Although extracorporeal cardiopulmonary resuscitation (ECPR) improves survival outcomes in refractory cardiac arrest, morbidity and mortality remain significantly high. Information on causes of death in ECPR is limited; however, some evidence suggests withdrawal of life sustaining therapy (WLST) is a major factor in ECPR-associated mortality. We sought to describe the patients experiencing WLST after ECPR.
The international Extracorporeal Life Support Organization (ELSO) Registry was retrospectively queried for patients more than 18years old supported with ECPR who underwent WLST due to family request from 2007 to 2017. These patients were split into groups for descriptive and multivariable analysis early (WLST<72 hours from cannulation) and routine WLST.
Overall, 411 ECPR patients experienced WLST (median age 42years IQR=28-51; 31.7% female) over the 10-year period. 55.5% (n=228) underwent early WLST with a median ECPR duration of 24 hours (IQR=7-48) versus routine WLST (median=147 hours; IQR=105-23adjust confounders for ECPR-associated death and focus on prognostication.Decreased expression of the δ subunit of the GABAA receptor (GABAAR) has been found in the dentate gyrus in several animal models of epilepsy and other disorders with increased excitability and is associated with altered modulation of tonic inhibition in dentate granule cells (GCs). In contrast, other GABAAR subunits, including α4 and γ2 subunits, are increased, but the relationship between these changes is unclear. The goals of this study were to determine if viral transfection of δ subunits in dentate GCs could increase δ subunit expression, alter expression of potentially-related GABAAR subunits, and restore more normal network excitability in the dentate gyrus in a mouse model of epilepsy. Pilocarpine-induced seizures were elicited in DOCK10-Cre mice that express Cre selectively in dentate GCs, and two weeks later the mice were injected unilaterally with a Cre-dependent δ-GABAAR viral vector. At 4-6 weeks following transfection, δ subunit immunolabeling was substantially increased in dentate GCs on the transfected side compared to the nontransfected side. Importantly, α4 and γ2 subunit labeling was downregulated on the transfected side. Electrophysiological studies revealed enhanced tonic inhibition, decreased network excitability, and increased neurosteroid sensitivity in slices from the δ subunit-transfected side compared to those from the nontransfected side of the same pilocarpine-treated animal, consistent with the formation of δ subunit-containing GABAARs. No differences were observed between sides of eYFP-transfected animals. These findings are consistent with the idea that altering expression of key subunits, such as the δ subunit, regulates GABAAR subunit assemblies, resulting in substantial effects on network excitability.
To characterize different phenotypes of early pulmonary hypertension (PH) in preterm infants and their respective associations with bronchopulmonary dysplasia (BPD) and survival.
A prospective cohort study in a tertiary university medical center from June 2016 until March 2019. Infants with a gestational age <30weeks and/or a birth weight <1000g were included. Echocardiographic assessment for PH was performed at 3-10days after birth. Subsequent development of BPD at 36weeks postmenstrual age and mortality were assessed.
Early PH was identified in 55% of 104 included infants, including 21% with persistent PH of the newborn (PPHN), 61% with flow-associated PH, and 18% PH without shunt. Only PPHN was associated with placental fetal vascular malperfusion, lower gestational age, and low Apgar score. Both PPHN and flow PH were associated with the development of BPD. Early PH was associated with poorer survival, driven by PPHN.

To test whether prospective classification of infants with bronchopulmonary dysplasia identifies lower-risk infants for discharge with home oxygen who have fewer rehospitalizations by 1year after neonatal intensive care unit discharge.
This is a prospective single-center cohort that included infants from 2016 to 2019 with bronchopulmonary dysplasia, defined as receiving respiratory support at 36weeks of postmenstrual age. "Lower-risk" infants were receiving ≤2L/min nasal cannula flow, did not have pulmonary hypertension or airway comorbidities, and had blood gas partial pressure of carbon dioxide <70mm Hg. We compared 3 groups by discharge status lower-risk room air, lower-risk home oxygen, and higher-risk home oxygen. The primary outcome was rehospitalization at 1 year postdischarge, and the secondary outcomes were determined by the chart review and parent questionnaire.
Among 145 infants, 32 (22%) were lower-risk discharged in room air, 49 (32%) were lower-risk using home oxygen, and 64 (44%) were higher-risk. Lower-risk infants using home oxygen had rehospitalization rates similar to those of lower-risk infants on room air (18% vs 16%, P=.75) and lower rates than higher-risk infants (39%, P=.018). Lower-risk infants using home oxygen had more specialty visits (median 10, IQR 7-14 vs median 6, IQR 3-11, P=.028) than those on room air. Classification tree analysis identified risk status as significantly associated with rehospitalization, along with distance from home to hospital, inborn, parent-reported race, and siblings in the home.

