Many marine organisms, from invertebrates to fish to marine mammals, use acoustic cues to find food, avoid predators, choose mates, and navigate. Ocean noise can affect animal behavior and disrupt trophic linkages. Substantial potential exists for area-based management to reduce exposure of animals to chronic ocean noise. Incorporating noise into spatial planning (e.g., critical habitat designation or marine protected areas) may improve ecological integrity and promote ecological resilience to withstand additional stressors. Previous work identified areas with high ship noise requiring mitigation. This study introduces the concept of "opportunity sites" - important habitats that experience low ship noise. Working with existing patterns in ocean noise and animal distribution will facilitate conservation gains while minimizing societal costs, by identifying opportunities to protect important wildlife habitats that happen to be quiet.Few studies report trace elements in dwarf sperm whale (Kogia sima). As high trophic level predators, marine mammals are exposed through diet to environmental contaminants including metals from anthropogenic sources. Inputs of Hg, Pb, and Cd are of particular concern due to toxicity and potential for atmospheric dispersion and subsequent biomagnification. Liver and kidney tissues of stranded K. sima from coastal South Carolina, USA, were analyzed for 22 trace elements. Age-related correlations with tissue concentrations were found for some metals. Mean molar ratio of HgSe varied with age with higher ratios found in adult males. Maximum concentrations of Cd and Hg in both tissues exceeded historical FDA levels of concern, but none exceeded the minimum 100μg/g Hg threshold for hepatic damage. Tissue concentrations of some metals associated with contamination were low, suggesting that anthropogenic input may not be a significant source of some metals for these pelagic marine mammals.This paper aims to evaluate the feasibility of applying LiFMg,Ti microdosimeters as a new method of dosimetry to volumetric-modulated arc therapy (VMAT) RapidArc. The response of microdosimeters presented a maximum variation of ±3.18% and ±0.510% using optically stimulated luminescence (OSL) and thermoluminescence (TL) techniques, respectively. Although studies were conducted on LiFMg,Ti microdosimeters previously, the microdosimeters in this study showed precision and high potential of application in VMAT dosimetry and in the verification of treatment planning using the VMAT technique.
The aims of this article were to review the rationale behind the development of combined angiotensin receptor/neprilysin inhibitors (ARNIs) for the management of chronic heart failure (HF) and to review the major clinical trials of LCZ696, the first drug in this class, that have been conducted to date.
A selected review was undertaken of publications examining the preclinical and clinical studies of drugs aimed at enhancing the activity of the endogenous natriuretic peptide system and their combination with inhibitors of the renin-angiotensin-aldosterone system, initially angiotensin-converting enzyme inhibitors (ACEIs) and more recently angiotensin II type 1 receptor blockers.
Selective neprilysin inhibitors are unlikely to be of benefit and may be associated with adverse effects when used in isolation in HF. Combining NIs with ACEIs is unsafe because of an unacceptably high prevalence of angioedema, which may be mediated by elevated levels of endogenous bradykinin. Combining a neprilysin inhibitor with an angiotensin II type 1 receptor blockers avoids the risk for angioedema. The ARNI LCZ696 was associated with greater reductions both mortality and morbidity compared with those with enalapril in a large-scale, Phase III clinical trial in patients with HF with reduced ejection fraction. Findings from a Phase II clinical trial suggested that LCZ696 may also be beneficial in HF with preserved ejection fraction, and a Phase III clinical trial of LCZ696 used for this indication is under way.

The aims of this review were to critically re-examine the evidence base and to highlight recent refinements in device therapy in heart failure.
Significant contemporary clinical trials and registries of device therapy in heart failure were examined and critically reviewed to draw conclusions on the clinical applications of implantable cardioverter-defibrillators, cardiac resynchronization therapy, remote monitoring of devices, and hemodynamic monitoring.
Advances regarding patient selection, technology, and implementation for the use of devices in heart failure have significantly improved outcomes.

