Therefore, we believe that the boosted MWSE open up a novel orientation toward developing multifunctional composites with high-efficient EMW response and thermal management.Milk yield and composition traits (fat (%) and SNF (%)) from 1229 test day records of 205 Bargur cattle maintained under a farmers' production system were studied. This breed is known for its adaptability to the hilly tracts of Erode District in Tamil Nadu. Performance recording was done in the natural habitat through the test-day milk yield from lactating cows in the farmers' herds. Effects of non-genetic factors like season, year, parity, stage of lactation, and days from calving were studied using mixed models with animal taken as a random effect. Estimated marginal means for daily milk yield (DMY, L/day), fat (%), and SNF (%) were 2.05 ± 0.03, 4.08 ± 0.03%, and 8.19 ± 0.01%, respectively. Stage of lactation was highly significant (P less then 0.01) for DMY, fat, and SNF. Season and year were significant for DMY and SNF, where higher productivity was obtained in the monsoon season, but fat remained constant in all the seasons. Parity was significant (P less then 0.05) only for SNF, and year was significant for DMY (P less then 0.01) and SNF (P less then 0.05). Persistency was 5.9% calculated using Wood's gamma function and 63.0% calculated using the ratio of predicted yields. This was slightly lower compared to dairy breeds of cattle. Thirteen different lactation curve models were used for fitting of Bargur cattle lactation curve, and the Parabolic exponential model was the best fitting model based on model diagnostic criteria.
Pilocytic astrocytomas are the most common low-grade glioma of the central nervous system that typically occurs in children, and much research has been dedicated to characterizing their molecular features and clinical courses. We provide an overview of the current literature through the use of a bibliometric analysis of the top 100 most cited publications discussing pilocytic astrocytomas.
We identified the top 100 most cited publications discussing pilocytic astrocytomas. Articles were ranked based on the number of citations. Descriptive statistics and univariate analysis were used to determine any trends or significant differences in the data.
Of the top 100 articles, 50 were basic science (50%), 34 were clinical (34%), and 16 were review (16%). The number of citations ranged from 79 to 921, with 123 being the median. The US had the most first authors and principal authors (n = 53 and n = 54, respectively). Years of publication had a left-skewed distribution and peaked during 2011 with 12 articles published in that year. Sixty percent of basic science articles investigated BRAF/MAPK pathways, while 67.6% of clinical articles focused on evaluating treatment options for pilocytic astrocytomas. Compared to basic science and clinical articles, review articles were published more recently (p < 0.001), had fewer authors (p = 0.025) and were published in journals with higher impact factors (p = 0.022).
Research regarding pilocytic astrocytomas has increased over the past three decades. Future directions of research point towards employing targeted therapies and discovering additional cellular pathways contributing to disease pathogenesis.

By definition pain is an unpleasant sensory and emotional experience. In the context of inflammatory rheumatic diseases, nociceptive pain is triggered by inflammatory mediators or structurally mechanical distress via activation of the nociceptors. In addition, inflammation is also locally enhanced by the release of proinflammatory substances, such as substance P or calcitonin gene-related peptide (CGRP) from activated nociceptors. The phenomena of inflammation and pain are linked in a self-reinforcing mechanism, which explains why pain can be effectively controlled by inhibiting inflammation and, conversely, why inhibiting pain also has positive effects on the inflammatory response. This review focuses on publications on multimodal rheumatological complex treatment and thermotherapy, which, under evidence-based study criteria, showed a reduction in pain and a partial influencing of molecular markers with a subsequent influence on the development of pain and the inflammatory process. The results are presented in the context of current physiological knowledge on the development of pain.Current data demonstrate for the first time the effects of multimodal rheumatological complex treatment (MRCT). As designed in 2005, MRCT successfully treats the pain and functional limitations caused by rheumatic and degenerative diseases and reduces disease activity. In addition to the presentation of the current evidence, the article provides concrete recommendations for implementation as well as assistance in the examination by the medical service.
EOX (epirubicin, oxaliplatin, and capecitabine) is one of the standard regimens for metastatic or locally advanced gastric cancer (GC). A new combination based on fractional docetaxel (low-TOX) has been developed in an attempt to increase the efficacy of EOX and reduce the heavy toxicity of classical docetaxel regimens.
Overall, 169 previously untreated GC patients were randomized between EOX (arm A) and low-TOX (arm B). G007-LK clinical trial The primary endpoint was progression-free survival (PFS), while secondary ones were overall survival (OS), overall response rate (ORR), disease control rate (DCR), and tolerability. The study was designed to detect a 35% (80% power at a two-sided 5% significance level) PFS increase with low-TOX and an interim analysis for futility was planned after the first 127 events.
At the cut-off date of interim analysis, median PFS was 6.3months [95% confidence interval (CI) 5.0-8.1] in arm A vs 6.3months (95% CI 5.0-7.8) in arm B, without statistical difference. OS was comparable in the two arms 12.4 in arm A (95% CI 9.1-19.2) vs 11.5months in arm B (95% CI 8.6-15.0). ORR was 33% and 24%, while DCR was 68% and 67%, respectively. Treatment modification (91% vs 78%, P = 0.017) and number of patients with CTC grade ≥ 3 adverse events (42 vs 35) were higher in arm B.
A triplet regimen based on the fractional dose of docetaxel achieves no improvement over EOX which remains a potential standard treatment in many patients with inoperable, locally advanced or metastatic GC.

