The main causes of readmission are graft rejection and infections, and they had similar incidence in the age groups we analyzed.The ABO incompatible (ABOi) living donor (LD) kidney transplant allows increasing the number of donors and reducing the time on the waiting list. The objectives of this study were to compare graft survival, patient survival, rejection risk factors and complications during the first year p ost-transplantation in patients who received an ABOi LD kidney transplant between 2014 and 2019 in our institution, matched according to sex, age and immunological risk with a control group of ABO compatible (ABOc) LD kidney transplants in the same period. Thirteen patients were included in each group. No significant differences were found between ABOi and ABOc in the incidence of delayed graft function (n = 0 vs. 1), bleeding (0 vs. 0), infections (13 vs. 13), cellular rejection (1 vs. 3) and humoral rejection (4 vs. 3) in the first year after transplantation. The rejection rate in ABOi do not seem to be related to blood incompatibility. No risk factors associated with rejection were found. Overall survival of patients was 100% in both groups, and graft survival was 92.3% in ABOi and 100% in ABOc (p = 1). ABOi kidney transplantation is an adequate feasible option in our environment for those who do not have compatible donors.Although ST-segment elevation myocardial infarction (STEMI) mortality decreased with the progress of reperfusion, the incidence of hospital complications has not changed. We aimed to describe the incidence of STEMI complications in the coronary unit, the timing of their occurrence and to identify their predisposing and protective factors. This is a prospective analysis of all consecutive patients with STEMI admitted to a coronary care unit of a third level reference hospital from September 2017 to March 2020. Of the 263 STEMI, 124 developed complications (47.2%), and the most frequent was heart failure. In the multivariate analysis, preadmission cardiac arrest (CA) (OR 9.8; CI 1.2-81.9; p = 0.03), left ventricular ejection fraction (Fey VI) 68 years suffered an event. Almost all complications happened within the first 48 hours.The atypical clinical features in multiple sclerosis (MS) have been rarely reported and suggest the possibility of an alternative diagnosis. The aim was to describe the clinical and demographic characteristics of MS patients who debuted with atypical symptoms and to estimate the sensitivity, specificity and positive predictive value (PPV) for MS diagnosis. A retrospective analysis of clinical records was performed. The following data were recorded patients with MS diagnosis according to current diagnostic criteria at the time of diagnosis, type of symptom at the onset, time to second relapse, presence of oligoclonal bands (OCB) in cerebrospinal fluid (CSF) and radiological red flags on MRI. A descriptive and inferential analysis was performed using the chi square test, and sensitivity, specificity and PPV were calculated. Six hundred two patients were diagnosed with MS, of which 22 (3.65%) had an atypical clinical presentation. 54.5% were women. The mean age was 29 years (SD ± 11.7). The most common atypical symptom was peripheral facial palsy (27%). The PPV for atypical onset was 6.14%; p less then 0.001. Sensitivity and specificity of these symptoms to MS diagnosis were 3.65% and 19%, respectively. In our research, the presence of atypical symptoms at the onset of MS was very low. Other diseases must be excluded, taking into account their low sensitivity, specificity and PPV.Juvenile myoclonic epilepsy (JME) is a benign disorder with a good response to antiepileptic drugs. Neuropsychological evaluations revealed mild cognitive deficits. find protocol The objective of this study is to determine the cognitive profile and mood symptoms in JME compared to normal controls. 30 patients with JME and 29 controls matched for age, gender, and education level were prospectively evaluated. The clinical characteristics were analysed. They were given a complete cognitive battery, a self-administered questionnaire of executive difficulties (DEX), the Neurological Disorders Depression Inventory for Epilepsy (NDDI-E), Beck Depression Inventory (BDI), Generalized Anxiety Disorder Scale (GAD-7) and suicide risk scale (MINI). No significant differences in age and education were observed between JME and controls. Average time of evolution of the disease 18 years, 53% have three types of seizures myoclonic, absence seizures and tonic-clonic seizures. Significant differences were found with greater failures in attention, executive function, a significantly higher score values in DEX in JME subjects. A higher score was found in the NDDI-E; BDI and GAD-7. No differences were found in the risk of suicide with respect to controls. The study confirms that JME presents greater failures in attentional functioning and executive skills related to flexibility and inhibition, with patients being aware of their difficulties in most cases. Knowing these difficulties would allow a better therapeutic approach to improve symptoms usually dismissed.Sarcopenia is the loss of muscle mass and function. Its presence darkens the prognosis of cirrhotic patients. The gold standard for the description of sarcopenia is the Skeletal Muscle Index (SMI). Thirty-four cirrhotic patients were included. Measurement was carried out through CT of both psoas separately that were then added obtaining the so called "sum of area of 2 psoas", in addition total muscular