (mean age 73,9±4,9) performing clinical (anamnesis, visual analysis, palpation, percussion, auscultation) sociological (GOHAI), additional (chewing efficiency evaluation, phonetic analysis, weight deficit of body calculation, OPG, CBCT) assessment. Among 322 of examined subjects working syndromological diagnosis was «decompensated dentition» related to lower jaw in 167 cases, 65 to upper jaw, 90 to both upper and lower jaws (altogether 412 dental arches). Chewing efficiency among edentulous patients was low considering that majority of patients had dentures. Score range for Geriartric General Oral Health Assessment Index (GOHAI) was from 7 to 25 points (mean 18,23±4,19) which is attributed as low level. General syndromes among patients were rough, disfiguring, ageing changes in facial appearance, major alteration of aesthetic norms. Equally important or even more was incapacitation impact of total loss of proper mastication, speech problems, phonetic alteration, problematic food swallowing. In our research we highlighted two pathological conditions among elderly and senile age groups decompensated dentition, and fully edentulous jaws. Taking into consideration suffering of the patients due to morphological functional and psychosocial disabling conditions, accompanying these diseases, we might consider this group of patients as handicapped and that could lead to the need to introduction of setting quotas of implant prosthetics treatment of the elderly and senile patients. The result, efficacy and efficiency of such rehabilitation are considered in compliance with the system of adequate treatment result for this part of population.Traumatic damages to the oral mucosa are the main predisposing factor for the development of the oncostomatological pathology among the old people. The aim of the investigation was to study the occurrence of traumatic lesions of the oral mucosa among people of old age groups and estimate the effectiveness of their treatment using modern domestic remedies. 967 people (569 males and 398 females) from 19 to 82 years old were examined to determine the occurrence of traumatic oral mucosal pathology. Value of effectiveness while treatment of 83 elderly patients using modern domestic agents «ASEPTA® adhesive balm for gums» and «ASEPTA® gel for gums with propolis» was assessed in comparison with vinylin for internal use. It is found that 9,41 and 9,52% of cases are traumatic lesions of the oral mucosa respectively among elderly and senile people respectively. The cause of them in 96,7-97,1% cases are traumatic factors while using removable dental prostheses; the untimely treatment of these factors in 7,9% leads to the formation of prosthetic fibroms. A comparative evaluation of the treatment effectiveness of prosthetic-etiology-traumatic lesions of the oral mucosa showed the advisability of using modern domestic agents like «ASEPTA® adhesive balm for gums» during the first 2 days from the start of therapy and then «ASEPTA® gel for gums with propolis» until complete healing of traumatic injuries, compared to the using of vinylin for internal use for the same purposes, which efficiency remains high. Timely detection and treatment of the traumatic defeats of oral mucosa are an important factors of prevention of oncostomatological pathologies among elderly and senile persons.Aging is closely related to the decline in the function of many organs. The rate of aging of an organism can vary significantly depending on the lifestyle of a particular person, environmental conditions, hereditary factors, etc. Onvansertib datasheet For several decades, markers of aging of the human body, reflecting the work of the main organs and systems, have been generally accepted. At the same time, at the present level of development of medical sciences, the study of the genetic component of aging is relevant. The purpose of the study is to analyze and systematize data related to markers of human aging, determined in substrates of the oral cavity. The search for literary information was carried out on the basis of materials from the RSCI, Pubmed, Cochrane databases. The study included studies of various levels case-control studies, cohort studies, randomized controlled trials. As a result of the analysis and systematization of literature data, various genetic markers of aging of oral tissues were identified and described modification of DNA as a whole, changes in the expression of certain genes (over-or under-expressed). Literature review data suggest that exposure to aging predictors may improve treatment and rehabilitation outcomes for dental patients. The analysis of experimental and partially clinical data about researches of neurobiological effects of tenuigenin - the most important bioactive substance of Polygala tenuifolia Willd. in this review was given. The neuroprotective and neurotrophic action of given substance were described in detail. It was noted, that the capacities of the tenuigenin to decrease the secretion of beta amyloid and to protect of neurons from damage by already made beta ameloids, to inhibit the processes of the tau proteins` hyperphosphorylation and inflammations in microglia, as well as increase the main synaptic transmission can be used by the development of effective therapeutic drugs aimed to reduce the pathogenesis of Alzheimer`s disease. The effects of dopaminergic neurons and mitochondrial membrane potential protection as well as reduction of α-synuclein phosphorylation can influence the processes by Parkinson`s disease. It was concluded, that the tenuigenin deserves further study and possibly will be used as augmentation of Alzheimer`s, Parkinson`s and other neurodegenerative diseases therapy.Information technology is becoming an increasingly important tool of scientific knowledge due to the exponential growth of the number of publications presenting the results of neurobiological and psychological research. To identify predictors of cognitive reserves and methods for their determination based on the developed applied ontology, we performed a bibliometric analysis of publications contained in the PubMed database. It is shown that the annual growth in the number of publications relevant to the topic in the period 1990-2019 was 11%, which is higher than the average for science. We found the following psychophysiological indicators of cognitive reserves to gain the greatest increase in the interest of researchers prefrontal cortex, beta range (13-30 Hz) of EEG, inhibition and information load. Analysis of the role of these factors suggests that the development and/or realization of cognitive reserves is provided due to inhibitory functions of the prefrontal cortex for the effective selection of relevant stimuli under conditions of information load. The identified predictors of cognitive reserves and the most popular methods of their psychometric assessment can be useful for choosing new areas of research in gerontology.Age-dependent diseases high social burden makes relevant medical technologies development and implementation of human aging process functional state various disorders prenosological diagnosis, treatment and prevention. Age-related activity decrease of the brain functional state and its interdependence with the sleep-wake continuum leads to premature (pathological) aging with manifestations of neurodegeneration. Although disturbances of the sleep-wake cycle functioning are characteristic for the aging process, age-related changes are not always accompanied by cognitive dysfunction. There are sufficient number studies on the molecular level changes in age-related sleep disorders. Aging, sleep disorders and processes of cognitive dysfunction association pathogenetic mechanisms heterogeneity also include metabolism changes. Various neurotransmitters and other biologically active substances, so-called «sleep-promoting substances» are involved in the regulation of sleep processes. The urgent need for molecular-cell markers and diagnostic patterns of sleep disorders further scientific search in aging process is promising and will make a significant contribution to the breakthrough technologies development in the pathological aging prevention. The complex architecture of sleep undergoes pronounced changes throughout a person's life. In the literature, the relationship of circadian rhythm disturbances during aging and the development of cognitive dysfunction is actively debated. Approach from the positions of the modern neurobiology, somnology and sleep medicine perspectives seems extremely relevant and timely. This review is devoted to some aspects of age-related in sleep and their molecular-cell markers changes modern scientific research and our own data analysis.
