Peroneal tendon disorders remain an often overlooked source of lateral hindfoot pain and tenderness. Medical professionals who specialize in foot and ankle care are still far more likely to diagnose a peroneal tendon disorder as an ankle injury. This report presents a case of successful lower leg reconstruction with free flap using a retrograde recipient vein. A 43-year-old woman had a mass lesion in the lateral side of the right calf, which was diagnosed as a malignant granular cell tumor. During wide excision of the tumor, the anterior tibial artery (ATA) and veins were ligated and resected at the bifurcation point from the popliteal artery and veins because the tumor abutted on the ATA near the bifurcation. The skin defect measured 17 × 7 cm with the proximal part of the tibia and lateral condyle exposed. A 19 × 7.5 cm anterolateral thigh flap was elevated, and anastomoses were performed using the distal end of the ATA and anterior tibial vein as retrograde recipient vessels. The flap was well perfused, and no signs of venous insufficiency were observed. Complete survival with no postoperative complications was achieved. In free tissue transfers of the lower leg, retrograde venous flow can be considered as a recipient vein when an antegrade vein is not available or has risks of venous insufficiency.A new depside, olivieridepside (1), and a new secoiridoid, olivierigenin (2) were isolated from the aerial parts of Gentiana olivieri Griseb. along with four known compounds, gentiopicroside (3), olivierosides A (4) and B (5) and isoorientin (6). The structures of the isolates were determined by extensive 1 D and 2 D NMR spectroscopy and HR-MS analysis. This is the first report on the occurrence of a depside structure in the genus Gentiana. Moreover, a rare type of non-glycosidic secoiridoid (2) lacking an oxygenated group at C-1 is also being reported for the first time from this genus. The chemotaxonomic importance of the isolates was discussed in detail.
Patients hospitalized for severe coronavirus disease 2019 (COVID-19) infection are at risk for in-hospital cardiac arrest (IHCA). It is unknown whether certain characteristics of cardiac arrest care and outcomes of IHCAs during the COVID-19 pandemic differed compared with a pre-COVID-19 period.

All patients who experienced an IHCA at our hospital from March 1, 2020 through May 15, 2020, during the peak of the COVID-19 pandemic, and those who had an IHCA from January 1, 2019 to December 31, 2019 were identified. All patient data were extracted from our hospital's Get With The Guidelines-Resuscitation registry, a prospective hospital-based archive of IHCA data. Baseline characteristics of patients, interventions, and overall outcomes of IHCAs during the COVID-19 pandemic were compared with IHCAs in 2019, before the COVID-19 pandemic.

Mitral valve-in-valve (ViV) and valve-in-ring (ViR) are alternatives to surgical reoperation in patients with recurrent mitral valve failure after previous surgical valve repair or replacement. Our aim was to perform a large-scale analysis examining midterm outcomes after mitral ViV and ViR.

Patients undergoing mitral ViV and ViR were enrolled in the Valve-in-Valve International Data Registry. link2 Cases were performed between March 2006 and March 2020. Clinical endpoints are reported according to the Mitral Valve Academic Research Consortium (MVARC) definitions. Significant residual mitral stenosis (MS) was defined as mean gradient ≥10 mm Hg and significant residual mitral regurgitation (MR) as ≥ moderate.

Everolimus (EVR) has been approved for the treatment of various advanced cancers and its indications are increasingly expanding. Therefore, it is crucial for patients who have difficulty in swallowing, such as pediatric and elderly patients, to obtain a convenient formulation. The oral absorption of EVR is limited due to its low solubility in water, intestinal metabolism by CYP3A4 enzyme, P-gp-mediated efflux, and metabolism in the liver. The aim of this study was to develop a novel sublingual orodispersible film loading everolimus for improving patient compliance and enhancing oral bioavailability of EVR.

Sublingual orodispersible films loading EVR were prepared by the solvent casting method and evaluated by in vitro and in vivo studies.

The properties of films were determined by scanning electron microscopy, differential scanning calorimetry, X-ray diffraction, and Fourier-transform infrared spectrometry. The addition of acacia gum appeared to be crucial for protecting the drug from oxidation. Pharmacokinetic studies showed that loading into the sublingual orodispersible films significantly increased the oral bioavailability of EVR.

Night shift workers are at cardiometabolic risk due to circadian misalignment. EI1 molecular weight investigated whether infrequent exercise before each night shift that intentionally would not improve physical performance improves glucose tolerance and 24-h blood pressure profiles and synchronizes circadian rhythms of melatonin and cortisol in rotating night shift workers.

A total of 24 rotating night shift workers (mean age, 35.7 ± 11.8 years) were randomized to exercise or no intervention. Workers in the exercise group performed 15.2 ± 4.5 exercise sessions within 2 h before each night shift. Before and after 12 weeks of exercise intervention and 12 weeks after the intervention, spiroergometry, oral glucose tolerance testing and 24-h blood pressure profiles were performed. Plasma melatonin and cortisol levels were measured in 3-hourly intervals during one 24-h period on each study day.

Exercise did not significantly change serum glucose nor insulin levels during oral glucose tolerance testing. Timed physical exercise had no effect on physical performance, nor did it change the circadian rhythms of melatonin and cortisol or influence 24-h blood pressure profiles.

All the tested essential oils were characterized by a high content of monoterpenes (oxygenated 49.2-80.3%; hydrocarbons 18.7-48.3%). Among them, 1,8-cineole (26.4-49.1%), α-pinene (4.5-34.8%), camphor (1.1-18.8%) and borneol (1.7-16.2%) were the main constituents. The high amount of 1,8-cineole/α-pinene/camphor/borneol may suggest the presence of an intermediate rosemary chemotype. Statistical analysis was also performed on the essential oil (EO) composition evidenced an expected difference depending on the collection seasons, to the geographical areas and soil composition. A comparison with the essential oil composition from S. rosmarinus collected in all the other islands of Tuscan Archipelago (Italy) was also reported, together with a Multivariate Statistical Analysis.
