Over the last decades, the number of acellular dermal matrix (ADM)-assisted implant-based breast reconstructions (IBBR) has substantially increased. However, there is still a lack of prospective data on complication rates.
We performed a non-interventional, multicenter, prospective cohort study to evaluate complication rates of a human ADM in patients undergoing an IBBR after skin- and nipple-sparing mastectomies. Patients with primary reconstruction (cohort A) and patients undergoing a secondary reconstruction after capsular fibrosis (cohort B) using the human ADM Epiflex® (DIZG gGmbH, Berlin, Germany) were enrolled in this study. Patients were followed-up for 12 months after surgery.
Eighty-four eligible patients were included in this study of whom 28 women underwent a bilateral breast reconstruction, leading to 112 human ADM-assisted reconstructions in total (cohort A 73, cohort B 39). In 33.0% of the reconstructed breasts at least one of the complications of primary interest occurred, including implant loss 7.1%, seroma 15.2%; infection 5.4%, rash 8.0%, and Baker grade III/IV capsular fibrosis 2.7%, with no statistically significant differences between the cohorts. Previous radiation therapy was significantly associated with occurrence of any postoperative complication (OR 20.41;
value 0.027).
The rates of most complications were comparable to the rates reported for other ADMs with relatively low rates of capsular fibrosis and infections. The rate of seroma was increased in our study. Prior radiation therapy increased the risk of any postoperative complications. Therefore, the use of ADM in these patients should be considered carefully.

Prior radiation therapy increased the risk of any postoperative complications. Therefore, the use of ADM in these patients should be considered carefully.
The aim of this study was to compare the risk of complications and recurrence between oncoplastic and conventional breast surgery.
This is a retrospective analysis of a consecutive series of 436 patients with stage I-III breast cancer who underwent surgery at the University Hospital of Basel between 2011 and 2018.
The nipple/skin-sparing mastectomy (NSM/SSM) group showed significantly more delayed wound healing (32.7 vs. 5.8%,
< 0.001) and skin necrosis (13.9 vs. 1.9%,
= 0.020) compared to conventional mastectomy (CM), which corresponded to significantly higher odds of short-term complications (OR 2.34, 95% CI 1.02-5.35,
= 0.044). The incidence rate of long-term morbidity in oncoplastic breast-conserving surgery (OBCS) was significantly higher compared to conventional breast-conserving surgery (CBCS; 25.5 vs. 11.3 per 100 patient years [PY],
< 0.001), in particular concerning chronic pain (13.3 vs. 6.6,
= 0.011) and lymphedema (4.1 vs. 0.4,
= 0.003). Seroma as a long-term morbidiof adjuvant therapy.
Immunoglobulin κC (IGKC)-positive tumor-infiltrating plasma cells are associated with better prognosis in node-negative breast cancer patients without adjuvant systemic therapy. In the present study we evaluated the prognostic significance of IGKC in breast cancer patients treated with adjuvant chemotherapy ± tamoxifen.
IGKC expression was immunohistochemically analyzed in 193 breast cancer patients who were treated with adjuvant chemotherapy, either with cyclophosphamide, methotrexate, and 5-fluorouracil (CMF) or 5-fluorouracil, epirubicin, and cyclophosphamide (FEC) between 1993 and 2001 with a median follow-up of 11 years. The prognostic impact of IGKC expression was evaluated by Kaplan-Meier survival analyses as well as uni- and multivariate Cox regression. An interaction term was used to investigate a possible association between tamoxifen treatment and prognostic effect of IGKC expression.
Kaplan-Meier analyses identified IGKC as a prognostic marker for metastasis-free survival (MFS) higher IGKC expression was associated with a better outcome (
= 0.02, log rank). Results of univariate Cox regression confirmed the prognostic impact of IGKC expression patients with a strong IGKC expression had a longer MFS (hazard ratio [HR] 1.931; 95% confidence interval [CI] 1.087-3.431;
= 0.025). Multivariate Cox regression analysis showed the independent prognostic significance of IGKC expression (HR 2.070; 95% CI 1.088-3.938;
= 0.027). The interaction term confirmed a significant interaction between tamoxifen treatment and the prognostic impact of IGKC expression (
= 0.04).
IGKC expression had an independent prognostic impact in early breast cancer patients who received adjuvant chemotherapy. There was a significant interaction with the use of tamoxifen.

