To compare prognosis between individuals without diabetes, type 1 and type 2 diabetes in a nationwide atrial fibrillation cohort in Sweden and study the significance of severe hypoglycaemia.
Using data from all-inclusive national registers, 309,611 patients with non-valvular atrial fibrillation were enrolled during 2013-2014. Of these, 2,221 had type 1 and 58,073 had type 2 diabetes. Patients were followed for all-cause mortality until 27 March 2017, and for myocardial infarction, ischaemic stroke and first-ever diagnosis of heart failure or dementia until 31 December 2015. Hazard ratios (HRs) and 95% confidence intervals (CIs) were calculated using Cox and competing risk regression.
Using individuals without diabetes as reference (HR = 1), the adjusted HRs in type 1 vs. type 2 diabetes were for mortality 1.87 (CI 1.73-2.02) vs. 1.51 (CI 1.47-1.55), heart failure 1.59 (CI 1.42-1.78) vs. 1.41 (CI 1.34-1.48), myocardial infarction 2.49 (CI 2.17-2.85) vs. 1.70 (CI 1.59-1.81), ischaemic stroke 1.59 (CI 1.35-1.87) vs. 1.31 (CI 1.22-1.40), and dementia 1.46 (CI 1.15-1.85) vs. 1.28 (CI 1.18-1.40). Among individuals with type 2 diabetes, those with previous severe hypoglycaemia had increased risk of mortality (HR 1.26; CI 1.17-1.36) and dementia (HR 1.37; CI 1.08-1.73) compared with those without previous severe hypoglycaemia.

Potassium intake has been shown to be inversely associated with blood pressure and premature mortality. Previous studies have suggested that the association between potassium intake and blood pressure is modified by obesity, but whether obesity similarly influences the association between potassium intake and mortality is unclear.
We investigated whether potassium intake, reflected by 24-h urinary excretion, is associated with all-cause mortality, and explored potential effect modification by obesity.
We performed a prospective cohort study in community-dwelling individuals. The association between urinary potassium excretion and all-cause mortality was investigated by using multivariable Cox regression. We performed multiplicative interaction analysis and subgroup analyses according to BMI and waist circumference.
In 8533 individuals (50% male), the mean age was 50±13 y, mean urinary potassium excretion was 71±21mmol/24h, median BMI (in kg/m2) was 25.6 (IQR 23.1, 28.4) and mean waist circumference wad with increased mortality risk.Metabolic rate (MR) usually changes (scales) out of proportion to body mass (BM) as MR = aBMb, where a is a normalisation constant and b is the scaling exponent that reflects how steep this change is. This scaling relationship is fundamental to biology, but over a century of research has provided little consensus on the value of b, and why it appears to vary among taxa and taxonomic levels. By analysing published data on fish and taking an individual-based approach to metabolic scaling, I show that variation in growth of fish under naturally restricted food availability can explain variation in within-individual (ontogenetic) b for standard (maintenance) metabolic rate (SMR) of brown trout (Salmo trutta), with the fastest growers having the steepest metabolic scaling (b ≈ 1). Moreover, I show that within-individual b can vary much more widely than previously assumed from work on different individuals or different species, from -1 to 1 for SMR among individual brown trout. The negative scaling of SMR for some l selection for fast-growing individuals with steep metabolic scaling (b ≈ 1) early in life, where size-selective mortality is high for fishes. I support this by showing that b for SMR tends to increase with natural mortality rates of fish larvae within taxa.Objective. As cardiovascular diseases are a leading cause of death, early and accurate diagnosis of cardiac abnormalities for a lower cost becomes particularly important. Given electrocardiogram (ECG) datasets from multiple sources, there exist many challenges to the development of generalized models that can identify multiple types of cardiac abnormalities from both 12-lead ECG signals and reduced-lead ECG signals. In this study, our objective is to build robust models that can accurately classify 30 types of abnormalities from various lead combinations of ECG signals.Approach. Given the challenges of this problem, we propose a framework for building robust models for ECG signal classification. Firstly, a preprocessing workflow is adopted for each ECG dataset to mitigate the problem of data divergence. Secondly, to capture the lead-wise relations, we use a squeeze-and-excitation deep residual network as our base model. Thirdly, we propose a cross-relabeling strategy and apply the sign-augmented loss function to tackle the corrupted labels in the data. Furthermore, we utilize a pos-if-any-pos ensemble strategy and a dataset-wise cross-evaluation strategy to handle the uncertainty of the data distribution in the application.Main results. In the Physionet/Computing in Cardiology Challenge 