AIM The frequency, incidence, and types of fracture of the elbow are different between children and adult. The epidemiology of elbow fracture in children has been the subject of a limited number of studies. This study aims to observe the pattern of elbow fractures in children 18 years of age and younger, during a nine years period. METHODS This is a retrospective study about elbow fracture cases in children 18 years old and younger treated in Saiful Anwar General Hospital Malang in the period of June 2009 until December 2018. The data obtained from the medical record include the age at the time of accidents, fracture site, type of lesion and ipsilateral injuries, time of accidents, and the mechanism of injury. RESULTS There is a total of 99 elbow fracture, and there are 62 male (63%) and 37 female patients (37%). The mean age for the entire group is 7.3 years (8.1 years for male and 7.1 years for female). Most cases are supracondylar fracture (n = 77, 78%). The supracondylar fracture is composed of 17 fractur Panji Sananta, Langga Sintong, Budhi Prasetio, Marvin Anthony Putera, Sri Andarini, Umi Kalsum, Respati Suryanto Dradjat.BACKGROUND Diabetes and malignancy are two chronic diseases that have a major impact on the healthy population in developing countries, both contribute to the increase of morbidity and mortality of the patients. Diabetes presented with hyperglycemia due to insufficiency or resistance of insulin has been associated with an increased risk of thrombosis. Malignancy or cancer is number two killer in the developing countries after infectious diseases, one of the most common cause of death in cancer despite the disease itself is thrombosis, which occurred frequently as cancer cause changes in tumor biology, abnormal vascularisation, endothelial dysfunction, activation of coagulation and inflammation. Poor glucose control reflects by HbA1C has a significant correlation with a hypercoagulable state. AIM This study was to evaluate the relation between hypercoagulable state and glycemic control in diabetic patients with malignancy. METHODS This is a case control study, eighty samples of diabetes (40 samples were diabetes with malignancy) were collected from private hospitals and clinics. HbA1c, blood glucose level and Ddimer were measured. RESULTS The forty five percent of the blood glucose level of the samples was not well controlled. Sixty five percent of the samples in this group were norm weight, overweight 20% and obese 10%. The Ddimer levels were elevated in both groups and there was a significant correlation between HbA1C and Ddimer (p-0.046), blood glucose and age (p = 0.017). CONCLUSION Chronic hyperglycemia will increase the risk of hypercoagulable state, that will also increase the morbidity and mortality rate in diabetes with malignancy patients. This technique, when carried out for the correct indications, is fast, relatively inexpensive and carries a reduced short-term risk for the patient compared with conventional way of removing well-fixed cement. AIM To analyze the effectiveness of cement in cement revision of the femoral stem while performing a revision Total Hip Arthroplasty (THA). METHODS We analyzed a consecutive series of 79 patients who underwent a cement in cement revision THA between June-2015 to June-2018. All the patients were retrospectively analysed for operative time, complications, clinical and radiological outcomes. RESULTS Average age was 76 years (49-86). The mean follow-up was 16.2 months (12-45). The average operative time was 184.6 (90-290) minutes. Most common indication was cup loosening in 28 patients (42.4%), dislocation in 14 patients (21.2%) and stem loosening in 12 patients (18.2%) Nine patients (11%) had one or more complications. Pre-operatively, 10 patients (13%) had lucency at the cement bone interface. Recent review has shown that 8 of these patients' radiographs have remained unchanged, and in 2 of them there is a slight progression of lucency. Common post op clinical complaintswere persistent pain and abductor weakness. Five (6.3%) patients required a re-revision. Most of the patients had a good or satisfactory outcome.No stems showed radiological loosening. CONCLUSION The cement-in-cement technique for revision of the femoral component gave promising results and had the advantages of speed, less blood or bone stock loss, less risk of femoral perforation or fracture, decreased financial costs and reduced post op morbidity. However, in determining initial diagnosis of schizophrenia, measurements focus on the presences of positive and negative symptoms, and general psychopathological features without concerning the BDNF serum, which involves in central nervous system as the main symptom of schizophrenia. AIM To determine the relation of BDNF serum level to cognitive functions of schizophrenia patients based on sub-domain of