The best QSAR model screened via MLR technique indicated that the exact mass, topological diameter and numerical rotational bonding structural properties of benzimidazole derivatives highly affect the bioactivity of these compounds against α-amylase. Based on the structural properties identified via linear QSAR model favorable for improving pIC50 of benzimidazole derivatives, fourteen new molecules bearing benzimidazole radicals were designed and their biological inhibitory activity against α-amylase was improved. QSAR model predictions showed that the designed molecules exhibited a higher potential biological level activity IC50 than acarbose used in positive control (IC50= 1.46 μM). Screening of drug-like properties, pharmacokinetics and toxicity of the proposed molecules led to select three molecules as candidates for use as drug aid to ingest starch and glycogen. As a result, using molecular docking simulations, the docking poses of the three molecules inside the α-amylase receptor pocket (PDB code 1HNY) were predicted. Also, the most important potential interactions between the active amino acid sites in α-amylase protein pocket and the proposed drug molecules were described. The obtained hypotheses regarding the stability of the proposed molecules inside α-amylase pocket were validated by carrying out molecular dynamic simulations in aqueous background similar to the ones of proteins. The DM results confirmed the optimal stability of the α-amylase backbone with the drug molecules proposed in this computational investigation.
Biliopancreatic diversion with duodenal switch and single anastomosis duodenal-ileal bypass with sleeve gastrectomy (SADI-S) are technically demanding hypo-absorptive bariatric procedures generally indicated in super-obese patients (BMI ≥ 50kg/m
). Data from the literature prove the procedure to be safe and effective, with promising bariatric and metabolic effects. Anyway, international societies support the creation of multicentric national and international registries to obtain more homogeneous data over the long period. We aimed to report our experience with this procedure.
Among 2313 patients who underwent bariatric procedures at our institution, between July 2016 and August 2021, 121 (5.2%) consenting patients were scheduled for SADI-S as primary (SADIS) or revisional procedure after sleeve gastrectomy (SADI) (respectively 87 and 34 patients). Early and late post-operative complications, operative time, post-operative stay, and follow-up data were analyzed.
Overall, the median preoperative BMI was 52.3 (48.75-57.05) kg/m
with a median age of 44 (39-51) years, the median operative time was 120 (100-155) min. Complications at 30th-day post-op were registered in 4 (3.3%) patients and late complications in 4 (3.3%) patients. At a median follow-up of 31 (14-39) months, the median percentage excess weight loss was 79.8 (55.15-91.45)%, and the median total weight loss was 57.0650 (43.3925-71.3475)%.
Our data, coherently with the literature, confirm that SADI-S is a safe, effective procedure with acceptable complications rate. Larger studies with longer follow-ups are necessary to draw definitive conclusions.

Ventral hernias commonly affect patients after major abdominal surgery. To reduce postoperative pain, the effects of the transversus abdominis plane (TAP) block, epidural analgesia and medication-only protocol have been investigated. The primary outcome was the cumulative dosage of opioids (morphine milligram equivalents MME), of acetaminophen and diclofenac for postoperative pain control on postoperative day (POD) 0, 1, and 2. Secondary outcomes were length of stay (LOS) and the pain scale rating using the numeric rating scale (NRS) on POD 0, 1, and 2.
The data were retrospectively extracted from the charts of the patients admitted for a surgical operation for OVHR from January 2015 to December 2019.
Patients receiving medication-only analgesia had longer LOS (mean 6.1days; p < 0.00001). Cumulative opioid consumption was significantly lower at 24 and 48h after surgery in the TAP block group than in the other groups (mean MME 1.9mg and 0.7mg, respectively; p < 0.05). The cumulative consumption of diclofenac was significantly lower in the TAP block group than in the others (44.
