This study examined 495 American siblings who completed a web-based questionnaire about themselves, their brother-sister with IDD, parents and whether their families completed 11 future planning activities.

Although virtually all families completed some future planning, on average, families completed slightly over half of the 11 activities (75% completed eight or fewer). Families more frequently identified a successor to current caregivers and engaged in planning discussions with one another and with the brother-sister; least often, families completed a letter of intent or began securing residential placements. Future planning activities comprised three domains (1) legal activities, (2) residential activities and (3) family discussions about the future. Variables relating to one or more domains included whether the brother-sister lived in or outside of family home; brother-sister independent living abilities; presence of an intellectual disability; parent caregiving ability; and current sibling caregiving and involvement with the brother-sister with IDD.

Platinum-based chemotherapy is widely used for epithelial ovarian cancer (EOC). As high as 20-25% of EOC patients will not respond to the initial chemotherapy. Accumulated evidences have implied that DNA methylation may serve as a potential bio-marker for chemotherapy-resistant phenotypic screening; however, the pattern underlying primary platinum resistance remains unclear.

Reduced representation bisulfite sequencing (RRBS) analysis was performed to identify differences in methylation status between primary platinum-resistant patients Progression free survival (PFS) (PFS < 6 months, n = 8) and extreme sensitive patients (PFS ≥ 24 months, n = 8). The Qubit 3.0 Fluorometer was used for the quantification of RRBS library. The RRBS library was sequenced on Illumina HiSeq2500 sequencer as 50 bp paired-end reads.

Several factors may affect survival; however, the role of social support has been less studied. Living situation could be an indicator of a person's social support, which predicts mortality in the general population. In this longitudinal cohort study, we considered whether living alone was a risk factor for post-hip fracture mortality compared with living with a partner. Information on hip fractures from all hospitals in Norway from 2002 to 2013 was combined with the 2001 National Population and Housing Census. The association between living situation and mortality during 12.8 years of follow-up in 12,770 men and 22,067 women aged 50 to 79 years at fracture was investigated using flexible parametric survival analysis. We also estimated relative survival of hip fracture patients compared with that of the non-fractured background population in the same living situation (alone or with a partner). Higher mortality after hip fracture was found in both men and women living alone versus with a partner (hazard ratio [HR] men = 1.37, 95% confidence interval [CI] 1.29-1.44; HR women = 1.23, 95% CI 1.18-1.28, adjusting for age, education level, urbanization degree, and number of children). We demonstrated the strongest association in male hip fracture patients aged less then 60 years (long-term mortality HR = 3.29, 95% CI 2.25-6.49). Compared with the general population, relative survival 8 years after a hip fracture was 43% in men and 61% in women living alone, whereas relative survival in those living with a partner was 51% in men and 67% in women. In conclusion, hip fracture patients who lived alone had higher mortality than those living with a partner and lower survival relative to the general population. © 2021 American Society for Bone and Mineral Research (ASBMR).
Many cardiac surgeons receive training for sternotomy-based cardiac surgical operations in residency programs and only a few education programs offer training specifically in minimally invasive cardiac surgery. In this report, we aimed to search and analyze the learning curve for robotic-assisted mitral valve (MV) repair in cardiac surgeons.

Between January 2010 and July 2019, 60 robotic-assisted isolated MV repair surgeries were performed with DaVinci Robotic Systems in our center. Different kinds of surgical techniques were used. The assessment of the learning curve was based on cardiopulmonary bypass (CPB) and transthoracic aortic clamp (CC) times.

There were 23 (38.3%) men and 37 (61.7%) women with a mean age of 48.3 years. The lesions of the MV were posterior leaflet prolapsus (n = 42, 70.0%), anterior leaflet prolapsus (n = 8, 13.3%), Barlow disease (n = 3, 5%), and annular dilatation (n = 7, 11.6%). The patients underwent notochordal implantation (n = 27, 45%), quadrangular or triangular resection (n = 23, 38.3%), isolated ring annuloplasty (n = 7, 11.7%), resection, and leaflet reduction (n = 2, 3.3%) or edge to edge repair (n = 1, 1.7%). The maturation of the learning curve appeared to be about 30 cases. The statistical analysis showed that the mean CPB and CC times for the first 30 cases were greater compared with the 30 after learning curve (155.3 vs. 118.9 min[p = .00], 102.3 vs. 80 min[p = .00], respectively). There was no case of conversion to open surgery. No perioperative mortality was observed.

A total of 17 371 specimens were collected between August 2016 and July 2017, and five species were identified Nyssomyia intermedia (Lutz & Neiva, 1912), Migonemyia migonei (França, 1920), Nyssomyia whitmani (Antunes & Coutinho, 1939), Pintomyia pessoai (Coutinho & Barretto, 1940), and Pintomyia fischeri (Pinto, 1926). In relation to the number of sand flies collected, we observed a significant difference between rural and periurban areas and between the neighbourhoods. We observed that N. Veliparib molecular weight intermedia has a relatively greater adaptation to the peridomiciliary environment. This is the first report of Pi. pessoai and Pi. fischeri (only females) in Seropédica as well as the first record of the frequency of phlebotomine species focusing on some ecological indices and the spatial distribution of sand flies. Thyroid ultrasound, thyroid scintigraphy and radioactive iodine uptake (RAIU) tests are helpful in the differential diagnosis of thyrotoxicosis and hyperthyroidism. In the setting where these techniques are unavailable or unusable more accessible, and cheaper techniques would be helpful.

We evaluated the capability of free triiodothyronine to free thyroxine ratio (FT3/FT4) to differentiate Graves' Disease (GD) and destructive thyroiditis (DT).

In total, 318 patients with GD and 140 patients with DT were included in the study. Patients were assigned to two groups GD and DT (subacute thyroiditis, painless thyroiditis, postpartum thyroiditis). Serum thyroid-stimulating hormone (TSH), FT4, FT3 levels and FT3/FT4 ratio were evaluated in each group. To obtain the optimal diagnostic cut-off value of FT3, FT4 and FT3/FT4 ratio, ROC curve analysis was performed of all untreated thyrotoxicosis patients.

The optimal FT3/FT4 ratio cut-off value was 2.96, with a sensitivity of 71.7%, the specificity of 88.6%. The area under the ROC curve of the FT3/FT4 ratio regarding the diagnosis of GD was 0.864 (95% CI 0.830-0.894). The cut-off level of FT3 for GD was determined as 6.6pg/mL which had a sensitivity of 72.3% and specificity of 68.6% (AUC=0.771 P<.001). The cut-off level of FT4 for GD was determined as 3.65ng/dl with a sensitivity of 35.5% and specificity of 83.6% (AUC=0.615 P<.001). When a high specificity is needed, FT3/FT4 cut-off value increases to 3.63 with 99.3% specificity and 36.5% sensitivity.

FT3/FT4 ratio helps distinguish GD and DT. In cases of situations where RAIU/scintigraphy and TRAb cannot be studied, the FT3/FT4 ratio is a viable diagnostic tool. Cut-off values with higher specificity can be more helpful in differential diagnosis of GD.
