Quantitative Assessment and also Prognostic Organizations with the Immune Scenery within Ovarian Obvious Cellular Carcinoma.
Multivariate analyses revealed that an older age and no surgery for metastasis were independent predictors of disease-specific death. These findings remained after excluding stage IV cases. Older age was a potent risk factor of rapid disease-specific death after met/rec. © 2020 Japanese Society of Pathology and John Wiley & Sons Australia, Ltd.A FLUENT software able to predict and assess the electrooxidation of ammonia from the simulation of ammonia concentration and flow field distribution was developed in this study. The flow field-based models of ammonia removal were simulated and modified through the experimental results. The parameter of reaction constant k is corrected to 0.00195, and the modified model fitted well with experimental values, with the error less than 4%. The electrode depth of 4 cm was assessed to be optimal for ammonia removal based on the comparison of the simulation results on ammonia concentration and flow field distribution. The prediction result applied in the industrial wastewater treatment indicated that complete ammonia could be achieved at 0.27 Ah/L, and about 50% of total nitrogen was removed at 0.8 Ah/L. About 7% of chloride ions are converted into inorganic by-products, indicating low biological toxicity and risk on environment. The energy consumption increased with the promotion of removal efficiency of total nitrogen, requiring 5.4 kWh/m3 to remove 50% total nitrogen at 0.8Ah/L. The results show the practicability and feasibility of this FLUENT software tool on the simulation and prediction of electrooxidation process, which can provide the simulation parameter settings for the subsequent application. METHODS LixiLan-G was a 26-week, randomized, open-label study in adults with T2D inadequately controlled while on glucagon-like peptide 1 receptor agonist (GLP-1 RA), with metformin, with/without pioglitazone and/or a sodium-glucose cotransporter 2 inhibitor. This analysis investigated the efficacy of switching to iGlarLixi by fasting baseline quartile C-peptide levels and baseline quartile of duration of T2D compared with continued GLP-1 RA use. RESULTS Change in glycated hemoglobin (HbA1c) from baseline to week 26 was significantly greater with iGlarLixi compared with continued GLP-1 RAs across all fasting C-peptide quartiles (-1.00% to -1.06% range vs. -0.23% to -0.54% range, respectively) and irrespective of all T2D duration quartiles (-0.94% to -1.07% vs. -0.25% to -0.50% range). A significantly greater proportion of participants in the iGlarLixi arm achieved an HbA1c of less then 7% across all C-peptide quartiles (51%-73% range) than in the GLP-1 RA arm (19%-32% range). The greatest reductions in HbA1c in participants receiving iGlarLixi were observed in those with the shortest duration of disease, though consistently greater than reductions observed with continued GLP-1 RAs. Reductions in HbA1c were comparable across C-peptide quartiles within the iGlarLixi arm. CONCLUSIONS The results of this study suggest that iGlarLixi is an effective treatment option, irrespective of C-peptide levels or duration of diabetes, in adults with insufficiently controlled T2D receiving GLP-1 RAs. Beard transplantation is the procedure to restore congenital absence or inefficient growth of beard and mustache. OBJECTIVES The aim of the study is to report our results and experience in beard transplantation. METHODS Patients with a demand for beard restoration who were treated with follicular unit extraction (FUE) hair transplantation method were included in this retrospective, case series study. Patients' birth date, race, presence of eunuchoidism, hair color, type and shaft thickness, and treatment details were retrieved from medical records. Patients were evaluated at least one year after the operation, and defect closure rates were calculated. At the follow-up examination, patients were asked to evaluate their satisfaction from the procedure. RESULTS Ten patients were consecutively treated between May 2007 and June 2016. The mean age of the patients was 29.6 years. Three of our patients were eunuchoid and the others wanted beard transplantation to have a more masculine look. Follow-up period was median of 39 months. Four patients were satisfied and four patients were very satisfied with the procedure with a mean 53% closure rate. Better closure rates were associated with brown and red hair color, medium and fine hair thickness, and absence of eunuchoidism. Patients with straight hair were satisfied with the procedure significantly. CONCLUSION Beard reconstruction by using the FUE method is a safe and effective method with high patient satisfaction rates. © 2020 Wiley Periodicals, Inc.Medical schools and residency programs have become very adept at teaching medical students and residents an enormous amount of information. However, it is much less clear whether they are effective at fostering virtuous qualities like empathy or professionalism in trainees. This would come as no surprise to Plato, who famously argued in the Meno that virtue cannot be taught. This pedagogical challenge threatens to stymie medical educators, who increasingly recognize the importance of professionalism, compassion, and empathy in the practice of good medicine. As medical educators, we are motivated to demonstrate that virtue is teachable and to find a way to do so, as this is how we will be able to improve the conduct of physicians and the