Due to the increasing use of dynamic breast MRI and the limited availability of MR-guided interventions, MRI-detected lesions usually undergo a second-look ultrasound (SLUS). We investigated the safety of a negative SLUS and a benign SLUS correlate in excluding malignant and high-risk lesions (B3) and evaluated criteria for the rate of detection on SLUS.
In the retrospective analysis, all breast MRIs performed between 2011 and 2013 were screened for newly detected lesions. We analyzed the SLUS detection rate dependent on breast density, mass character, lesion size, and histology. We calculated the sensitivity, specificity, positive predictive value (PPV), and negative predictive value (NPV) of a negative and benign SLUS for malignant lesions (B5) and lesions requiring surgical excision (including high-risk and B5 lesions).
We successfully correlated 110 of 397 lesions. The detection rate was significantly higher for mass than for non-mass lesions and correlated with lesion size for mass lesions only. Lesions without/with a benign SLUS correlate were more frequently benign (including B3) or required no further procedure (B2). The sensitivity of SLUS in the detection of B3 and B5 lesions was 58%, and 73% in the detection of B5 lesions. The NPV of a negative or benign SLUS for B3 and B5 lesions was 89%, and 96% for B5 lesions.
SLUS is a safe diagnostic tool for the management of MRI-detected lesions and can spare patients from undergoing invasive procedures.

However, in doing so, developers are often unaware of the consequences of their changes. Changes to one part of a model may affect other parts of the same model and/or even other models, possibly created and maintained by other developers. The consequences are incomplete changes and with it inconsistencies within or across models. Extensive works exist on detecting and repairing inconsistencies. However, the literature tends to focus on inconsistencies as errors in need of repairs rather than on incomplete changes in need of further propagation. Many changes are non-trivial and require a series of coordinated model changes. read more As developers start changing the model, intermittent inconsistencies arise with other parts of the model that developers have not yet changed. These inconsistencies are cues for incomplete change propagation. Resolving these inconsistencies should be done in a manner that is consistent withing the sequence of repairs resulting from the change propagation.We present a new method for model-based mutation-driven test case generation. Mutants are generated by making small syntactical modifications to the model or source code of the system under test. A test case kills a mutant if the behavior of the mutant deviates from the original system when running the test. In this work, we use hyperproperties-which allow to express relations between multiple executions-to formalize different notions of killing for both deterministic as well as non-deterministic models. The resulting hyperproperties are universal in the sense that they apply to arbitrary reactive models and mutants. Moreover, an off-the-shelf model checking tool for hyperproperties can be used to generate test cases. Furthermore, we propose solutions to overcome the limitations of current model checking tools via a model transformation and a bounded SMT encoding. We evaluate our approach on a number of models expressed in two different modeling languages by generating tests using a state-of-the-art mutation testing tool.Today, Fe-Al intermetallic compounds are receiving a great interest from the mechanical, aerospace, and biomedical industries. A novel production process for Fe-Al intermetallic powders based on the generation of metallic tapes by rapid solidification and disintegration by water vapor was proposed. In this research work, a comparison is made between the energy required to manufacture of Fe-Al powder using the aforementioned process and one of the most commonly used manufacturing processes within the industry such as mechanical alloying. In addition, some other benefits of the proposed manufacturing process are analyzed. To carry out this comparison, the theoretical equations that take into account the most important variables involved during the process such as the type of material and hardness, the initial and final particle size, the grinding stages and the heating of the treatment powder were considered. In the case of calculating the energy required for the new proposed process, the two main stages were considered such as (1) the production of FeAl metal tape and (2) the subsequent transformation of the tape into powder by means of injection water vapor. For the first stage, the CASTRIP process is considered, and for the second stage, the energy required for the generation steam. Although the calculations may have certain limitations, it is obvious that the energy required to Fe-Al powder production using the new process is much lower than that required by mechanical alloying, resulting in at least three orders of magnitude lower (2.75 × 106 versus 2.206 × 109 kJ/ton). This lower energy implies considerable economic savings in the production process. On the other hand, when using water as a grinding medium during the process, it results in less environmental and acoustic pollution, less manipulation risks for humans and finally, no harmful agents or additives are used, making the proposed process sustainable.