2021, our approach achieved the challenge metric scores of 0.57, 0.59, 0.59, 0.58, 0.57 on 12-, 6-, 4-, 3- and 2-lead versions and an averaged challenge metric score of 0.58 over all the lead versions.Significance. Using the proposed framework, we have developed the models from several large datasets with sufficiently labeled abnormalities. Our models are able to identify 30 ECG abnormalities accurately based on various lead combinations of ECG signals. The performance on hidden test data demonstrates the effectiveness of the proposed approaches.Magnetic silicene junctions are versatile structures with spin-valley polarization and magnetoresistive capabilities. Here, we investigate the temperature effects on the transport properties of single magnetic silicene junctions. We use the transfer matrix method and the Landauer-Büttiker formalism to calculate the transmittance, conductance, spin-valley polarization and tunneling magnetoresistance. We studied the case for T = 0 K, finding the specific parameters where the spin-valley polarization and the tunneling magnetoresistance reach optimized values. Regarding the temperature effects, we find that its impact is not the same on the different transport properties. In the case of the conductance, depending on the spin-valley configuration the resonant peaks disappear at different temperatures. The spin polarization persists at a considerable value up to T=80 K, contrary to the valley polarization which is more susceptible to the temperature effects. In addition, a stepwise spin-valley polarization can be achieved at low temperature. The tunneling magnetoresistance is attenuated considerably as the temperature rises, decreasing more than two orders of magnitude after T=20 K. These findings indicate that in order to preserve the spin-valley polarization and magnetoresistive capabilities of magnetic silicene junctions is fundamental to modulate the temperature adequately.Objective.Mask positive-pressure ventilation could lead to lung ventilation inhomogeneity, potentially inducing lung function impairments, when compared with spontaneous breathing. Lung ventilation inhomogeneity can be monitored by chest electrical impedance tomography (EIT), which could increase our understanding of mask ventilation-derived respiratory mechanics. We hypothesized that the two-handed mask holding ventilation technique resulted in better lung ventilation, reflected by respiratory mechanics, when compared with the one-handed mask holding technique.Approach.Elective surgical patients with healthy lungs were randomly assigned to receive either one-handed mask holding (one-handed group) or two-handed mask holding (two-handed group) ventilation. Mask ventilation was performed by certified registered anesthesiologists, during which the patients were mechanically ventilated using the pressure-controlled mode. EIT was used to assess respiratory mechanics, including ventilation distribution, global and que could not improve the inhomogeneity of lung ventilation when monitored by EIT during mask ventilation although it obtained larger expiratory tidal volumes than the one-handed mask holding technique.The extent to which the pathophysiology of autosomal dominant Alzheimer's disease corresponds to the pathophysiology of 'sporadic' late onset Alzheimer's disease is unknown, thus limiting the extrapolation of study findings and clinical trial results in autosomal dominant Alzheimer's disease to late onset Alzheimer's disease. Although amyloid burden accumulation as determined by PET was greater after symptomatic onset in autosomal dominant Alzheimer's disease than in late onset Alzheimer's disease participants, CSF assays of amyloid-β42, amyloid-β40, tau and p-tau181 were largely overlapping in both groups. Rates of change in cognitive performance and hippocampal volume loss after symptomatic onset were more aggressive for autosomal dominant Alzheimer's disease participants. These findings suggest a similar pathophysiology of autosomal dominant Alzheimer's disease and late onset Alzheimer's disease, supporting a shared pathobiological construct.An acute shortage of infant formulas in the United States occurred in early 2022, exacerbating a longer-standing, less severe shortage that has occurred over the last several years. The shortage has been particularly problematic for specialized formulas such as those needed for infants and children with food allergies, intestinal failure, kidney disease, and metabolic disorders. Although undoubtedly the magnitude of the current shortage will abate over time, it has affected many children and caused tremendous distress for thousands of families. We propose a series of interventions to be undertaken as soon as feasible to help ensure that the conditions that led to this problem do not recur and families regain confidence in the safety and supply reliability of formulas for infants and young children regardless of their medical needs.
