We aimed to assess the risk of preterm birth in those with real-time dynamic cervical shortening.

A retrospective matched case-control study. The study group comprised all women with dynamic cervical shortening (≥4 mm) noted from 24 to 34 weeks of gestation during 2010-2017 at a university hospital. Two control groups of women were established by matching the minimal and maximal cervical length measured, as well as age, parity, gestational age, history of spontaneous preterm birth, symptoms of preterm labor, and delivery year.

Data from 339 women were analyzed, 113 with dynamic cervical shortening comprised the study group, and two groups with 113 women each, matched for the minimal and maximal cervical lengths measured comprised the control groups. Rates of spontaneous preterm birth rate at <37 weeks (32.7% vs. 15.9%; OR [95% CI] 2.60 (1.36, 4.87),
 = .004) and <35 weeks (15.9% vs. 5.3%; OR [95% CI] 3.38 (1.29, 8.86),
 = .013) were significantly higher among those with dynamic cervix than among the control group matched for the maximal cervical length, and comparable to the control group matched for the minimal cervical length. The negative predictive values of cervical length for preterm birth occurrence at various cutoff values were lower in those with dynamic cervix.

The minimal cervical length measured should be used to guide patient management when dynamic cervix is noted. In the setting of dynamic cervical change, the value of cervical length as a negative predictor of preterm birth is limited.

AnaConDa (Anesthetic-Conserving-Device; ACD; Sedana Medical, Sweden), connected between ventilator and the patient, retains isoflurane during expiration, and releases it back during inspiration. The reflection efficiency (RefEff) corresponds to the percentage of expired anesthetic molecules that are re-inspired. We compared RefEff of AnaConDa-S (ACD-50) and AnaConDa-100 (ACD-100) under laboratory (DRY) and simulated clinical conditions (CLIN) using a test lung.Methods Measurements were made under DRY and CLIN, with different tidal volumes (TV 300 mL & 500 mL) and infusion rates (0.5-10 mL·h-1). RefEff was calculated from the isoflurane concentration in the test-lung (CISO) and plotted against the anesthetic vapor volume exhaled in one breath (V-exh = CISO·TV).Results DRY RefEff of both devices was ≈90% over a wide range of V-exh, but decreased when V-exh exceeded 5-7 mL (ACD-50) or 10-15 mL (ACD-100).CLIN RefEff of ACD-50 was 70-80% (ACD-100 80-90%), decreasing gradually with increasing V-exh. For 1 Vol.% isoflurane at TV500, the infusion rate with ACD-50 was twofold higher compared to ACD-100 (4 versus 2 mL·h-1).Conclusion Under DRY and concentrations less then 1.5 Vol.%, RefEff of both devices is around 90%. Under CLIN, ACD-100 performs better with RefEff between 80% and 90% (ACD-5070-80%), decreasing with increased vapor volume exhaled in one breath.In this investigation, we focused on ceramide IIIB, a skin component whose depletion tends to augment multiple skin disorders and fungal infections. Ceramide IIIB was included into PEGylated surfactant-based vesicular phospholipid system to formulate 'PEGylated cerosomes' (PCs) loaded with fenticonazole nitrate (FTN). FTN is a potent antifungal agent adopted in the treatment of mixed mycotic and bacterial infections. The ceramide content of the vesicles may provide protective and regenerative skin activity whereas Brij®; the PEGylated surfactant, can enhance drug deposition and skin hydration. Both components are expected to augment the topical effect of FTN. PCs were prepared by thin-film hydration technique. A 23 full-factorial design was applied to study the effect of ceramide amount (X1), Brij type (X2) and Brij amount (X3) on the physicochemical properties of the formulated PCs namely; entrapment efficiency (EE%;Y1), particle size (PS;Y2), polydispersity index (PDI;Y3) and zeta potential (ZP;Y4). The optimal formula was selected for further in-vivo dermatokinetic and histopathological study. The optimal FTN-loaded PC (PC6) showed nanosized cerosomes (551.60 nm) with high EE% (83.00%w/w), and an acceptable ZP value of 20.90 mV. Transmission electron micrographs of the optimal formula illustrated intertwined tubulation form deviated from the conventional spherical vesicles. Finally, the dermatokinetic study of PC6 showed higher drug concentration and localization of FTN in skin layers when compared with FTN suspension and the histopathological study confirmed its safety for topical application. The overall findings of our study verified the effectiveness of utilizing PEGylated cerosomes to augment the activity of FTN as a topical antifungal agent.Phoneme production may be affected by limited speech motor control in Childhood Apraxia of Speech (CAS), with a general instability of acoustic targets across multiple repetitions of speech stimuli. The ethyl acetic extracts of P. grandidentatus and P. ecklonii registered the highest antioxidant activity, whereas acetonic, methanolic and ethyl acetic extracts of P. ecklonii, P. grandidentatus, P. madagascariensis and P. saccatus concerning the enzymatic inhibition assays revealed high anti-tyrosinase and anti-collagenase activities. From the isolated compounds tested, abietane diterpenes and triterpenes were highly active against tyrosinase and elastase activity. Overall, the experimental results showed the powerful antioxidant and inhibitory action on skin-related enzymes tyrosinase, collagenase and elastase of Plectranthus spp. extracts and/or isolated compounds, supporting their further research as bioactive metabolites against skin sagging and hyperpigmentation in cosmetic and pharmaceutical formulations.
