The 2015 American Thyroid Association risk stratification system (ATA RSS) is used in patients with differentiated thyroid carcinoma (DTC) to assess their risk of persistent/recurrent disease. Our aims were to validate the 2015 ATA RSS in a registry of DTC patients and to examine whether the addition of factors not included in it, such as pre-radioactive iodine therapy stimulated thyroglobulin (pre-RAI sTg), gender, and age could increase its predictive ability.

We studied 403 patients with DTC, treated at a tertiary center from 1990 to 2018 and subjected to total thyroidectomy. All patients had received RAI therapy, except those with low-risk papillary microcarcinoma.

Of our patients, 81.9% were women and 91.1% had papillary thyroid carcinoma. After a median follow-up of 5.0years, 53 cases of persistent and 21 cases of recurrent disease were recorded. The proportion of variance explained (PVE) regarding the outcome (presence or absence of recurrent/persistent disease) using the 2015 ATA RSS alone was 1 inclusion of pre-RAI sTg values; pre-RAI sTg was an independent predictor of the disease outcome, with high negative prognostic value.

The ethical dimensions of living and deceased uterus transplantation are explored and addressed in the paper through their comparison to the ethical elements of an existing, legal, assisted reproduction practice in some high-income countries, i.e., gestational surrogacy. A set of six ethics lenses is used in the comparative analysis reproductive autonomy and rights, informed choice/consent, relevant critical relational theories, health equity, theoretical application of the accepted living donation standard, and comparative benefits and burdens considerations. Gestational surrogacy, as currently practiced in some high-income countries, is the assumed, theoretical base-threshold for determination of ethical acceptability in assisted reproduction practices. The analysis demonstrates that (at the present time) 1) the ethical acceptability of living uterus donation/transplantation is less than that of gestational surrogacy in high-income countries, and 2) the ethical acceptability of deceased uterus donation/transplantation is roughly equivalent to that of gestational surrogacy. This leads to the conclusion that, at the present time, only one version of uterus transplantation practice, i.e., deceased uterus transplantation, should be considered ethically acceptable for possible clinical implementation in high-income countries.
The Cobb angle method is used to determine the severity of scoliosis. Therapeutic decisions for adolescent idiopathic scoliosis (AIS) are guided by the Cobb angle. Therefore, high reliability is crucial. The objective of this study was to determine the intra- and inter-observer reliability of the digital Cobb angle measurements and the definition of end vertebrae in patients with AIS. Moreover, the influence of the observer's medical specialty and experience on Cobb angle measurement was evaluated.

Intra- and inter-observer reliability of the digital Cobb angle and end vertebrae is assessed in postero-anterior radiographs of 39 patients with AIS. Measurements were performed blinded and twice by six observers, with a two to 3 week interval. Intra- and inter-observer reliability was analysed by means of intraclass correlation coefficients (ICC).

Both intra- and inter-observer reliability analyses resulted in ICC's higher than 0.864 for the Cobb angle and definition of end vertebrae. In addition, for the observer's experience and medical specialty group the inter-observer ICC's were higher than 0.984. The average inter-observer variability for the Cobb angle were 3°, and 1.1-1.6 levels for the cranial and caudal end vertebrae selection. The variability in measured Cobb angle was 1° for the experience group and 2° for the medical specialty group. Cronbach's alpha varied from 0.990 to 0.996. Bland-Altman plots showed moderate variation with a few outliers.

In a structured anonymous scientific survey from January 29 to April 26, 2021 a questionnaire was sent to 72 heads of orthopedic and trauma departments. The doctors were asked to distribute and recollect the questionnaire after obtaining informed consent from the hospital administration, the nursing management and the works council. Descriptive statistics were used to explain the results.

355 answers from 5 departments were evaluated. In 65 out of 72 hospitals the top managers of either hospital or nursing administration or the works council declined participation, 2 heads of department were not interested. 50,7% of HCW were already vaccinated or had a scheduled appointment for vaccination, 14,9% had not yet made up their mind and 34,4% refuse vaccination. The majority of HCW vote against compulsory vaccination (47,6%), less than a quarter approves it (23,4%).

The willingness to vaccinate was extremely low in this survey. The main reason for refusal to vaccinate is fear of side effects, so an information campaign should primarily focus on this topic.
