It presents with severe proximal muscle weakness, high creatine kinase levels and muscle fiber necrosis. Treatment with intravenous immunoglobulins and immunosuppressants is often necessary. This entity is not commonly known among dermatologists as there are usually no extramuscular manifestations. We report a rare case of statin-associated anti-HMGCR immune-mediated necrotizing myopathy with dermatomyositis-like cutaneous features. The possibility of anti-HMGCR immune-mediated necrotizing myopathy should be considered in patients with cutaneous dermatomyositis-like features associated with severe proximal muscle weakness, highly elevated creatine kinase levels and possible statin exposure. This indicates the importance of muscle biopsy and specific autoantibody testing for accurate diagnosis, as well as significant therapeutic implications. We present a case of milker's nodules in a 41-year-old rancher from Saskatchewan, Canada, with secondary complications consisting of papular erythema and lymphadenopathy.Acne vulgaris is one of the most common skin diseases in the United States and can affect any gender or ethnic group. Post-inflammatory hyperpigmentation (PIH) and scarring from acne can have a negative psychosocial impact on patients. Skin of color patients are particularly prone to PIH, as the dark marks left from acne may take several months to resolve, far after the acne has cleared. Here, we report a case of moderate acne with associated scarring in a transgender, Asian American female who was successfully treated with fixed combination topical therapy with clindamycin phosphate and benzoyl peroxide gel 1.2%/3.75% and tretinoin gel microsphere 0.06%.
An occupational injury is a global health issue, and in hotel industries, little is known about it. The aim of this study was therefore to assess the epidemiology of occupational injuries in international brand hotels.
An institutional cross-sectional study was conducted among 330 workers using a systematic random sampling method. Using EpiData and SPSS, respectively, data were entered and analyzed. The final results were explained and presented with the adjusted odds ratio and the 95% confidence interval, and the results of the qualitative data were triangulated with quantitative data.
Of the 330 study participants, 125 (37.9%; 95% confidence interval (32.7%, 43.8%)) reported having sustained an occupational injury. The study showed those who had four or less family members (adjusted odds ratio 0.484, 95% confidence interval (0.286, 0.818)), those who had low educational background (adjusted odds ratio 1.466, 95% confidence interval (1.098, 1.959)), those who had 2 years or less work experience (adjustng sleep disorders/problems were statistically significant. It is therefore recommended that health and safety training and reinforcement be provided to increase awareness and understand the risk factors at the workplace. Background Magnetic resonance imaging (MRI) is an important yet complex data acquisition technology for studying the brain. MRI signals can be affected by many factors and many sources of variance are often simply attributed to "noise". Unexplained variance in MRI data hinders the statistical power of MRI studies and affects their reproducibility. We hypothesized that it would be possible to use phantom data as a proxy of scanner characteristics with a simplistic model of seasonal variation to explain some variance in human MRI data. Methods We used MRI data from human participants collected in several studies, as well as phantom data collected weekly for scanner quality assurance (QA) purposes. Selleck GDC-6036 From phantom data we identified the variables most likely to explain variance in acquired data and assessed their statistical significance by using them to model signal-to-noise ratio (SNR), a fundamental MRI QA metric. We then included phantom data SNR in the models of morphometric measures obtained from human anatomical MRI data from the same scanner. Results Phantom SNR and seasonal variation, after multiple comparisons correction, were statistically significant predictors of the volume of gray brain matter. However, a sweep over 16 other brain matter areas and types revealed no statistically significant predictors among phantom SNR or seasonal variables after multiple comparison correction. Conclusions Seasonal variation and phantom SNR may be important factors to account for in MRI studies. Our results show weak support that seasonal variations are primarily caused by biological human factors instead of scanner performance variation. The phantom QA metric and scanning parameters are useful for more than just QA. Using QA metrics, scanning parameters, and seasonal variation data can help account for some variance in MRI studies, thus making them more powerful and reproducible. While it is well established that the haemodynamic cause of hypoxic pulmonary hypertension is increased pulmonary vascular resistance, the molecular pathogenesis of the increased resistance remains incompletely understood. Macrophage migration inhibitory factor is a pleiotropic cytokine with endogenous tautomerase enzymatic activity as well as both intracellular and extracellular signalling functions. In several diseases, macrophage migration inhibitory factor has pro-inflammatory roles that are dependent upon signalling through the cell surface receptors CD74, CXCR2 and CXCR4. Macrophage migration inhibitory factor expression is increased in animal models of hypoxic pulmonary hypertension and macrophage migration inhibitory factor tautomerase inhibitors, which block some of the functions of macrophage migration inhibitory factor, and have been shown to attenuate hypoxic pulmonary hypertension in mice and monocrotaline-induced pulmonary hypertension in rats. However, because of the multiple pathways through which it acts, the integrated actions of macrophage migration inhibitory factor during the development of hypoxic pulmonary hypertension were unclear.