The role of devices continues to increase, and devices have a positive impact on patients' quality of life and survival.The transformation of recorded electrocardiographic leads (source leads) into leads that are wanted but were not recorded (target leads) has many practical applications. In general, two transformation methods are put to use, a purely statistical one and a model-based one. They are briefly reviewed and compared. Lead transformations were first used in the early nineteen-sixties to transform the component leads of one vectorcardiographic lead system into those of another. Since then, the use of lead transformations has proliferated and they are currently applied for a variety of purposes. Lead transformations can be grouped according to the source and target leads that are involved. A few applications of lead transformations from the different groups are presented, with a focus on the practicality of the application. The validity and value of the dipole approximation in relation to lead transformations is discussed.We report here a case with unidirectional abnormalities of smooth eye movements without gaze nystagmus. Abnormalities of eye movements were confined to unidirectional (leftward) horizontal pursuit and slow phase of OKN; however, horizontal VOR (slow phase of caloric nystagmus) and saccade were normal, and vertical eye movements were also normal. No lesions were detected in the central nervous system, and any history of drug intake was denied. Although the cause of the unidirectional abnormality in eye movement of this case is still not clear, a congenital origin seems to be the most probable.
The aim of the study is to evaluate the usefulness of multislice computed tomography (MSCT) using multiplanar reconstruction (MPR) in obtaining preoperative information on the ossicular lesions of middle ear diseases by comparing the ossicular findings of MPR images with the operative findings.
Sixty-two ears and 10 ears with preoperative middle ear diseases underwent 4- and 64-detector row CT of the temporal bone in Kagawa University Hospital, respectively. MPR images of three ossicles were created at the planes parallel to the long axis of ossicles.
The findings of the three ossicles in MPR images were compatible with their operative findings in approximately 91% of 72 ears with various middle ear diseases. There was no significant difference in the coincidence rate of both findings between 4- and 64-detector row CT scanners. The ears with no soft tissue shadows around the ossicles had the coincidence rate of 96-100% in each ossicular part, whereas the coincidence rate was lower in the ears with soft tissue shadows around the ossicles.
MPR imagings of the ossicles provide accurate preoperative information on the ossicular lesions in middle ear diseases. The 4-detector CT is still a useful device for imaging of the ossicles.

Several studies correlated between vitamin D deficiency and the development, and the recurrence of benign positional paroxysmal vertigo (BPPV), but none of them proved that treatment of vitamin D deficiency would reduce the recurrence rate of BPPV. This study aims to detect the effect of treatment of severe vitamin D deficiency on the recurrence rate of BPPV.
The inclusion criteria of the study group were (1) Unilateral, idiopathic, posterior canal BPPV with no history suggestive of secondary BPPV and (2) 25-hydroxyvitamin D3 level ≤10 ng/ml. All subjects enrolled in the current study underwent detailed clinical history, audiovestibular evaluation consisting of pure-tone audiometry, Immittancemetry, Videonystugmography, serum 25-hydroxyvitamin D3 assessment, and Dual-energy X-ray absorptiometry (DXA). Vitamin D therapy was prescribed for the study group. Serum 25-hydroxyvitamin D3 level was evaluated twice, on recruitment into the study group and 3 months after commencing vitamin D therapy. According to t CI 1.41-14.58, p<0.01). The relapse attacks of BPPV affected both ears irrespective of the ear showing the original BPPV attack.
The present study indicates that improvement of serum 25-hydroxyvitamin D3 levels is associated with substantial decrease in recurrence of BPPV.

To evaluate the feasibility and safety of multimodal perioperative care after laparoscopic hysterectomy.
A prospective observational study was performed at Clara Campal Comprehensive Cancer Center, Madrid, Spain, between April 1, 2011, and July 30, 2014, and included women who were scheduled to undergo a laparoscopic hysterectomy. Fast-track (multimodal) care included provision of full preoperative verbal and written information, intravenous dexamethasone (8 mg) during surgery, removal of the urinary catheter at the end of surgery, and early mobilization and solid food intake. The main outcome was the length of hospital stay.
Overall, 88 patients were included. The median hospital stay was 22.7 hours (range 5-72). Same-day discharge was feasible in 24 (27%) patients. A total of 51 (58%) women were discharged after overnight stay, 46 (90%) of whom elected to do so because of personal preference. Finally, 13 (15%) patients were discharged at least 36 hours after surgery. Eleven (13%) women visited the emergency unit after discharge and 3 (3%) were readmitted.
