Deep learning can be used to classify large amounts of smoking data from the medical narrative with good accuracy. The results highlight the detrimental effects of persistent smoking in oncologic patients and emphasize that smoking cessation should always be an essential element of patient counseling.Medullary thyroid cancer (MTC) represents a rare neuroendocrine neoplasm originating from neoplastic C-cells in the thyroid gland. While localized disease is potentially curable with an optimized surgical approach, the number of relapses is high, and a considerable number of patients present with primary metastatic disease. Multidisciplinary management including standardized surveillance following surgery, but also early involvement of medical oncologists, is therefore important. Several oncogenic pathways are involved in the pathogenesis of MTC including vascular endothelial growth factor receptor, epidermal growth factor receptor, MET, and most importantly RET, and the multi-tyrosine kinase inhibitors vandetanib and cabozantinib have been approved for advanced MTC based on data from phase III studies. As activating RET mutations represent the most important driver, specific RET inhibitors were introduced and suggest high response rates with limited off-target toxicities. The current review provides a practical overview on clinical presentation and management from early to advanced MTC.Nanobiomaterials (NBMs) are a special category of nanomaterials used in medicine. As applications of NBMs are very similar to pharmaceuticals, their environmental release patterns are likely similar as well. Different pharmaceuticals were detected in surface waters all over the world. Consequently, there exists a need to identify possible NBM exposure routes into the environment. As the application of many NBMs is only carried out at specific locations (hospitals), average predicted environmental concentrations (PECs) may not accurately represent their release to the environment. We estimated the local release of poly(lactic-co-glycolic acid) (PLGA), which is investigated for their use in drug delivery, to Swiss surface waters by using population data as well as type, size and location of hospitals as proxies. The total mean consumption of PGLA in Switzerland using an explorative full-market penetration scenario was calculated to be 770 kg/year. 105 hospitals were considered, which were connected to wastewater treatment plants and the receiving water body using graphic information system (GIS) modelling. The water body dataset contained 20,167 river segments and 210 lake polygons. Using the discharge of the river, we were able to calculate the PECs in different river segments. While we calculated high PLGA releases of 2.24 and 2.03 kg/year in large cities such as Geneva or Zurich, the resulting local PECs of 220 and 660 pg/l, respectively, were low due to the high river discharge (330 and 97 m3/s). High PLGA concentrations (up to 7,900 pg/l) on the other hand were calculated around smaller cities with local hospitals but also smaller receiving rivers (between 0.7 and 1.9 m3/s). Therefore, we conclude that population density does not accurately predict local concentration hotspots of NBMs, such as PLGA, that are administered in a hospital context. In addition, even at the locations with the highest predicted PLGA concentrations, the expected risk is low.Microplastics ( less then 5 mm) are ubiquitous in the global environment and are increasingly recognized as a biological hazard, particularly in the oceans. Zooplankton, at the base of the marine food web, have been known to consume microplastics. However, we know little about the impacts of microplastics across life history stages and on carbon settling. Here, we investigated the effects of ingestion of neutrally buoyant polystyrene beads (6.68 μm) by the copepod Acartia tonsa on (1) growth and survival across life history stages, (2) fecundity and egg quality, (3) and fecal characteristics. We found that microplastic exposure reduced body length and survival for nauplii and resulted in smaller eggs when copepods were exposed during oogenesis. Combining these life history impacts, our models estimate a 15% decrease in population growth leading to a projected 30-fold decrease in abundance over 1 year or 20 generations with microplastic exposure. In addition, microplastic-contaminated fecal pellets were 2.29-fold smaller and sinking rates were calculated to be 1.76-fold slower, resulting in an estimated 4.03-fold reduction in fecal volume settling to the benthos per day. this website Taken together, declines in population sizes and fecal sinking rates suggest that microplastic consumption by zooplankton could have cascading ecosystem impacts via reduced trophic energy transfer and slower carbon settling.Commercially available bovine-specific assays are limited in number, and multiplex assays for this species are rare. Our objective was to develop a multiplex assay for the bovine inflammatory cytokines IL-1β, IL-6, and TNF-α using the Meso Scale Discovery U-PLEX platform. "Do-It-Yourself" ELISA kits that contained polyclonal antibodies, both unlabeled and biotinylated, and the specific recombinant bovine cytokine standard, were purchased for each of these three cytokines. The biotinylated antibodies were coupled to linkers that bind to specific locations within each well of the U-PLEX plate. Unique linkers were used for each of the cytokines. The unlabeled antibodies were conjugated with electrochemiluminescent labels to serve as detection antibodies. Each cytokine assay was optimized individually prior to performing an optimization on the multiplex assay containing reagents for all three cytokines. To calculate cytokine concentrations, standard curves were developed using the recombinant cytokines and were run concurrently on each plate. Standard curves for IL-1β and TNF-α were run at concentrations ranging from 0 to 50,000 pg/mL, and for IL-6 from 0 to 10,000 pg/mL. The average lowest level of detection concentration measured by the standard curves were 5.3 pg/mL, 0.92 pg/mL, and 22.34 pg/mL for IL-1β, IL-6, and TNF-α respectively, as determined by data from seven plates containing bovine plasma samples from a combination of healthy and diseased cattle. The U-PLEX platform was a viable means to develop custom analyte- and species-specific multiplex assays using privately developed or purchased sets of commercially available reagents.