Genetic and chemical modifications allow for modular design of protein nanocarriers with effective drug encapsulation, targetability, stimuli responsiveness, and in vivo half-life. Protein nanocarriers have been developed to deliver various therapeutic molecules including small molecules, proteins, and nucleic acids with proven in vitro and in vivo efficacy. This article reviews recent advances in protein nanocarriers that are not derived from natural protein nanostructures, such as protein cages or virus like particles. The protein nanocarriers described here are self-assembled from rationally or de novo designed recombinant proteins, as well as recombinant proteins complexed with other biomolecules, presenting properties that are unique from those of natural protein carriers. Design, functionalization, and therapeutic application of protein nanocarriers will be discussed.
The environmental effect in heart failure (HF) patients is well established. However, the data is limited from low-to middle-income countries like India. This study determined the impact of environment on acute decompensated HF (ADHF) admissions and mortality in India.
Retrospectively, the data of all HF patients admitted between April 2017 and March 2019 was accessed through electronic hospital records. Simultaneously, the environmental-related data was collected from the central pollution control board.
The study included 4561 patients of ADHF. The peak of monthly ADHF events (admission and mortality) was observed during the chilly month (January) while the lowest rates were observed in summer months (May-June). The most significant factor correlating inversely with the monthly ADHF admission (r=-0.78, p=0.003) and mortality (r=-0.65, p=0.004) was the maximum air temperature, and it was found to be the independent predictor for both ADHF mortality [t=-2.78, β=-0.84; 95%CI(-6.0 to-0.6), p=0.021] and admission [t=-4.83, β=-0.91; 95%CI(-19.8 to-6.9), p=0.001]. The above correlation was better seen in the elderly subset and male gender. Humidity and the air pollution attributes did not have a significant correlation with ADHF admission or mortality.
In conclusion, even in low-to middle-income country like India, a periodic effect of season was demonstrated for ADHF mortality and admission, with a peak in ADHF events noted during winter months especially in the regions having extremes of seasons. Air pollution could not affect the ADHF outcome for which further studies are needed.

Air pollution could not affect the ADHF outcome for which further studies are needed.This study aims at the transformation of the waste lignocellulosic biomass, Simarouba glauca seed shell obtained from biofuel industries, into a value-added adsorbent for the removal of dyes from aqueous media. The basic dye direct red (DR) 12B was adsorbed using chemically (ZnCl2) and thermally activated Simarouba seed shell carbon (ZASRC and SRC, respectively). Dye removal in batch mode was studied by evaluating adsorbent dosage, contact time, pH, adsorption isotherm and kinetics. Enhanced adsorption of DR12B was attained within 80 min at pH 5 with the maximal adsorption capacity (Q0) of 17.48 and 64.94 mg g-1, for SRC and ZASRC, respectively. Further, the dye removal followed Freundlich isotherm model and pseudo second-order kinetics. The mean-free energy of adsorption demonstrated that dye adsorption onto ZASRC occurs through ion-exchange. Thus, ZASRC can be safely and easily applied for the removal of direct red 12B from aqueous solutions.The present study focused on microwave assisted synthesis of zirconium nanoparticles (ZrO2NPs) using leaf extract of Phyllanthus niruri as ecofriendly approach and assessed its antimicrobial and bioremediation efficiency. Alexidine solubility dmso Visual color transition from yellow to brown indicates the formation of ZrO2NPs which was further substantiated by UV-Visible absorption peak at 300 nm. Dynamic Light Scattering (DLS) analysis revealed that the average particle size of ZrO2NPs as 121.5 nm with negative zeta potential of -22.6 mv. Scanning electron microscopic analysis showed spherical shaped nanoparticles with an average size of 125.4 nm. Results of photocatalytic studies revealed that ZrO2NPs exhibited 74%, 62% and 57%, dye degradation for methyl red, methyl orange, and methyl blue respectively. Antimicrobial studies depicted that ZrO2NPs exhibited bactericidal activity against Bacillus subtilis, Staphylococcus aureus, Escherichia coli, Klebsiella pneumoniae, and Aspergillus niger at dose of 200 μg/mL. Overall results of the present study revealed biogenic synthesis of ZrO2 NPs with potent bioremediation and antimicrobial properties.
