With respect to the young age of patients, a radiation-induced long-term risk for malignant diseases should be taken into consideration.Polymorphisms in arsenic (+ 3 oxidation state) methyltransferase (AS3MT) have been shown to be related to interindividual variations in arsenic metabolism and to influence adverse health effects in acute promyelocytic leukemia (APL) patients treated with arsenic trioxide (As2O3). The occurrence of hyperleukocytosis with As2O3 treatment seriously affects the early survival rate of APL patients, but no definite explanation for such a complication has been clearly established. To clarify the causes of this situation, AS3MT polymorphisms 14215 (rs3740390), 14458 (rs11191439), 27215 (rs11191446), and 35991 (rs10748835) and profiles of plasma arsenic metabolites were evaluated in a group of 54 newly diagnosed APL patients treated with single-agent As2O3. High-performance liquid chromatography-hydride generation-atomic fluorescence spectrometry (HPLC-HG-AFS) was used to determine the concentrations of plasma arsenic metabolites. Plasma arsenic methylation metabolism capacity was evaluated by the percentage of inorganic arsenic (iAs), monomethylarsonic acid (MMA), dimethylarsinic acid (DMA), primary methylation index (PMI, MMA/iAs), and secondary methylation index (SMI, DMA/MMA). The results showed that APL patients who developed hyperleukocytosis had a higher plasma iAs%, but a lower MMA% and PMI than those who did not develop hyperleukocytosis during As2O3 treatment. In addition, patients with the AS3MT 14215 (rs3740390) CC genotype had significantly higher plasma iAs% and incidence of hyperleukocytosis, but lower PMI than patients with the CT + TT genotype. Conversely, we did not observe statistically significant associations between the occurrence of hyperleukocytosis and AS3MT 14458 (rs11191439), 27215 (rs11191446), and 35991 (rs10748835) polymorphisms in our study subjects. These results indicated that AS3MT 14215 (rs3740390) might be used as an indicator for predicting the occurrence of hyperleukocytosis in APL patients treated with As2O3.In addition to hepatocytes, the liver comprises a host of specialised non-parenchymal cells which are important to consider in the development of in vitro models which are both physiologically and toxicologically relevant. We have characterized a 3D co-culture system comprising primary human hepatocytes (PHH) and non-parenchymal cells (NPC) and applied it to the investigation of acetaminophen-induced toxicity. Firstly, we titrated ratios of PHHNPC and confirmed the presence of functional NPCs via both immunohistochemistry and activation with both LPS and TGF-β. Based on these data we selected a ratio of 21 PHHNPC for further studies. We observed that spheroids supplemented with NPCs were protected against acetaminophen (APAP) toxicity as determined by ATP (up to threefold difference in EC50 at day 14 compared to hepatocytes alone) and glutathione depletion, as well as miR-122 release. APAP metabolism was also altered in the presence of NPCs, with significantly lower levels of APAP-GSH detected. Expression of several CYP450 enzymes involved in the bioactivation of APAP was also lower in NPC-containing spheroids. The aim of this analysis was the most comprehensive review of intraoperative and postoperative outcomes to date. METHODS Prospective evaluation of intraoperative features in all eyes scheduled to have cataract surgery with Clareon® IOL implantation using the AutonoMe® implantation system between December 2017 and September 2018 with follow-up at 4-6 weeks. Inclusion of all patients, regardless of comorbidities and accompanying ocular surgery. RESULTS A total of 391 eyes were evaluated for the intraoperative analysis and 144 eyes were evaluated for postoperative follow-up. There was one IOL-associated intraoperative complication in which the posterior haptic was not correctly loaded and should not have been implanted. The best corrected distance visual acuity (BCDVA) increased from a mean of 0.33 logMAR (SD 0.21) preoperatively to 0.09 logMAR (SD 0.21) postoperatively, independent of ocular comorbidities. The BCDVA in patients without ocular comorbidities increased from 0.27 logMAR (SD 0.13) preoperatively to 0.03 logMAR (SD 0.05) postoperatively. Using the SRK®/T formula 87.5% of all eyes had a refractive error of ≤0.5 D and 98.0% had a refractive error of ≤1 D 4-6 weeks after surgery. CONCLUSION This study showed that using the recommended safety guidelines, the AutonoMe® system enables safe implantation of the IOL. The Clareon® IOL showed good postoperative visual outcomes and refraction results using the SRK®/T formula.Intraocular infections associated with Abiotrophia defectiva are rare. This article reports the case of a 57-year-old woman with endophthalmitis associated with Abiotrophia defectiva 3 months after uncomplicated cataract surgery combined with the implantation of a glaucoma gel-stent in the right eye. The patient had complained of redness of the right upper nasal conjunctiva and pain for 2 weeks prior to the endophthalmitis. A topical steroid eyedrop treatment without antibiotic additives had temporarily improved the situation. The patient presented with hypopyon, acute deterioration of vision and severe periocular pain of the right eye since the early morning. The gel-stent had spontaneously perforated the conjunctiva. The patient was immediately started on local and systemic antibiotics and underwent pars plana vitrectomy with intravitreal antibiotic application 6 h after presentation. Unlike other ocular infections with Abiotrophia defectiva, this case had a relatively benign course most likely due to the prompt intervention. In clinical routine, patients, who present with acute deterioration of vision and pain after glaucoma surgery, should be examined urgently considering a possible spontaneous conjunctival perforation and late onset endophthalmitis. Additionally, conjunctivitis of unclear origin following ocular surgery should always be treated with antibiotics, particularly when steroids are administered and monitored closely.Patients undergoing lens surgery nowadays have the option to choose between different intraocular lens (IOL) options depending on the ocular situation and personal preferences. For example, it is possible to implant a toric IOL to compensate for extensive corneal distortion or a multifocal IOL to have good visual acuity at various distances. The high level of competition in the lens market leads to fast development of new lens models giving patients the advantage to choose between a variety of options. This review article presents the most commonly used premium IOL options namely, aspheric, toric and multifocal IOLs and a summary of the current study situation for the different lens types. Compared to standard lenses, these lenses can improve uncorrected distance and near visual acuity; however, in some patients the added benefit can be only marginal and this is then an unnecessary cost factor for the patient. Furthermore, if inclusion and exclusion criteria are not respected the special forms of lenses can even lead to a poorer overall result.Newborn screening (NBS) is an essential, preventive public health programme for early identification of disorders whose early treatment can lead to significant reduction in morbidity and mortality. The still high diagnostic delay of DMD and the current availability of drugs such as steroid, ataluren, eteplirsen, golodirsen and forthcoming new drugs, improving the clinical conditions if early started, make appropriate to begin a concrete discussion between stakeholders to identify best practice for DMD screening. A two-step system CK/DNA screening programme is presented to be performed in male infants aged between 6 months and 42 months involving more than 30,000 male infants. Five to eight DMD subjects are believed to be diagnosed. METHODS This retrospective analysis investigated 119 psychiatric consultations for cancer patients in a one year period. The assessment covered demographics and cancer diagnosis, psychiatric diagnosis and proceedings. RESULTS One third of all patients were treated for hematological cancer, followed by lung cancer. Depression was the most common psychiatric disorder. One third of all patients with hematological cancer were diagnosed with depression, followed by delirium. Inpatient psychiatric treatment and psychotherapy were most commonly recommended by the consultant psychiatrist. Furthermore, 80.2% of all patients received recommendations for medication with antidepressants.
