The most diverse cluster was defined by oropharyngeal-like flora (OF) while the others were characterized by the dominance of a single pathogen. Patients with the diverse OF microbiota cluster had lower sputum inflammatory markers and higher FEV1% predicted compared to patients with a pathogen-dominated microbiota including Pseudomonas aeruginosa. Our results suggest that the diversity of the airway microbiota is an important biomarker of the severity of airway inflammation linking dysbiosis to lung function decline in patients with CF.
Myxoinflammatory fibroblastic sarcoma (MIFS) is a rare low-grade sarcoma presenting as a slow-growing mass that occurs mainly in the distal extremities of adults. Relatively little is known about the cytopathology of MIFS. We evaluated cytologic characteristics of MIFS on fine-needle aspiration (FNA).
A search was made of our cytopathology and surgical pathology databases for cases diagnosed as MIFS. FNA biopsy smears and cell-block were performed and examined using standard technique.
Eight cases were retrieved from patients aged 22-90 years (mean, 56 years), and MF ratio of 11. Six tumors (75%) were primary, and 2 (25%) locally recurrent. Distal lower limb was involved in all but one case (88%). One (13%) recurrent case was correctly diagnosed cytologically as MIFS; remaining single diagnoses were varied myxofibrosarcoma, low-grade sarcoma, malignant neoplasm, myxoid neoplasm, atypical fibrohistiocytic neoplasm, atypical cells with chronic inflammation, and spindle cells with atypia. Among 7 cases with available cytologic slides for review, common features were spindle cells with variable atypia (100%), rare virocyte/Reed-Sternberg -like cells (86%), background mixed inflammation (71%), and variable myxoid stroma (57%). Pseudolipoblasts and multinucleated giant cells were rare. Hemosiderin and branching capillaries were largely absent. Immunohistochemistry was non-specific.

In this report, we describe 2 siblings who acquired influenza during travel on a world cruise ship and were treated with decapsulated oseltamivir. The siblings' mother was instructed to decapsulate a 75 mg oseltamivir capsule, suspend the powder in 15 mL of water (5 mg·mL-1), stir well, and administer the required amount of medicine orally to each patient using a syringe. Both patients recovered successfully with no complications. The presented case suggests that suspending decapsulated oseltamivir in water and measuring the required amount with a syringe for orally administration to children with influenza can be a safe treatment strategy in resource-limited settings.
Animal-related fatal accidents occur annually while individuals are outside working. This study intended to evaluate the trends of occupational fatal accidents related to mammals and compare them with nonoccupational accidents.
We collected occupational and nonoccupational fatalities related to mammals between 2000 and 2019 from data on fatal occupational accident cases certified by the Ministry of Health, Labor and Welfare, Japan, in addition to other statistics from relevant ministries, media reports such as newspapers, and academic case reports.
A total of 109 fatal accidents related to mammals were confirmed as occurring during work, and 129 were confirmed as taking place outside of work. Of the former, accidents among livestock farmers and horse racing-related workers accounted for about half. Among the accidents not related to work, dog attacks, infectious diseases from cats or dogs, traffic accidents with wild animals, and bear attacks during wild vegetable picking were common. Deaths from infectious diseases caused by dogs and cats have rarely been reported in the mass media.
Although the number of fatal occupational accidents related to mammals is not large, we concluded that it is necessary to continue preventive measures centered on accidents at livestock farms and in horse racing-related workplaces. It is also necessary to inform people about the risks of infectious diseases mediated by dogs and cats.
