Feedback latency was shown to be a critical parameter in a range of applications that imply learning. The therapeutic effects of neurofeedback (NFB) remain controversial. We hypothesized that often encountered unreliable results of NFB intervention could be associated with large feedback latency values that are often uncontrolled and may preclude the efficient learning.

We engaged our subjects into a parietal alpha power unpregulating paradigm faciliated by visual neurofeedback based on the invidually extracted envelope of the alpha-rhythm at P4 electrode. NFB was displayed either as soon as EEG envelope was processed, or with an extra 250 or 500-ms delay. The feedback training consisted of 15 two-minute long blocks interleaved with 15s pauses. We have also recorded two minute long baselines immediately before and after the training.

The time course of NFB-induced changes in the alpha rhythm power clearly depended on NFB latency, as shown with the adaptive Neyman test. NFB had a strong effect on the alpn delivered to human subjects at short latency, which guarantees that NFB arrives when an alpha spindle is still ongoing. Such a considerable effect of NFB latency on the alpha-activity temporal structure could explain some of the previous inconsistent results, where latency was neither controlled nor documented. Clinical practitioners and manufacturers of NFB equipment should add latency to their specifications while enabling latency monitoring and supporting short-latency operations.
Currently, continuous blood pressure (BP) measurements are mostly based on multi-sensor combinations and datasets with limited BP ranges. Besides, most BP-related features derive from the photoplethysmogram (PPG) signal. The mechanism of PPG signal formation is not considered. We aimed to design a noninvasive and continuous method for estimation of BP using a single PPG sensor, which takes the mechanism of PPG signal formation into account.

We prepared a dataset containing PPG signals for 294 patients from three public databases for constructing the BP estimation model. The features used in the model consisted of two types novel features based on a multi-Gaussian model and existing features. The multi-Gaussian model fitted the different components (i.e. the main wave, the dicrotic wave and the tidal wave) of the PPG signal. Ensemble machine learning algorithms were applied to estimate systolic blood pressure (SBP) and diastolic blood pressure (DBP). When partitioning the dataset, there was an overlap between the training set and the testing set.

Datasets with a wide-range of SBP and DBP values (SBP ranging from 74 to 229 mmHg and DBP ranging from 26 to 141 mmHg) were used to evaluate our method. The mean and standard deviation of error for SBP and DBP estimations were -0.21 ± 5.21 mmHg and -0.19 ± 3.37 mmHg, respectively. The model performance fully met the Association for the Advancement of Medical Instrumentation standard and was grade 'A' on the British Hypertension Society standard.

The gut microbiota continuously adapts to the host environment and serves multiple critical functions for their hosts, including regulating host immunity, procuring energy from food, and preventing the colonization of pathogens. Mounting evidence has suggested gut microbial imbalance (dysbiosis) as a core pathophysiology in the development of GI motility and metabolic disorders, such as irritable bowel syndrome and diabetes. Current research has focused on discovering associations between these disorders and gut microbial dysbiosis; however, whether these associations are a consequence or cause is still mostly unexplored. State-of-the-art studies have investigated how gut microbes communicate with our body systems through microbiota-derived metabolites and how they are able to modulate host physiology. There is now mounting evidence that alterations in the composition of small intestinal microbes have an association with GI dysmotility and metabolic disorders. Although treatment options for gut microbial dysbiosis are currently limited, antibiotics, fecal microbiota transplantation, probiotics, and dietary interventions are currently the best options. However, treatment with broad-spectrum antibiotics has been viewed with skepticism due to the risk of developing antibiotic resistant bacteria. Studies are warranted to elucidate the cellular and molecular pathways underlying gut microbiota-host crosstalk and for the development of a powerful platform for future therapeutic approaches. Here, we review recent literature on gut microbial alterations and/or interactions involved in the pathophysiology of GI dysmotility and metabolic disorders.
Infertile couples consider patient information a very important dimension of patient-centred care. Although testicular sperm extraction (TESE) followed by intracytoplasmic sperm injection (ICSI) has long been offered to infertile couples, little is known about couples' informational needs. The aim of this study was to identify the informational needs of couples undergoing TESE and ICSI, including information content and the channels providing the information as a first step to improve patient-centred care.

We conducted a qualitative study consisting of semi-structured interviews with 11 couples. The topic guide was based on a literature review and interviews with an expert panel. The number of interviews was determined with data saturation. The data were analysed using a constant comparative method.

The couples needed information about many topics. They considered information about the success rates of the treatment, an explanation of the treatment procedure, and other patient experiences the most important. Regarding information channels, the couples preferred face-to-face information, but they also valued a leaflet, website, or an online application, especially when it is personalized or providing interactive functionalities.

We obtained in-depth insight into the information needs of couples undergoing TESE and ICSI. The results of this study give fertility clinics an opportunity to develop patient information that meets the needs of their patients and thus improve patient-centred fertility care.

Inhibition of TAMs is of great significance to reshape the protumor environment to benefit therapeutic outcomes. In this work, we developed a novel TAMs and tumor cells dual-targeting nanoparticle (ATpep-NPs) system for cancer chemotherapy by integrating a docetaxel (DTX)-loaded nanocarrier and a multi-function peptide ATpep, which is composed of a phagocytosis-stimulating peptide-tuftsin (Tpep) fused with a substrate peptide-alanine-alanine-asparagine (AAN) of endoprotease legumain. In vitro protelytic and cellular uptake assays confirmed ATpep-NPs can be responsively activated into Tpep-NPs by cleavage of legumain that is overexpressed in both tumor cells and TAMs, which then promoted tumor cells internalization and TAMs phagocytosis through neuropilin-1/Fc receptor pathways. Due to AAN deactivation effect, ATpep-NPs can effectively decrease the Tpep-induced non-specific uptake by M1-polarized and normal macrophage during systemic circulation. Our results of in vivo experiments demonstrated ATpep-NPs outperformed Tpep-NPs in tumor and TAMs dual-targeting delivery efficiency with markedly enhanced efficacy against both tumor growth inhibition and TAMs depletion. A comprehensive characterization of the membranes was carried out through ATR-IR, XRD, and WCA measurements. According to the in vitro hydrolytic and enzymatic degradation and drug release in different aqueous media for two months, the insertion of alkyl pendant grafts and the crosslinking process provided tuneable additional resistance to the whole membrane suitably for the final application of the membranes. Cell culture showed the cytocompatibility and cell proliferation until 7 days. Osteogenic differentiation and mineralization of pre-osteoblastic MC3T3 cells occurred for most of membranes after 35 days as valued by measuring ALP activity (50 nmol 4-np/h/nf DNA) and the deposition of calcium (120-140 μg ml-1).
