Benefits of national-level stay-at-home order imposed in Italy to prevent SARS-CoV-2 transmission need to be carefully weighed against its impact on citizens' health. In a country with a strong familial culture and where welfare relies on households, confinement drastically decreased support provided by elder relatives, which may have resulted in mental health worsening.
A web-based cross-sectional study (LOST in Italy) was conducted on a representative sample of Italian adults during lockdown (27th of April-3rd of May 2020). We asked 3156 subjects to report on reduced help in housework and childcare from retired parents to assess the impact of confinement on mental health, through validated scales before and during lockdown.
Overall, 1484 (47.0%) subjects reported reduced housework help from parents, and 769 (64.0%, of the 1202 subjects with children) diminished babysitting support. For example, in vision and somatosensory systems, inhibition can adjust the relationships between input and output, a computation referred to as gain modulation. Inhibition may modulate gain within the motor system as well. Changes in cortico-spinal inhibition observed during human behavior can reflect adjustments in motor system gain and may be sensitive to latent behavioral states. This review summarizes roles for inhibition in gain modulation, drawing principally on evidence from non-motor systems, and examines the hypothesis that homologous functions operate in the animal and human motor systems to facilitate action preparation.
Phosphatidylserine-containing liposomes (PSLs) can mimic the immunomodulatory effects of apoptotic cells by binding to the phosphatidylserine receptors of macrophages. Sodium butyrate, an antiinflammatory short-chain fatty acid, is known to facilitate the M2 polarization of macrophages. This study aimed to investigate the effect of sodium butyrate on PSLs-induced macrophage polarization.
PSLs physical properties and cellular uptake tests, reverse transcription-quantitative polymerase chain reaction, enzyme-linked immunosorbent assay, immunofluorescence staining, and flow cytometry analysis were performed to assess the polarization-related indicators of M1/M2 macrophages.
The results showed that sodium butyrate did not affect the size and cellular uptake of PSLs. For M1 macrophage polarization, sodium butyrate significantly intensified the antiinflammatory function of PSLs, inhibiting LPS-induced proinflammatory genes expression, cytokines and enzyme release (tumor necrosis factor-alpha, interleukin (IL)-1β, IL-6, and inducible nitric oxide synthase), as well as CD86 (M1 marker) expression. In addition to the enhancing effect of antiinflammation, sodium butyrate also promoted PSL-induced M2 macrophages polarization, especially elevated thymus and activation-regulated chemokine (TARC) and arginase-1 (Arg-1) enzyme levels which are involved in tissue repair.

Therefore, sodium butyrate may represent a novel approach to enhance PSL-induced macrophage polarization.Multisystem inflammatory syndrome in children (MIS-C) represents a rare but severe complication of severe acute respiratory syndrome coronavirus 2 infection affecting children that can lead to myocardial injury and shock. Vascular endothelial dysfunction has been suggested to be a common complicating factor in patients with coronavirus disease 2019 (COVID-19). This study aims to characterize endothelial glycocalyx degradation in children admitted with MIS-C. We collected blood and urine samples and measured proinflammatory cytokines, myocardial injury markers, and endothelial glycocalyx markers in 17 children admitted with MIS-C, ten of which presented with inflammatory shock requiring intensive care admission and hemodynamic support with vasopressors. All MIS-C patients presented signs of glycocalyx deterioration with elevated levels of syndecan-1 in blood and both heparan sulfate and chondroitin sulfate in the urine. The degree of glycocalyx shedding correlated with tumor necrosis factor-α concentration. Five healthy age-matched children served as controls. Patients with MIS-C presented severe alteration of the endothelial glycocalyx that was associated with disease severity. Future studies should clarify if glycocalyx biomarkers could effectively be predictive indicators for the development of complications in adult patients with severe COVID-19 and children with MIS-C. KEY MESSAGES Children admitted with MIS-C presented signs of endothelial glycocalyx injury with elevated syndecan-1 and heparan sulfate level. Syndecan-1 levels were associated with MIS-C severity and correlated TNF-α concentration. Syndecan-1 and heparan sulfate may represent potential biomarkers for patients with severe COVID-19 or MIS-C.
The placebo effect in randomized controlled trials (RCTs) for East Asian patients with ulcerative colitis (UC) has been poorly characterized. This systematic review and meta-analysis aimed to determine the placebo rates of clinical and endoscopic remission in East Asian patients with UC and to identify factors that influenced placebo outcomes.
A comprehensive search was performed to identify all eligible RCTs. The placebo rates of clinical and endoscopic remission were pooled by random effects. Univariate meta-regression was performed to evaluate the influence of different factors on these placebo rates.
Fourteen induction and seven maintenance RCTs were included, comprising 1781 East Asian patients with UC. The pooled placebo rates of clinical and endoscopic remission for induction trials were 13% (95% confidence interval [CI] 10-16%) and 26% (95% CI 21-32%), respectively. Corresponding values for maintenance trials were 15% (95% CI 10-23%) and 24% (95% CI 19-29%). Heterogeneity existed among the studies. On univariate meta-regression, the route of drug administration, outcome definition, disease severity, disease duration, distal disease and concomitant corticosteroids influenced the outcomes of placebo arms.

Use of high-flow oxygen, noninvasive mask ventilation, and the technique of awake proning can sometimes avoid the need for intubation. Mechanical ventilation follows the principles of ventilation for acute respiratory distress syndrome (ARDS; lung protective ventilation) and is generally supplemented by consequent positioning therapy (with at least 16 h in prone position in multiple cycles). Antiviral therapy options such as remdesivir usually come too late for patients with COVID-19 in the ICU, the only exception being the administration of monoclonal antibodies for patients without seroconversion. The value of immunomodulatory therapy such as dexamethasone is undisputed. Interleukin‑6 antagonists, on the other hand, are rather problematic for ICU patients, and for Janus kinase inhibitors, data and experience are still insufficient in this context.
Dual-mobility hip arthroplasty utilizes a freely rotating polyethylene acetabular liner to protect against dislocation. As liner motion has not been confirmed in vivo, we undertook this using dynamic radiostereometry (RSA).
6 patients with Anatomical Dual Mobility acetabular components were included. Markers were implanted in the liners using a drill guide. Static RSA recordings and patient-reported outcome measures were obtained postoperatively and at 1-year follow-up. Dynamic RSA recordings were obtained at 1-year follow-up during passive hip movement abduction/external rotation, adduction/internal rotation (modified FABER-FADIR), to end-range and at 45° hip flexion. Liner and neck movements were described as anteversion, inclination, and rotation.
Liner movement during modified FABER-FADIR was detected in 12 of 16 patients. Median (range) absolute liner movements were anteversion 10° (5-20), inclination 6° (2-12), and rotation 11° (5-48) relative to the cup. Median absolute change in the resulting liner/neck angle (small articulation) was 28° (12-46) and in liner/cup angle (larger articulation) was 6° (4-21). Static RSA showed changes in median liner anteversion from 7° (-12 to 23) postoperatively to 10° (-3 to 16) at 1-year follow-up and inclination from 42° (35-66) postoperatively to 59° (46-80) at 1-year follow-up. Liner/neck contact was associated with high initial liner anteversion (p = 0.01).
