Endoscopic stenting is a well-established option for the treatment of malignant obstruction, temporary management of benign strictures, and sealing transmural defects, as well as drainage of pancreatic fluid collections and biliary obstruction. In recent years, in addition to expansion in indications for endoscopic stenting, considerable strides have been made in stent technology, and several types of devices with advanced designs and materials are continuously being developed. In this review, we discuss the important developments in stent designs and novel indications for endoluminal and transluminal stenting. Our discussion specifically focuses on (i) biodegradable as well as (ii) irradiating and drug-eluting stents for esophageal, gastroduodenal, biliary, and colonic indications, (iii) endoscopic stenting in inflammatory bowel disease, and (iv) lumen-apposing metal stent.
Contralaterally controlled neuromuscular electrical stimulation (CCNMES) is a novel electrical stimulation treatment for stroke; however, reports on the efficacy of CCNMES on lower extremity function after stroke are scarce.
To compare the effects of CCNMES versus NMES on lower extremity function and activities of daily living (ADL) in subacute stroke patients.
Forty-four patients with a history of subacute stroke were randomly assigned to a CCNMES group and a NMES group (
= 22 per group). Twenty-one patients in each group completed the study per protocol, with one subject lost in follow-up in each group. selleck kinase inhibitor The CCNMES group received CCNMES to the tibialis anterior (TA) and the peroneus longus and brevis muscles to induce ankle dorsiflexion motion, whereas the NMES group received NMES. The stimulus current was a biphasic waveform with a pulse duration of 200
s and a frequency of 60 Hz. Patients in both groups underwent five 15 min sessions of electrical stimulation per week for three weeks. Indicators of motor function and ADL were measured pre- and posttreatment, including the Fugl-Meyer assessment of the lower extremity (FMA-LE) and modified Barthel index (MBI). Surface electromyography (sEMG) assessments included average electromyography (aEMG), integrated electromyography (iEMG), and root mean square (RMS) of the paretic TA muscle.
Values for the FMA-LE, MBI, aEMG, iEMG, and RMS of the affected TA muscle were significantly increased in both groups after treatment (
< 0.01). Patients in the CCNMES group showed significant improvements in all the measurements compared with the NMES group after treatment. Within-group differences in all post- and pretreatment indicators were significantly greater in the CCNMES group than in the NMES group (
< 0.05).

Gastrodin (GAS) is a phenolic glycoside extracted from the traditional Chinese herb-Gastrodia elata-and has been reported as a potential therapeutic agent for AD. However, its efficiency is reduced for AD patients due to its limited BBB permeability. Studies have demonstrated the feasibility of opening the blood-brain barrier (BBB) via focused ultrasound (FUS) to overcome the obstacles preventing medicines from blood flow into the brain tissue. We explored the therapeutic potential of FUS-mediated BBB opening combined with GAS in an AD-like mouse model induced by unilateral intracerebroventricular (ICV) injection of Aβ 1-42. Mice were divided into 5 groups control, untreated, GAS, FUS and FUS+GAS. Combined treatment (FUS+GAS) rather than single intervention (GAS or FUS) alleviated memory deficit and neuropathology of AD-like mice. The time that mice spent in the novel arm was prolonged in the Y-maze test after 15-day intervention, and the waste-cleaning effect was remarkably increased. Contents of Aβ, tau, and P-tau in the observed (also the targeted) hippocampus were reduced. BDNF, synaptophysin (SYN), and PSD-95 were upregulated in the combined group. Overall, our results demonstrate that FUS-mediated BBB opening combined with GAS injection exerts the potential to alleviate memory deficit and neuropathology in the AD-like experimental mouse model, which may be a novel strategy for AD treatment.Handwritten characters recognition is a challenging research topic. A lot of works have been present to recognize letters of different languages. The availability of Arabic handwritten characters databases is limited. Motivated by this topic of research, we propose a convolution neural network for the classification of Arabic handwritten letters. Also, seven optimization algorithms are performed, and the best algorithm is reported. Faced with few available Arabic handwritten datasets, various data augmentation techniques are implemented to improve the robustness needed for the convolution neural network model. The proposed model is improved by using the dropout regularization method to avoid data overfitting problems. Moreover, suitable change is presented in the choice of optimization algorithms and data augmentation approaches to achieve a good performance. The model has been trained on two Arabic handwritten characters datasets AHCD and Hijja. The proposed algorithm achieved high recognition accuracy of 98.48% and 91.24% on AHCD and Hijja, respectively, outperforming other state-of-the-art models.Blood cell count is highly useful in identifying the occurrence of a particular disease or ailment. Nowadays, deep learning-based methods are in practice to analyze the data. These methods are less time-consuming and require less sophisticated equipment. This paper implements a deep learning (D.L) model that uses the DenseNet121 model to classify the different types of white blood cells (WBC). The DenseNet121 model is optimized with the preprocessing techniques of normalization and data augmentation. This model yielded an accuracy of 98.84%, a precision of 99.33%, a sensitivity of 98.85%, and a specificity of 99.61%. The proposed model is simulated with four batch sizes (BS) along with the Adam optimizer and 10 epochs. It is concluded from the results that the DenseNet121 model has outperformed with batch size 8 as compared to other batch sizes. The dataset has been taken from the Kaggle having 12,444 images with the images of 3120 eosinophils, 3103 lymphocytes, 3098 monocytes, and 3123 neutrophils. In order to enhance the effect of image artistic style transfer, the image is recognized by using a multi-scale discriminator. The experimental results show that this algorithm is effective and has great application and promotion value.Recently, the electroencephalogram (EEG) signal presents an excellent potential for a new person identification technique. Several studies defined the EEG with unique features, universality, and natural robustness to be used as a new track to prevent spoofing attacks. The EEG signals are a visual recording of the brain's electrical activities, measured by placing electrodes (channels) in various scalp positions. However, traditional EEG-based systems lead to high complexity with many channels, and some channels have critical information for the identification system while others do not. Several studies have proposed a single objective to address the EEG channel for person identification. Unfortunately, these studies only focused on increasing the accuracy rate without balancing the accuracy and the total number of selected EEG channels. The novelty of this paper is to propose a multiobjective binary version of the cuckoo search algorithm (MOBCS-KNN) to find optimal EEG channel selections for person identification.
