To investigate microvascular parameters that are related to the severity of diabetic retinopathy (DR) with optical coherence tomography angiography (OCTA).

In total, 105 eyes from 105 diabetic patients were recruited in this prospective cross-sectional study, including 37 eyes with no clinical signs of DR (NoDR), 43 eyes with nonproliferative diabetic retinopathy (NPDR), and 25 eyes with proliferative diabetic retinopathy (PDR). Angiogram images from the parafoveal superficial capillary plexus (SCP), the deep capillary plexus (DCP), and the radial peripapillary capillary plexus were analyzed, and metrics were compared among groups. Multivariate regression analysis was used to identify the best OCTA parameters that could distinguish DR severity among groups.

Parafoveal vessel diameter index in the SCP and vessel density (VD) in the DCP showed the strongest correlation with the severity of DR (P < 0.01). Extrafoveal avascular area in the SCP was the parameter that could most distinguish NoDR from NPDR (P < 0.01) with sensitivity and specificity of 83.72% and 78.38%, respectively. VD in the DCP also was the most sensitive biomarker to distinguish NPDR from PDR (P < 0.01) with sensitivity and specificity of 84.00% and 79.07%, respectively.

The microvascular changes in the SCP and DCP in DR may have different characteristics that could be identified with specific OCTA parameters. OCTA serves as a promising technology to discriminate eyes with different severity of DR.

To determine whether Mel4-coated antimicrobial contact lenses (MACLs) can reduce the incidence of corneal infiltrative events (CIEs) during extended wear.

A prospective, randomized, double-masked, single-center, contralateral, extended contact lens wear clinical trial was conducted with 176 subjects. Each participant was randomly assigned to wear a MACL in one eye and an uncoated control contact lens in the contralateral eye or an extended-wear biweekly disposable modality for 3 months. The main outcome measures were the incidence of CIEs per 100 eye-months, identification of the microbial types colonizing the contact lenses or eyes at the time of the CIEs, and their susceptibility to Mel4.

Nine participants (5.1%) experienced unilateral CIEs; six participants had contact lens acute red eye, and three participants had infiltrative keratitis. The incidence rate for CIEs (0.4 events per 100 participant months; 1.7%) in the Mel4-coated lenses (test) was 69% less than that of the control lenses (1.3 events per 100 participant months; 3.4%; P = 0.29). All Gram-negative bacteria isolated from lenses and lids of participants with CIEs (Citrobacter diversus, Acinetobacter haemolyticus, and Acinetobacter lwoffii) were susceptible to Mel4 peptide; minimum inhibitory concentrations ranged from 15.6 to 62.5 µg/mL. Reduction of adhesion of these bacteria by Mel4-coated lenses ranged from 2.1 to 2.2 log10 colony-forming units/lens.

MACLs had the capacity to reduce CIEs by at least 50% compared with uncoated control lenses during extended wear over 3 months; however, due to the relatively low rates of CIEs, the reduction was not statistically different compared with control lenses.

This study provides evidence that antimicrobial contact lenses have the potential to reduce the incidence of corneal infiltrative events during extended wear.Due to their large size (∼3-5 Gb) and high repetitive content, the study of cephalopod genomes has historically been problematic. However, with the recent sequencing of several cephalopod genomes, including the Hawaiian bobtail squid (Euprymna scolopes), whole-genome studies of these molluscs are now possible. Of particular interest are the sepiolid or bobtail squids, many of which develop photophores in which bioluminescent bacterial symbionts reside. The variable presence of the symbiosis throughout the family allows us to determine regions of the genome that are under selection in symbiotic lineages, potentially providing a mechanism for identifying genes instrumental in the evolution of these mutualistic associations. To this end, we have used high-throughput sequencing to generate sequence from five bobtail squid genomes, four of which maintain symbioses with luminescent bacteria (E. hyllebergi, E. PD123319 albatrossae, E. scolopes and Rondeletiola minor), and one of which does not (Sepietta neglecta). When we performed K-mer based heterozygosity and genome size estimations, we found that the Euprymna genus has a higher predicted genome size than other bobtail squid (∼ 5 Gb as compared to ∼ 4 Gb) and lower genomic heterozygosity. When we analyzed the repetitive content of the genomes, we found that genomes in the genus Euprymna appear to have recently acquired a significant quantity of LINE elements that are not found in its sister genus Rondeletiola or the closely related Sepietta. Using Abyss-2.0 and then Chromosomer with the published E. scolopes genome as a reference, we generated E. hyllebergi and E. albatrossae genomes of 1.54-1.57 Gb in size, but containing over 78-81% of eukaryotic single-copy othologs. The data we have generated will enable future whole-genome comparisons between these species to determine gene and regulatory content that differs between symbiotic and non-symbiotic lineages, as well as genes associated with symbiosis that are under selection.The sperm is essential for reconstitution of embryonic diploidy and highly specialized developmental functions. We describe ischemic events (cardiovascular death, stroke, or myocardial infarction) and major bleeding in participants from the randomised, double-blind COMPASS study by number of cardiovascular medications and concomitant medical conditions. We compared event rates and hazard ratios (HR) for rivaroxaban plus aspirin versus aspirin alone by the number of medications and concomitant conditions, and tested for interaction between polypharmacy or multimorbidity and the antithrombotic regimen.The risk of ischemic events was higher in patients with more concomitant drugs (HR 1.7, 95%CI 1.5-2.1 for >4vs 0-2) and with more comorbidities (HR 2.3, 1.8-2.1 for >3vs 0-1). Multimorbidity, but not polypharmacy, was associated with a higher risk of major bleeding. The relative efficacy, safety, and net clinical benefit of rivaroxaban were not affected by the number of drugs or comorbidities. Patients taking more concomitant medications derived the largest absolute reduction in the net clinical outcome with added rivaroxaban (1.1%vs 0.4% reduction with >4vs 0-2 cardiovascular drugs, NNT 91vs 250).

Adding low-dose rivaroxaban to aspirin resulted in benefits irrespective of the number of concomitant drugs or comorbidities. Multiple comorbidities and/or polypharmacy should not dissuade the addition of rivaroxaban to aspirin in otherwise eligible patients.
