The purpose of this article is to review the current state and role of neuroimaging in this continuously evolving field.The sartorius muscle transposition flap is the traditional method of femoral vessel coverage after superficial inguinal lymphadenectomy for regionally-metastatic cancers to the inguinal lymph nodes. However, if the groin has undergone radiotherapy, the sartorius muscle is contained within the irradiated field, and may be problematic for wound healing, in addition to being thin at its insertion and intimately related to several nerves. The gracilis muscle has been used for soft tissue defects and vascular graft infections, but its utility as an alternative to the sartorius muscle flap in the setting of radiation has never been reported. Here, we report the successful use of the retroflexed gracilis muscle flap for femoral vessel coverage after superficial inguinal lymphadenectomy, in a patient who previously underwent chemoradiation for locally-metastatic anal squamous cell carcinoma to the groin. An 86-year old female presented with Stage IIIB anal squamous cell carcinoma metastatic to one left inguinal lymph node. She underwent modified Nigro protocol chemoradiation treatment, which included radiation to the inguinal node basins. A left superficial inguinal lymphadenectomy was performed with a retroflexed gracilis muscle flap to cover the femoral vessels. This was chosen over a sartorius flap because the gracilis muscle was not located within the field of radiation. Despite a subsequent groin wound infection, the gracilis muscle flap remained viable and successfully protected the major vessels. We report the gracilis muscle flap as a viable alternative to the sartorius transposition muscle flap for femoral vessel coverage after oncologic superficial inguinal lymphadenectomy in the irradiated groin.
Reconstruction after combined cardia resection and removal of the gastroesophageal junction can be carried out by the Merendino procedure or via a gastric conduit. This study compares postoperative complications and quality of life for both approaches.

All patients who underwent Merendino or gastric conduit reconstruction from 2011-2017 were included. Both groups were investigated regarding postoperative length of stay, complications, and gastrointestinal quality of life.

45 patients were identified, of which, 39 remained for analysis 22 patients in the Merendino group and 17 patients in the gastric conduit group. The median age of patients in the gastric conduit group (71 (53-92)years) was significantly higher than in the Merendino group (58 (19-75)years),

However, the large-scale commercialization of LIBs is impeded significantly by the degradation of the electrolyte, which reacts with the highly reactive lithium metal anode. Future improvement of the battery performance requires a knowledge of the reaction mechanism that is responsible for the degradation and formation of the solid-electrolyte interphase (SEI). In this work, we develop a hybrid computational scheme, Hybridab initiomolecular dynamics combined with reactive force fields, denoted HAIR, to accelerate Quantum Mechanics-based reaction dynamics (QM-MD or AIMD, for ab initio RD) simulations. The HAIR scheme extends the time scale accessible to AIMD by a factor of 10 times through interspersing reactive force field (ReaxFF) simulations between the AIMD parts. This enables simulations of the initial chemical reactions of SEI formation, which may take 1 ns, far too long for AIMD. We apply the HAIR method to the bis(trifluoromethanesulfonyl)imide (TFSI) electrolyte in 1,3-dioxolane (DOL) solvent at the Li metal electrode, demonstrating that HAIR reproduces the initial reactions of the electrolyte (decomposition of TFSI) previously observed in AIMD simulation while also capturing solvent reactions (DOL) that initiate by ring-opening to form such stable products as CO, CH2O, and C2H4, as observed experimentally. These results demonstrate that the HAIR scheme can significantly increase the time scale for reactive MD simulations while retaining the accuracy of AIMD simulations. This enables a full atomistic description of the formation and evolution of SEI.
