Retinal detachment (RD) describes the separation of neurosensory retina from the underlying pigment epithelium. There are various methods of treating RD but in many cases, an unusual delay between occurrence of retinal detachment and surgery has been observed. This study was conducted to find the extent of factors involved in delay in surgery. This cross sectional study was carried out at LRBT Eye Hospital, Lahore for 6 months. The non-probability, consecutive sampling technique was used. The demographic information was recorded. The mean age of patients was 52±9.86 years; the male to female ratio was 1.51. Statistically significant difference was found between the factors and education level of the patients i.e. p-value less then 0.05. Our study results concluded that people needed to be educated regarding the importance of retinal detachment and surgical procedures and complications associated with it.At present, the mechanism for clopidogrel resistance (CR) is incompletely understood. Here, we aimed to analyze of the association of plasma concentration of clopidogrel active metabolites (CAM) and CYP2C19 genetic polymorphism with CR. We assigned 77 patients to receive CLP at a loading dose of 300mg on day 1, followed by 75mg per day from day 2 to day 6. Three peripheral venous blood samples were collected for analysis. Our results showed that plasma concentration of CAM in extensive metabolizers (EMs) group (2.48(1.31, 5.67) ng/mL) was higher than that in intermediate metabolizers (IMs) group (1.44(1.18,3.55) ng/mL) and that in poor metabolizers (PMs) (1.18(1.12,1.33) ng/mL) group was the lowest (H=14.58, P=0.001). Besides, the incidence of CR in EMs group(11.1%) was lower than that in IMs group (20.0%) and that in IMs group was lower than that in PMs group (45.5%) (χ2=6.344, P=0.042). In addition, our findings confirmed that the incidence of chest tightness in IMs group (40.0%) and PMs group (50.0%) was higher than that in EMs group (9.1%) (P=0.015). Over the follow-up period, it was found that CYP2C19 and plasma concentration of CAM were related to the incidence of chest tightness. Our findings indicated that in addition to CYP2C19, plasma concentration of CAM may be an important factor in predicting CR.Almost all plants contain polyphenols. Literature shows that polyphenols exhibit many biological activities. Little has known about their protective effects against hypoxia-induced lethality. The protective effects of rutin (1) and chlorogenic acid (2) against hypoxia conditions in mice were determined by three different experimental models. Antihypoxic activity was especially pronounced in asphytic hypoxia. Both compounds (1&2) showed statistically significant (p>0.05) activities respect to the control. Compound (1) significantly prolonged the latency for death with respect to control (39.20±8.70 vs. 13.20±2.58min, p0.05). None of two phenolic acids had any activity in haemic hypoxia when compared to control.This study aimed to investigate the effects of mesenchymal stem cells (MSCs) on the biomechanics of stress-fractured rabbits after healing. Twenty-Four New Zealand white rabbits were selected to establish a stress fracture model. All rabbits were randomly divided into model group and MSCs group, and another 12 rabbits were chosen as control group. Compared with the control group, the mRNA expression levels of basic fibroblast growth factor (b-FGF), vascular endothelial growth factor (VEGF), osteoprotegerin (OPG) and receptor activator of nuclear factor-κ B ligand (RANKL) in the model group decreased significantly, and the OPG/RANKL ratio decreased (p less then 0.05). Compared with the model group, the mRNA expression levels of b-FGF, VEGF, OPG and RANKL in the MSCs group were significantly increased, and the OPG/RANKL ratio increased (p less then 0.05). Compared with the model group, the healing and bone mineral density in the MSCs group increased significantly (p less then 0.05), the biomechanical parameters (maximum load, elastic load, stiffness and maximum deflection) of the rabbits in the MSCs group were significantly improved (p less then 0.05). In conclusion, MSCs can significantly improve the bone mineral density of stress-fractured white rabbits; promote the release of bone formation factors and fracture healing, which can provide guidance for clinical treatment of stress fractures.The resistance of Acinetobacter baumannii to most antibiotics is increasing. The presence of metallo-beta-lactamase and carbapenemase enzymes has led to the resistance of these bacteria to carbapenems as one of the major classes of broad-spectrum antibiotics and has raised concerns in human societies. This research evaluated the presence of blaOXA-51, blaOXA-58 and blaOXA-23 genes in A. baumannii strains during a 12 months period. One hundred strains were isolated from the patients hospitalized in ICU of Ali Asghar and Shahid Rajaee trauma hospitals in Shiraz. Bacterial identity was determined by biochemical tests and antibiotic resistance was determined by disk diffusion method. The isolated strains were then evaluated in terms of carrying blaOXA-23, blaOXA-51 and blaOXA-58 genes, using the multiplex PCR method. The results showed that A. baumannii was resistant to carbapenems but most strains were susceptible to tigecyclin and colistin. The majority of strains carried the blaOXA-23 and blaOXA-51 genes, but very few carried the bla