Metabolic syndrome (MetS) is one of the preventable risk factors for cardiovascular disease (CVD). The aim of this study was to investigate the effect of MetS on CVD and cumulative organ damage in a multi-center, large cohort of patients with Takayasu arteritis (TAK).

This is a cross-sectional study involving 192 consecutive TAK patients from seven tertiary rheumatology centers in Turkey. Clinical data of TAK patients fulfilling the 1990 American College of Rheumatology classification criteria were collected from medical records. They were evaluated for risk factors of CVD, disease activity, damage, and MetS at their last visits.

A total of 192 consecutive TAK patients were included in this study. One hundred and fifty-eight (82%) were female, the mean age was 43.3±13years, and mean disease duration was 13.5±9.3years. MetS was detected in 50 (26%) of the patients and CVD was detected in 28 (14.6%). The presence of MetS was detected as an independent risk factor for CVD (P<0.001). In addition, the mean vasculitis damage index of the group with MetS was significantly higher than in the other patients (4.5±3.3 vs 3.2±2.2, respectively, P=0.004).

Glioma is characterized by hypoxia that activates the hypoxia inducible factor (HIF) pathway and controls a myriad of genes that drive cancer progression. HIF-1α promotes GLI1 transferring to the nucleus by activating the hedgehog pathway under hypoxic conditions. However, their mechanisms in glioma cells under hypoxia remain unknown.

Human glioma cell lines (LN229 and LN18) were transfected with HIF-1α or GLI1-specific short hairpin RNAs (shRNAs) and cultured under normoxic or hypoxic conditions. The protein levels of HIF-1α, GLI1, and epithelial-mesenchymal transition (EMT) markers including E-cadherin and vimentin were measured by Western blot analysis. RT-qPCR analysis was performed for the detection of HIF-1α and GLI1 mRNA expression. Cell migratory and invasive capacities were evaluated by wound healing and Transwell assays, respectively.

Hypoxia blocked the breakdown of the HIF-1α protein and upregulated GLI1 expression in glioma cells. Downregulation of HIF-1α expression inhibited hypoxia-induced cell migration and invasion, as well as reversed the effects of hypoxia on GLI1, E-cadherin, and vimentin expression in LN229 and LN18 cells. Depletion of GLI1 inhibited glioma cell migration and invasion induced by hypoxia. Silenced GLI1 did not affect HIF-1α expression but completely offset hypoxia-regulated expression of E-cadherin and vimentin in glioma cells.

This study aims to detect the effects of NNMT on lipid accumulation, triglyceride content, adipocyte differentiation-related transcription factors, genes related to lipid metabolism, adipokine expression, and autophagy in adipocytes. Lentivirus vectors and eukaryotic expression plasmids were used to interfere with NNMT expression. The Oil Red O method was used to detect lipid accumulation, and colorimetry was used to detect triglyceride levels. The transcription of adipocyte differentiation-related transcription factors (PPARγ, C/EBPα, and SREBP1), lipid metabolism-related genes (FABP4, FAS, FATP1 [SLC27A1], and LPL), adipokines (ADIPOQ and LEP) and autophagy-related genes (Beclin1, ATG7, ATG12, and ATG14) was detected by quantitative real-time polymerase chain reaction (RT-qPCR), and the protein expressions of PPARγ, ADIPOQ, LC3I, LC3II, Beclin1, and P62 were detected by western blot analysis. Compared with the control group, the knockdown of NNMT expression reduced lipid accumulation and triglyceride content in 3T3-L1 cells. The transcription of PPARγ, C/EBPα, SREBP1, FABP4, FASN, FATP1, LPL, Beclin1, ATG7, ATG12, and ATG14 decreased, while ADIPOQ and LEP transcription increased. The expression of PPARγ, LC3I/II, and Beclin1 proteins also decreased, while ADIPOQ and P62 protein expression increased. The over-expression NNMT group showed experimental results opposite to those described above. However, its impact on downstream processes such as nitrification and denitrification requires further investigation. This research characterizes the nitrogen removal performance, footprint, aeration mode, and microbial composition of a flow-through membrane aerated biofilm reactor (MABR) downstream of a chemically enhanced primary treatment (CEPT) carbon redirection unit. The batch and long-term studies demonstrated relatively higher nitrification rates than those reported using conventional primary treated wastewaters. The results indicated that reducing carbon in the liquid train positively impacted nitrification by achieving 87 ± 12% (1.4 ± 0.4 g/m2.d) ammonia removal with an effluent 2.5 ± 2.8 mg/L ammonia concentration at a short hydraulic retention time (HRT) of 2.5 h. Despite the lower (1.9 ± 1) soluble CODN, up to 75 ± 25% (0.6 ± 0.4 g/m2.d) total nitrogen removal was achieved at 4 h HRT by implementing intermittent aeration. The batch tests using the developed biofilms showed nitrification (denitrification) capacity up to 11 ± 1.7 gNH4-N/m2.d (8.5 ± 0.5 gNO3-N/m2.d) and 2.7 ± 0.6 gNH4-N/m2.d (2 ± 0.3 gNO3-N/m2.d) corresponding to ammonia and nitrate concentrations ranging from 10-30 mg/L and 2-10 mg/L, respectively. Microbial analysis indicated that the nitrifiers such as Nitrosomonas and Nitrospira were the dominant species. The ammonia-oxidizing, nitrite-oxidizing, and denitrifying bacteria relative abundances were 10.3 ± 1.5%, 20.7 ± 1.7%, and 20.0 ± 2.8% under continuous aeration and 1.3 ± 0.07%, 1.8 ± 0.09%, and 40.5 ± 3.1% under intermittent aeration, supporting the observed ammonia and total nitrogen removal processes, respectively. Overall, the results demonstrated that MABR downstream of the CEPT behave differently; thus, design guides should be updated accordingly.The clinical usefulness of serum placental growth factor (PlGF) as a predictive biomarker of preeclampsia is currently being examined. However, there are still conflicting results in the literature. We assessed the association between maternal low PlGF levels and the occurrence and severity of preeclampsia. This was an analytical cross-sectional study conducted among 60 women with preeclampsia, and an equal number of matched normotensive pregnant women. PlGF concentrations were analysed using the ELISA method. Large common bile duct (CBD) stones often require lithotripsy. In this study, we aimed to introduce a novel device-elbow basket catheter for mechanical lithotripsy (ML) of CBD stones and assess the efficacy and safety of the novel device.

This was a retrospective study. A total of 139 patients with large CBD stones who underwent lithotripsy using the elbow basket mechanical lithotripter (EBML) or conventional basket mechanical lithotripter (CBML) during endoscopic retrograde cholangiopancreatography (ERCP) from June 2016 to September 2021 in our center were enrolled in the study. The stone clearance rate, intubation time, overall procedure time, and adverse event rate were compared between the two groups.

The stone clearance rate in the first ERCP program in the EBML (n=71) group was significantly higher than that in the CBML (n=68) group (98.6% vs. 88.2%;
<0.05). The intubation time in the EBML group was significantly shorter than that in the CBML group (median time; 12s vs. 78s;
<0.001). The overall procedure time in the EBML group was significantly shorter than that in the CBML group (median time; 51min vs. 63min;
<0.001). The adverse event rate (7.4% vs. 1.4%
>0.05) was similar between the two groups.

ML using EBML in patients with large CBD stones is feasible and safe. Compared with the CBML, the stone clearance rate of the EBML is higher in a single procedure and decreases the time of intubation and the overall procedure.
