The optimum diesel removal efficiency from distinct types of soil accomplished by the unique application of such foams as well as the optimum conditions of the foaming constituents have not been reported in literature so far. Hence, in this work, the removal of diesel contaminant from different soil types (desert, coastal, clay soil) is optimized, and the optimized results are reported for the first time, using response surface methodology (RSM), for alkylpolyglucoside phosphate (APG-Ph) foam, stabilized by nZVI. The effect of concentrations of APG-Ph (0.02, 0.04, 0.06, 0.08, and 0.1 volume %) and nZVI (2, 3, and 3.5 mg/l) on diesel removal efficacy from soil is studied using Box-Behnken design (BBD) of response surface methodology (RSM). Maximum diesel removal efficiency obtained at a concentration of 0.1 volume % APG-Ph foam with 3.5 mg/l nZVI for desert, coastal, and clay soil is 94.6, 95.3, and 57.5%, respectively. The optimum concentrations of APG-Ph and nZVI are found to be 0.98 volume % and 0.8 mg/l, respectively. Validation of this optimal condition experimentally results in highest removal efficiency of 98.3, 97.2, and 75.9% for desert, coastal, and clay soil respectively. This is in good agreement with the predicted values by RSM (98.67, 97.57, and 76.85%). The maximum diesel removal efficiency predicted at optimal concentration of APG-Ph and nZVI is significantly larger than the results reported in literature in last three years.Folic acid magnetic nanoparticles have been prepared and utilized as an effective and reliable sorbent in magnetic dispersive solid phase extraction combined with dispersive liquid-liquid microextraction for the extraction of seven pesticides from different juices before their determination by gas chromatography-flame ionization detector. The sorbent is prepared through ball milling process using a proper mixture of folic acid and magnetic iron oxide. Characterization of the sorbent was done with X-ray diffraction pattern, scanning electron microscopy, and vibrating sample magnetometry. In the current study, limits of detection were in the range 0.12-0.33 μg L-1. Relative standard deviations at a concentration of 40 μg L-1 of each analyte were in the ranges of 2.15-5.14% for intra-day (n = 6) and 3.78-6.91% for inter-day (n = 4) precisions. Extraction recoveries and enrichment factors were obtained in the ranges of 70-88 % and 566-708, respectively. Cage subsidence is a very common complication after lumbar interbody fusion. It may compromise vertebral interbody fusion through progressive spinal deformity and consequently cause compression of neural elements. Clinical relevance remains, however, unclear, with few studies on this subject and even less information regarding its correlation with clinical findings. The aim of this study was to identify risk factors for cage subsidence and clinical evaluation after transforaminal (TLIF) and posterior (PLIF) lumbar interbody fusion.
A retrospective study in patients submitted to TLIF and PLIF between 2008 and 2017 was conducted.
A total of 165 patients were included (123 TLIF and 42 PLIF). Univariate analysis showed an increased risk of cage subsidence in spondylolisthesis comparing with degenerative disk disease (p = 0.007). A higher preoperative lumbar lordosis angle (p = 0.014) and cage placement in L2-L3 (p = 0.012) were associated with higher risk of subsidence. The posterior cage positioning on verroperly evaluated and considered, through HU values measurement in CT scan, a feasible and reliable tool in perioperative planning.
The present study shows that lower BMD and posterior cage positioning are relevant risk factors for lumbar cage subsidence. Low BMD is also a predictor of poor clinical results, so it must be properly evaluated and considered, through HU values measurement in CT scan, a feasible and reliable tool in perioperative planning.
Anastomotic leak after cytoreductive surgery (CRS) with hyperthermic intraperitoneal chemotherapy (HIPEC) remains a dreaded complication. There is no consensus statement regarding the optimal timing for bowel anastomoses to perform after or before HIPEC.
Patients who underwent CRS+HIPEC and had at least one bowel anastomosis were retrospectively analyzed to evaluate if timing of anastomosis done after or before HIPEC had an impact on bowel complication rates (anastomotic leak and perforation).
From 2013 to 2019, 214 of 370 patients underwent CRS+HIPEC and had at least one bowel anastomosis. Of these 214 patients, 104 and 110 patients had anastomosis after and before HIPEC, respectively. A total of 324 anastomoses were performed, with a mean of 0.87 anastomoses per patient (range 1-4). The incidence of anastomotic leaks was comparable between the pre- and post-HIPEC groups (3.6% vs. 4.8%; p>0.05), as was the bowel complication rate (7.6% vs. 7.2%). After multivariate analysis, prior surgical score >tention and repair of serosal tears, and thorough inspection of the bowel before closure helps to decrease bowel complications. The timing of anastomosis can be at the discretion of the surgeon.
To assess the in vitro activity of cefiderocol (CFDC) against a collection of both ceftazidime-avibactam (CZA) susceptible and resistant KPC-producingEnterobacterales (KPC-EB) isolates. Secondly, to assess its synergistic activity in combination with different antibiotics.