OXA-58 gene. The results revealed that the antibiotic resistance of A. baumannii is increasing, which causes a more outbreak of this organism.To study the effects of sinomenine on conditioned place preference (CPP) zebrafish induced by morphine and expression levels of intracephalic tyrosine hydroxylase (TH), NMDA receptor subunit 2B (NR2B), μ-opioid receptor (zfmor) and δ-opioid receptors (zfdor1 and zfdor2), morphine (40mg/kg) was administrated to zebrafish and the effect of CPP was detected in these zebrafish treated with sinomenine. The expression of TH and NR2B was detected by immunohistochemistry; and the mRNA expression of opioid receptors zfmor, zfdor1 and zfdor2 in the zebrafish brain was assayed by RT-qPCR. In the CPP test, morphine induced significant behavioral alteration, while pretreatment with sinomenine or methadone, resulted in decreased activity time in the morphine-paired compartment significantly. Morphine also increased the integral optical density value of TH- and NR2B-positive cells in the zebrafish brain, and reduced the amount of opioid receptors. However, the compound sinomenine could attenuate these effects. These findings demonstrate that sinomenine (80mg/kg) decreased the CPP effects of zebrafish induced by morphine significantly, downregulated expression of TH and NR2B, and upregulated μ-opioid (zfmor) and δ-opioid (zfdor1 and zfdor2) receptor expression in the CPP zebrafish brains.The objective of this study was to synthesize pure ZnO and metal doped ZnO nano-particles, determine its physical, chemical characteristics and antibacterial activity against selected bacterial strains. Pure ZnO was synthesized and metals including Manganese (Mn), Magnesium (Mg), Calcium (Ca), Copper (Cu) and Silver (Ag) were doped with ZnO to produce nanoparticles through co-precipitation method. X-ray diffraction (XRD), scanning electron microscopy (SEM) and Fourier transform infrared (FTIR) spectroscopy were used for detection of synthesized nanoparticles, their crystalline structures, size and other chemical characteristics. An altered version of Kirby Bauer method of disk diffusion was used for determining the antibacterial activity of these nanoparticles. The XRD results showed that the average size of pure ZnO nanoparticles was 55.3 nm. While the size of metal doped ZnO particles were affected by dopant elements. Results of SEM indicated that these nanoparticles were roughly spherical, rod shape and fiber like rod shape with certain degree of aggregation. Antibacterial studies showed that all samples had the potential to inhibit the growth of selected bacterial strains; E. coli, S. choleraesuis, S. typhimurium, S. marcescens, B. subtilus and S. aureus. With 90μg/ml concentration ZnO nanostructures inhibited B.subtilus and silver doped Zinc nanoparticles suppressed growth of S. marcescens. Characterization and antibacterial study indicated the importance of these nanoparticles at industrial and pharmaceutical level.Severe acute respiratory viral infections are frequently associated with multiple organ failure, including acute kidney damage. The present study aimed to investigate the associated influence of COVID-19 on renal function in patients admitted to the intensive care unit in Asir region, Saudi Arabia. Thirty patients infected with COVID-19 who were referred to the intensive care unit during November and October 2020 at Asir central hospital, Asir region, Saudi Arabia were recruited. The age of patients ranged between 30 and 90 years old. Renal function tests exhibited dramatic changes in the renal biomarkers in patients with COVID-19. Blood urea levels in COVID-19 patients were significantly higher than in the control group. In addition, significantly lower albumin levels with abnormally decreased total protein levels were found in COVID-19 patients. Among the different electrolytes analyzed, a significantly lower calcium level was observed in COVID-19 patients' groups than in the controls. Renal function tests for COVID-19-infected ICU patients revealed significant changes, indicating the major impact of COVID-19 on kidney function. Monitoring renal function tests may assist in the early prognosis of COVID-19 patients. It is, therefore, crucial to increase the understanding of renal function tests in COVID-19 patients who were admitted to the hospital before their condition deteriorated.
Botulinum toxin A is the first-line treatment for localised spasticity. However, the economic impact of this treatment is not fully known. This study aimed to describe the real costs of botulinum toxin A for the treatment of adult patients with spasticity at a spasticity clinic pertaining to a rehabilitation service, over a period of one year.
We retrospectively reviewed all medical procedures carried out during the year 2017. We collected data on the type of toxin used (incobotulinumtoxin A, onabotulinumtoxin A, or Abobotulinumtoxin A), the number of units injected, the anatomical region, and the time elapsed between infiltrations. The costs of medication and indirect costs, such as staff and consumables, were also calculated.
This is the first study to describe the real costs of botulinum toxin treatment of spasticity in adult patients in Spain. In 2017, 510 infiltration procedures were performed in 164 patients. The total cost of treating spasticity in our service was €116 789.70. The mean annual cost per patient was €603.
