Purpose This study aimed to comparatively analyse the epidemiologic characteristics of fractures among inpatients from rural and urban areas. Methods This study retrospectively analysed patients with traumatic fractures of the limbs, pelvis and spine treated in our hospital from January 2017 to December 2017. Patients from rural and urban areas were classified into Group A and Group B, respectively. Data on age, sex, distribution of fracture locations, injury mechanism, season, date and time when the fracture occurred, length of hospital stay, chronic comorbidities and in-hospital mortality were collected, and compared between both groups. Results A total of 10,046 patients (Group A 4,440; 3,062 males and 1,378 females and Group B 5,606; 3,374 males and 2,232 females) with traumatic fractures were included. The male-to-female ratio was significantly different between both groups (P less then 0.001). In Groups A and B, the patients aged 41.9 ± 21.6 and 45.0 ± 23.5 years old, respectively, showing significant dly. To find any correlation between both knees in ligament injuries, patients were divided into two groups according to coincided anterior cruciate ligament (ACL) rupture. The main body of the literature stem from regions with a high prevalence of penetrating injuries, while data from European institutions remain scarce. We aimed to evaluate a decade of ERT in a Norwegian trauma centre. Methods A prospectively collected series from the institutional trauma registry of all consecutive trauma patients who had an ERT at Stavanger University Hospital (SUH) from 2006 to 2018. Data were extracted using both registry and electronic patient record (EPR) data, including injury profile, demographics and outcomes. Comparison of groups were done by descriptive statistics. Results A total of 26 ERTs were performed during the study period, of which 20 were men (75%) and 6 women (25%). Five patients (19%) survived to hospital discharge, of which 3 men and 2 women with a median age of 46 years (range 24-68). All survivors had thoracic injury as location of major injury (LOMI.). Of the five survivors, four suffered blunt injury and one patient penetrating injury. At one-year of follow-up of the survivors, three patients scored 8/8 on Glasgow outcome scale-extended, 1 patient scored 7/8 and one patient 5/8. Conclusion In this study, ERT conferred good outcome with survival in one of every five procedures. Performing ERT in severely injured patients presenting in extremis appears to be justified even in low-volume centres and in blunt trauma.Background The keystone perforator flap is a simple, popular form of advancement flap that can solve many defects of the lower extremity that can avoid consideration of a microsurgical tissue transfer. The actual circulation to this local flap subtype has not yet been delineated. The recent introduction of inexpensive thermal imaging cameras that can be coupled with a SmartPhone allows thermagrams of these flaps where "hot spots" are known to correspond to the location of pertinent perforators that will provide the requisite flap vascularization. Methods Over the past year, 10 patients were considered for a keystone flap of the lower extremity in lieu of a skin graft or free flap. All patients had a "cold challenge" or thermal stress followed by a thermal recovery recorded by serial thermography to determine perforator "hot spot" location as well as their absence or "cold spots." Flaps were designed and harvested according to these findings. Results In one patient, inadequate "hot spots" were found in all adjacent donor regions that could have allowed a keystone flap alternative. Instead, more safely a skin graft was done. The other 9 patients had 10 flaps. Intra-operative thermagrams predicted 100% flap survival for 9 flaps and marginal ischemia for 1 flap, and was a correct assessment in all cases. selleck compound Dehiscence occurred in the latter flap that healed by secondary intention, while the rest healed uneventfully. Chemical carcinogenic compounds can induce genomic mutations. However, few studies have been conducted on the association between genomic mutation frequency, such as microsatellite instability (MSI), and the age of mice treated with a nitrosourea mutagen. In the current work, we treated young (6 weeks) and old (10 months) mice with N-methyl-N-nitrosourea (MNU) for 4 months; the MSI frequency was then measured using polymerase chain reaction (PCR) and short tandem repeat (STR) scanning. The percentage of animals with MSI in the old group was significantly higher than that in the young group (100% and 75%). The frequency of MSI events was significantly different between the two groups as well (15.8% for old and 9.4% for young). The ratio of MSI loci displayed no obvious difference between the two groups. In addition, a few loci, including D15Mit5 and D8Mit14 exhibited the highest frequency of MSI events. Since specific loci showed increased MSI in the present study and a higher frequency in previous studies, these loci could be regarded as "hot spot". These results suggested that old mice would be more susceptible to this mutagen, and prone to accrue MSI. The aim of this study is therefore to investigate the influence of bariatric surgery induced weight loss on DNA oxidation damage in morbidly obese subjects. For this aim, we used cryopreserved peripheral blood mononuclear cells in the FPG comet assay. Advanced protein oxidation products and 3-nitrotyrosine were measured as oxidative and nitrative protein stress markers. link3 Furthermore, expression of oxidative stress related proteins HSP70 and Nrf2 as well as mitochondrial enzyme citrate synthase and NADPH oxidase subunit p22 phox were analysed. Our findings revealed significantly reduced DNA strand breaks, but DNA base oxidation was not reduced. We observed significant reduction in plasma AOPPs and 3-nitrotyrosine, which indicated an improvement in oxidative/nitrative stress. However, expression of HSP70 and Nrf2 were not altered after weight loss. In addition, expression of citrate synthase and p22 phox were also unaltered. Overall, bariatric surgery induced significant reduction in excess body weight and improved the patients' health status, including reduced DNA strand breaks and slightly improved antioxidant status in some of the investigated endpoints, while cellular ROS formation and DNA oxidation damage stayed unaltered. This complex situation may be due to combined beneficial effects of weight loss and burdening of the body with fat breakdown products. In the future, collecting samples two years after surgery, when patients have been in a weight plateau for some time, might be a promising approach.At the 2019 annual meeting of the European Environmental Mutagen and Genomics Society a workshop session related to the use of read across concepts in toxicology was held. There are several approaches and algorithms available for calculating chemical similarity based on molecular descriptors, distance/similarity measures and weighting schemata for specific endpoints. Therefore, algorithms that adapt themselves to the data (endpoint/s) and provide a good ability to distinguish between structural similar and not similar molecules regarding specific endpoints are needed and their use discussed. Toxico-dynamic end points are usually in the focus of read across cases. However, without appropriate attention to kinetics and metabolism such cases are unlikely to be successful. To further enhance the quality of read across cases new approach methods can be very useful.