Montreal Cognitive Assessment Indonesia Version (MoCA-Ina). METHODS This study was carried out based on observational analysis with cross-sectional design study. The samples were collected by non-probability sampling and consecutive sampling by recruiting 65 of male schizophrenia patients at Prof. Dr. M. Ildrem Mental Hospital, Medan, North Sumatera, Indonesia. BDNF serum levels were analysed throughout quantitative sandwich enzyme immunoassay method, while the cognitive functions were conducted by performing the MoCA-Ina, which concern to attentions and concentrations, executive function, memory, languages, visuoconstructional abilities, numerical calculation, and orientation. RESULTS The serum level of BDNF was accounted averagely for 27161.26, with 5350.37 of standard deviation. Its mortality prediction value needs to be determined. AIM To assess the prognostic value of Albumin creatinine ratio (ACR) in septic patients and its ability to predict mortality in comparison with Acute physiology and chronic health evaluation II score (APACHE II). METHODS Seventy-five Septic patients were included within 24 hours of sepsis diagnosis and were admitted to the intensive care unit (ICU). ACR values were obtained within 6 hours of ICU admission for all patients. Prognostic scoring systems [APACHE II and Sequential organ failure assessment (SOFA) scores] were calculated. RESULTS Twenty percent of enrolled patients died within 28 days of hospital admission. ACR was significantly higher in non-survivor in comparison to survivors (55.1 ± 20.5 versus 30.2 ± 35.7, p = 0.006). ACR ≥ 40 (mg/gm creatinine) was the cut-off point for predicting mortality with a sensitivity of 90.7% and specificity of 71.8% with total accuracy of 66% and AUC 0.75 (CI 0.62-0.88). Mean APACHE II score was significantly higher in non-survivors than survivor groups (21.4 versus 10.8, p less then 0.001). ACR was positively correlated with highest SOFA score (r = 0.3, P less then 0.05). CONCLUSION ACR is a simple prognostic marker in septic patient and could be used as a mortality predictor, particularly in early (within 6 hours) septic patients. METHODS Electrical Cardiometry system was used to measure Heart rate variability (HRV) at five different time points before fluid loading (T0, baseline), after fluid loading (T1), 5 min after spinal anaesthesia (T2), 15 min after spinal anaesthesia (T3) and 30 min after spinal anaesthesia (T4). Traditional HRV measurement was determined using time-domain analysis. This Observational descriptive cohort study was conducted in Kasr Al-Ainy Hospital, Faculty of Medicine, Cairo University from February 2018 till June 2019, after approval of the Ethical Committee and written patients consent. RESULTS The main finding of the current study is that heart rate variability measuring using electrical cardiometry is not reliable as a predictor for hypotension following spinal anaesthesia in preeclamptic parturients undergoing elective cesarean section. CONCLUSION Heart rate variability cannot be used as a predictor for hypotension following spinal anaesthesia in preeclamptic patients undergoing elective caesarean section using electrical cardiometry. Surgical correction of the over projected nose is the most difficult and least predictable component of rhinoplasty surgery. AIM By performing rhinoseptoplasty we aimed to achieve an ideal landmarks position according to concepts of nasal projection as well as by making preoperative analyses to determine the ideal position for the nasal tip after having understood and defined the ideal position for the nasion. METHODS We performed a retrospective study of patients who were admitted to ENT University Clinic, University Campus "St. Mother Theresa" Skopje, the Republic of Macedonia in the period 2011-2019. Operative technique rhinoseptoplasty was realised in 70 patient, Marjan Marolov, Vesna Petreska-Dukovska.BACKGROUND The failure of mucus clearance in bronchiectasis can be improved by chest physiotherapy or/and mucoactive agents. AIM To assess the effects of long-term use of carbocysteine on frequency and duration of exacerbations in patients with bronchiectasis. METHODS We performed an observational, non-randomized, open study (a real-life study) including 64 patients with bronchiectasis divided into two groups, examined group (EG) and control group (CG). All participants were treated with appropriate treatment for the stable disease, but in the study, subjects of EG two capsules 375 mg carbocysteine three times a day was added over three months. Daily diary cards realised collection of data regarding the occurrence and duration of exacerbation in all study subjects. RESULTS Over the study period 43 exacerbations were documented, 17 in the EG and 26 in the CG, 10 (23.4%) of which required hospital treatment (four in the EG [23.5%] and six in the CG [23.1%]).