One hundred KPC-EB isolates were tested 60 CZA susceptible and 40 CZA resistant. Among them, 17 pairs of CZA susceptible and resistant KPC-producing Klebsiella pneumoniae (KPC-Kp) isolates were collected from 17 distinct patients before and after CZA treatment, respectively. CFDC susceptibility was evaluated by both broth microdilution (lyophilized panels; Sensititre; Thermo Fisher) and disk diffusion testing. Results were interpreted using EUCAST breakpoints. Synergistic activity of CFDC in combination with CZA, meropenem-vaborbactam, imipenem, and amikacin against six characterized KPC-Kp strains, before and after acquisition of CZA resistance, was evaluated using gradient diffusion strip crossing method.
CFDC resistance rate was significantly higher in CZA resistant EB subset than in the susceptible one (p < 0.001) 82.5% vs 6.7%. MIC50 and MIC90 values were 0.25 and 2mg/L, 8 and 64mg/L in CZA-susceptible and CZA-resistant subset, respectively. KPC-Kp isolates harboring KPC-D179Y or KPC-Δ242-GT-243 variants showed CFDC MICs ranging from 4 to 64mg/L. CFDC showed in vitro synergistic effect mostly with CZA, against both CZA susceptible and resistant isolates, resulting in a synergy rate of 66.7%.
CZA resistance mechanisms in KPC-EB impair the in vitro activity of CFDC, often leading to co-resistance. CFDC in combination with the new β-lactamases inhibitors might represent a strategy to enhance its activity.

CFDC in combination with the new β-lactamases inhibitors might represent a strategy to enhance its activity.There is relatively little contemporary information regarding clinical characteristics of patients with Pseudomonas aeruginosa bacteremia (PAB) in the community hospital setting. This was a retrospective, observational cohort study examining the clinical characteristics of patients with PAB across several community hospitals in the USA with a focus on the appropriateness of initial empirical therapy and impact on patient outcomes. Cases of PAB occurring between 2016 and 2019 were pulled from 8 community medical centers. Patients were classified as having either positive or negative outcome at hospital discharge. Several variables including receipt of active empiric therapy (AET) and the time to receiving AET were collected. Variables with a p value of less then 0.05 in univariate analyses were included in a multivariable logistic regression model. Two hundred and eleven episodes of PAB were included in the analysis. AET was given to 81.5% of patients and there was no difference in regard to outcome (p = 0.62). There was no difference in the median time to AET in patients with a positive or negative outcome (p = 0.53). After controlling for other variables, age, Pitt bacteremia score ≥ 4, and septic shock were independently associated with a negative outcome. A high proportion of patients received timely, active antimicrobial therapy for PAB and time to AET did not have a significant impact on patient outcome.
Idiopathic pulmonary fibrosis (IPF) is a chronic, progressive fibrotic lung disease of unknown cause with a variable course. Acute exacerbations of IPF (AE-IPF) is sudden accelerations of the disease or a superimposed idiopathic acute injury significantly reducing lung function. To examine the serum concentrations of Progranulin (PGRN) and activin A in patients with AE-IPF in a pilot study.
Twenty-one patients with AE-IPF were compared with 23 patients with stable IPF as a control group. Serum PGRN and activin A levels, arterial blood gas measurements, and lung function were determined in these two groups.
Peripheral blood PGRN and activin A levels in patients with AE-IPF were 83.7 + 10.0 and 14.2 ± 1.7ng/ml (mean + SD), respectively; higher than those in the control group 61.0 + 5.8 and 5.8 + 1.0 (p < 0.001). PGRN and activin A levels were significantly negatively correlated with carbon monoxide diffusion capacity r = - 0.857 (p < 0.001) and r = - 0.757 (p < 0.001).

They may be possible markers of disease activity in AE-IPF.An efficient and simple method to obtain aortic media for primary culture of rat vascular smooth muscle cells (RVSMCs) is developed. link2 The main steps to obtain aortic media include isolation of rat aortic artery, removal of the fat tissue and branches, separation of longitudinal cutting edge, and peeling off the adventitia. link3 Then, aortic media was used to obtain RVSMCs by our tissue explants method and the enzyme digestion method. The removal efficiency of the intima and adventitia was confirmed by hematoxylin-eosin and immunohistochemical staining. Morphology and immunofluorescent staining were used to identify cells and cell purity. RVSMCs at the 3rd and 8th passages were isolated by our tissue explants method; the enzyme digestion method and the traditional tissue explants method were compared respectively. Western blotting and gel contraction assay were used to investigate the phenotype and contraction ability of RVSMCs obtained by the different methods. Compared with the other methods, RVSMCs isolated by our method showed higher purity and demonstrated "contractile" phenotype with retained contraction ability for more passages.
