The homeostatic model assessment (HOMA), based on fasting glucose and fasting insulin levels, was used to index insulin resistance. Age, sex, race, year of assessment, physical activity, cigarette smoking, alcohol use, and waist circumference were used as covariates. The relationships between total carbohydrate intake (F = 6.7, p = 0.0121), simple carbohydrate (F = 4.7, p = 0.0344) and HOMA-IR were linear and direct. The associations between fiber intake (F = 9.1, p = 0.0037), total protein (F = 4.4, p = 0.0393), total fat (F = 5.5, p = 0.0225), monounsaturated fat (F = 5.5, p = 0.0224), and total unsaturated fat (F = 6.5, p = 0.0132) were linear and inversely related to HOMA-IR, with 62 degrees of freedom. Starch, polyunsaturated fat, and saturated fat intakes were not related to HOMA-IR. In conclusion, in this nationally representative sample, several macronutrients were significant predictors of insulin resistance in U.S. adults.Necrotizing enterocolitis (NEC) is the main gastrointestinal emergency of preterm infants for whom bowel rest and parenteral nutrition (PN) is essential. Despite the improvements in neonatal care, the incidence of NEC remains high (11% in preterm newborns with a birth weight <1500 g) and up to 20-50% of cases still require surgery. In this narrative review, we report how to optimize PN in severe NEC requiring surgery. Glutamatergic involvement across diverse neuropathologies including psychoses, neurodevelopmental, neurodegenerative conditions, and brain injury forms the rationale for this review. A conceptual overview of brain glutamatergic activity and peripheral interfacing is presented, including the common mechanisms that diverse diseases share when glutamate homeostasis is disrupted.Tapioca resistant maltodextrin (TRM) is a novel non-viscous soluble resistant starch that can be utilized in oral nutrition supplements (ONS). This study aims to evaluate acute and long-term metabolic responses and the safe use of ONS containing TRM. This study comprised of two phases In Phase I, a randomized-cross over control study involving 17 healthy adults was conducted to evaluate three ONS formulations original (tapioca maltodextrin), TRM15 (15% TRM replacement), and TRM30 (30% TRM replacement). Plasma glucose, serum insulin, and subjective appetite were evaluated postprandially over 180 min. In Phase II, 22 participants consumed one serving/day of ONS for 12 weeks. Blood glucose, insulin, lipid profile, and body composition were evaluated. Gastrointestinal tolerability was evaluated in both the acute and long-term period. During phase I, TRM30 decreased in area under the curve of serum insulin by 33.12%, compared to the original formula (2320.71 ± 570.76 uIU × min/mL vs. 3470.12 ± 531.87 uIU × min/mL, p = 0.043). In Phase II, 12-week TRM30 supplementation decreased HbA1C in participants (from 5.5 ± 0.07% to 5.2 ± 0.07%, p < 0.001), without any significant effect on fasting glucose, insulin, lipid profile, and body composition. The ONS was well-tolerated in both studies. TRM is therefore, a beneficial functional fiber for various food industries.Obesity is a public health crisis in Kuwait. However, not all obese individuals are metabolically unhealthy (MuHO) given the link between obesity and future cardiovascular events. We assessed the prevalence of the metabolically healthy obese (MHO) phenotype and its relationship with high sensitivity C-reactive protein (hs-CRP), serum alanine aminotransferase (ALT), and insulin resistance (HOMA-IR) in Arab and South Asian ethnic groups in Kuwait. The national cross-sectional survey of diabetes and obesity in Kuwait adults aged 18-60 years were analysed. The harmonised definition of metabolic syndrome was used to classify metabolic health. Multinomial logistic regression analysis was used to model the relationship between the MHO and MuHO phenotypes and hs-CRP, ALT and HOMA-IR levels. Overall, the prevalence of MHO for body mass index (BMI)- and waist circumference (WC)-defined obesity was 30.8% and 56.0%, respectively; it was greater in women (60.4% and 61.8%, respectively) than men (39.6% and 38.2%, respectively). Prevalence rates were also lower for South Asians than for Arabs. The MHO phenotype had hs-CRP values above 3 µg/mL for each age group category. Men compared to women, and South Asians compared to Arabs had a lower relative risk for the MHO group relative to the MuHO group. This study shows there is high prevalence of MHO in Kuwait.Carbon black (CB) is used in polyethylene (PE) pipes to protect against thermal and photooxidation. However, when CB is not properly dispersed in the PE matrix during processing, white regions having little or no CB concentration, known as "windows," appear within the CB/PE mixed black compound. In some cases, windows can drastically affect the structural integrity of both the pipe and butt fusion joint. In this work, PE pipes with varying amounts of windows were investigated for their characteristic window patterns, as well as quantifying the area fraction of windows (% windows). Tensile test on specimens with known % windows determined a critical limit above which the fracture strain rapidly degrades. Micro-tensile and micro-indentation results showed tear initiation at the window-black PE matrix boundary; however, they did not confirm the mechanism of tear initiation. In support of this work, a method of making thin shavings of a whole pipe cross section was developed, and the best viewing windows under cross-polarized monochromatic light were identified. In addition, a phased array ultrasonic test (PAUT) and microwave imaging (MWI) were directly applied to the pipe and confirmed the presence and patterns of the windows.Customarily, retarders serve as the setting time regulators of cement-based composites to meet the demands of various construction environments. However, the limited ability to adjust the setting time restricts the application of polysaccharides in special environments. In this study, we reported a naturally high-efficiency retarder, konjac glucomannan (KGM), and studied the mechanism of its effect on the hydration of ordinary Portland cement. Incorporating KGM could significantly prolong cement hydration without strength damage. Furthermore, the active hydroxyl group (-OH, rich in KGM) could chelate with Ca2+ (released from cement hydration) to form a cross-linking network, which is adsorbed on the surface of cement clinker, thereby being conducive to delaying the process of cement hydration and reducing the heat of hydration.