quality of their care of patients. As such, we address the question of the teachability of virtue in the realm of medicine, analysing Plato's contradictory analyses in the Meno and Protagoras, and drawing upon modern neuroscience to turn an empirical lens on the question. We explore the ways in which Noddings' Ethic of Care may offer a way forward for medical educators keen to foster virtue in trainees. We conclude by demonstrating how, by harnessing the power of caring relationships, the principles of Noddings' Ethic of Care have already been applied to medical education at a university in Israel. © 2020 John Wiley & Sons, Ltd.AIM Infants with fetal growth restriction (FGR) are at an increased risk of perinatal morbidity and mortality. The long noncoding RNA H19 gene is expressed abundantly in placental villi and recent studies suggest that it regulates FGR. However, the role of H19 in the FGR placenta remains unclear. This study aimed to clarify the relationship between H19 expression and FGR using normotensive placentas after 34 weeks of gestation. METHODS Formalin-fixed paraffin-embedded tissues from human placentas collected from pregnancies resulting in small for gestational age (SGA) and appropriate for gestational age (AGA) newborns were used. The histopathological features of placenta tissues, such as villous stromal fibrosis, the numbers of terminal villi, villous vessels and cytotrophoblasts were analyzed using hematoxylin and eosin, Masson's trichrome staining and immunostaining. The localization and expression of H19 in the placentas were demonstrated by in situ hybridization and reverse transcription-quantitative polymerase chain reaction (RT-qPCR), respectively. Moreover, the expression levels of H19-regulated molecules such as IGF2 and decorin (DCN) were measured by RT-qPCR. RESULTS Histopathological features of the placental villous were not different between placentas associated with SGA and AGA. H19 localized to the villous stroma, endothelial cells and cytotrophoblasts. Moreover, the expression level of H19 in SGA placentas was significantly lower than that in AGA placentas. The expression levels of IGF2 and DCN in SGA placentas tended to be lower than those in AGA placentas similarly to H19. CONCLUSION This study highlights the potential importance of regulatory events mediated by H19 in SGA placentas without histopathological abnormalities in late pregnancy. © 2020 Japan Society of Obstetrics and Gynecology.Precise hepatocellular carcinoma (HCC) risk prediction will play increasingly important role with the contemporary HCC etiologies, i.e., non-alcoholic fatty liver disease and resolved HCV. Because HCC incidence rate in this emerging patient population is relatively low (~1% per year), identification of a subset of patients at the highest risk is critical to concentrate the effort and resources of regular HCC screening to those who most need it. Omics profiling has been derived several candidate HCC risk biomarkers, which could refine HCC screening by enabling individual-risk-based personalized or risk-stratified patient management. Various types of biomolecule have been explored as sources of information to predict HCC risk at various time horizons. Germline DNA polymorphisms likely reflect race/ethnicity- and/or etiology-specific susceptibility to HCC development or chronic liver disease progression toward carcinogenesis. Transcriptomic dysregulations in the diseased liver capture functional molecular status supporting oncogenesis such as inflammatory pathway and myofibroblast activation. Circulating nucleic acids, proteins, and metabolites may serve as less-invasive measure of molecular HCC risk. Characterization of gut microbiota may also inform HCC risk estimation. Each biomarker may have its niche of clinical application depending on logistics of use, performance, and costs with a goal to eventually improve patient prognosis as a part of the whole algorithm of chronic liver disease management. Surgery and/or radiotherapy (RT) are effective local treatments, yet many dogs succumb to distant metastasis. Immunotherapy represents an attractive strategy for this potentially immunogenic tumor. The objective of this multi-institutional retrospective study was to examine the clinical outcome of dogs with OM treated with ONCEPT melanoma vaccine. Most dogs also underwent surgery and/or RT (8 Gy × four weekly fractions). Dogs with distant metastasis at diagnosis and those receiving concurrent chemotherapy were excluded. One hundred thirty-one dogs treated with ONCEPT were included 62 had adequate local tumor control defined as complete tumor excision or irradiation of residual microscopic disease; 15 were treated in the microscopic disease setting following an incomplete excision without adjuvant RT; and 54 had gross disease. Median time to progression, median progression-free survival, and median tumor-specific overall survival were 304, 260, and 510 days, respectively. In multivariable analysis, presence of gross disease correlated negatively with all measures of clinical outcome. Other negative prognostic indicators were primary tumor ≥2 cm, higher clinical stage (stages 2 and 3), presence of lymph node metastasis at diagnosis, and caudal location in the oral cavity. Radiotherapy had a protective effect against tumor progression. Bcl-2 inhibitor To date, this is the largest reported series of dogs with OM treated with ONCEPT. Several previously reported prognostic indicators were confirmed. © 2020 American College of Veterinary Radiology.