area, SMI and total area of psoas were measured. Ultrasonography was also performed and the transverse area of the quadriceps rectus anterior was measured by ultrasound and Hand Grip (HG). The correlation between the different measurements was analyzed comparing with the total muscle area, with quadriceps area (r = 0.39; p = 0.019), with total psoas area (r = 0.71; p less then 0.01), with sum of area of 2 psoas (r = 0.72; p less then 0.001) and with HG (r = 0.45; p = 0.0069). Compared with SMI, in women HG had 86% sensitivity and 66% specificity (AUC = 0.89). In men, the sum of 2 psoas had 91.7% sensitivity and specificity (AUC = 0.82) and the total area of psoas had 83.3% sensitivity and 76.9% specificity (AUC = 0.8). There was a very good correlation between the tomography methods with the gold standard. The correlation with ultrasound and strength was good. In the evaluation of sensitivity, specificity and AUC, it was not found a useful method when comparing with the SMI.The determination of adenosine deaminase (ADA) is useful in countries where the prevalence of tuberculosis (TB) is high/ moderate. The objective of this study was to analyze the cut-off point for ADA in patients with TB at our institution. All patients with pleural effusion were included, from May 2016 to March 2019, except those with positive serology for HIV and transudates. They were grouped into the following diagnoses TB, neoplasm, parapneumonic or other with an unclear cause. ADA determination and culture for mycobacteria were performed on all samples of pleural fluid. A ROC curve was performed to establish the best ADA cut-off point for the diagnosis of TB. Information was collected from 309 patients; 220 were included and 87 had a diagnosis of TB. The best cut-off point obtained for ADA was 52 U/l, with a sensitivity of 79% and a specificity of 90%. The area under the curve (AUC) was 0.90. In patients under 40 years old the best cut-off point was 41 U/l, below that obtained for the total study population. In our population the cut-off point for the ADA value in pleural exudate for the diagnosis of tuberculosis of 52 U/l presents the highest specificity and sensitivity.Carbapenemaseproducing bacteria are involved in infections and colonizations and associated with high morbidity and mortality. Their identification facilitates the design and implementation of interventions aimed at reducing the risk of infections and deaths. The objective of this study was to determine the prevalence of carbapenemase-producing microorganisms in the main public hospital in the city of Corrientes, Argentina, and to determine their susceptibility to antibiotics commonly used in clinical practice. We analyzed 674 clinical samples from the same number of adults admitted to different inpatient units of the Hospital Escuela Gral. José Francisco de San Martín during the period September-December 2018. The bacterial identification was carried out through manual biochemical tests and the susceptibility to antimicrobials was evaluated according to the recommendations of the Clinical and Laboratory Standards Institute. We identified 115 carbapenemase-producing bacteria of the following types KPC (90, 78%), OXA-ACI (24, 21%) and OXA-163 (≈1%). Among the microorganisms involved in infections (n = 56; 49%), most of those of the KPC type (n = 32; 57%) showed sensitivity only against amikacin (27/32; 84%), while most of those of the OXA-ACI type ( 24; 43%) showed significant sensitivity only against minocycline (17/24; 71%) and colistin (n = 19/24; 79%). This study demonstrated the presence of carbapenemase-producing microorganisms in all the investigated hospital units and a high frequency of resistance to antimicrobials commonly used in clinical practice. This information is relevant to adapt the therapeutic schemes and hygienic-sanitary measures to the local reality.Infective endocarditis (IE) is a disease that in recent decades has shown changes in its presentation, diagnosis and treatment. This is a prospective study of 252 patients admitted at a reference hospital in Buenos Aires, Argentina, with a diagnosis of IE and they were grouped according to the decade of admission Group A from January 1988 to December 1997 (89 patients -35.3%-), Group B from January 1998 to December 2007 (88 patients -34.9%-), and Group C from January 2008 to December 2018 (75 patients -29.8%). The characteristics were analyzed and compared age, sex, underlying heart disease, blood cultures and germs, presence of vegetations, surgical treatment and in-hospital mortality. Over the three decades, the predisposing heart condition showed that rheumatic valve disease decreased significantly (p less then 0.0001) while the IE in cardiac devices also increased significantly (p less then 0.0001). The percentage of blood culture-negatives decreased significantly over the years (p less then 0.0001). In-hospital mortality showed a downward trend in the last decade (p = 0.069). The development of complications during hospitalization, the indication for surgery, and the presence of heart failure on admission were independent predictors of in-hospital mortality. The presence of vegetations and febrile syndrome on admission were independent predictor for lower mortality. The comparison over the years showed important changes in the epidemiological profile of IE. Probably due to advances in diagnostic techniques, treatment, and the implementation of interdisciplinary IE teams in the last decade, in-hospital mortality shows a strong tendency to decrease.
