No data exist on the epidemiology of children incidentally diagnosed with advanced kidney failure (KF) during evaluation for non-specific symptoms. This is likely related to unrecognized symptoms and signs of CKD. The objective of our study was to evaluate incidentally diagnosed patients with advanced KF requiring long-term kidney replacement therapy (KRT).
An IRB-approved retrospective chart review of children who started KRT with dialysis (hemo- or peritoneal) was conducted. Included were children with no prior knowledge or diagnosis of underlying kidney disease with chronic kidney disease (CKD) disease stage 4 (GFR 15-29mL/min/1.73 m
) or 5 (GFR < 15mL/min/1.73 m
) at initial presentation and started on chronic KRT within 2months of presentation.
Of 177 patients initiating KRT during the study period, 26 (15%) were categorized as incidental advanced KF. This cohort with mean age 12.25years consisted of 42% males, 54% African Americans included 46% with glomerular, and 54% with non-glomerular etiology for kidney failure. Vomiting (42%) and fatigue (39%) were most common, while growth failure (19%) and hyperkalemia (7%) were less frequent on initial presentation. Anemia (100%), hypertension (96%), hyperparathyroidism (96%), and hyperphosphatemia (92%) were the most frequently seen CKD comorbidities. Chronic KRT was started within 24h in 62% and within 2weeks in 88% of the cohort.

Metal artefact reduction (MAR) techniques still are in limited use in positron emission tomography/computed tomography (PET/CT). This study aimed to investigate the effect of Smart MAR on quantitative PET analysis in the vicinity of hip prostheses.
Activities were measured on PET/CT images in 6 sources with tenfold activity concentration contrast to background, attached to the head, neck and the major trochanter of a human cadaveric femur, and in the same sources in similar locations after a hip prosthesis (titanium cup, ceramic head, chrome-cobalt stem) had been inserted into the femur. Measurements were compared between PET attenuation corrected using either conventional or MAR CT. In 38 patients harbouring 49 hip prostheses, standardized uptake values (SUV) in 6 periprosthetic regions and the bladder were compared between PET attenuation corrected with either conventional or MAR CT.
Using conventional CT, measured activity decreased with 2 to 13% when the prosthesis was inserted. In a realistic hip prosthesis phantom, Smart MAR restores quantitative accuracy by recovering counts in underestimated sources. In patient studies, Smart MAR increases SUV in all areas surrounding the prosthesis, most markedly in the femoral neck region. This study is aimed to assess land use-land cover changes (LULCCs) around a wetland ecosystem using high spatial resolution CORONA and Google Basemap satellite imageries. The imageries were processed by digitizing land cover features at 13000 scale in ArcGIS 10.1. The imageries were classified into nine classes, and an estimation of accuracy was performed utilizing the Kappa coefficient and error matrices. The overall accuracy obtained was 94% for the 2017 dataset. The key findings indicated a loss of 23% in the wetland area from 1980 to 2017. While in the vicinity, a significant increase in green spaces (706.2%) and roads (89.4%) was observed. Morphometric analysis revealed that the wetland has lost a surface area of 10.2 ha from 1980 to 2017. The maximum length (Lmax) of the wetland was reduced by 722 m while the maximum width (Wmax) decreased by 78 m. Bathymetric analysis revealed that the wetland is shallow with a depth ranging from 10 to 174 cm. The Carlson's Trophic State Index (TSI) of wetland ranging from 74 to 87 indicates hyper-eutrophic waters. Overall, the loss of wetland area, together with the reduction in morphometric features, low depth, and higher trophic status speak of anthropogenic pressures that are compromising the ecological integrity of this wetland. Therefore, landscape planning and governance are of pivotal importance for the conservation and management of wetland ecosystems in this region.A feeding trial was conducted for 4 months to study the effect of a nutraceutical conglomerate at different levels (0, 0.1, and 0.5%) on stress-exposed (high stocking density, H and feed restriction, R) Labeo rohita fingerlings. Six isonitrogenous diets with a crude protein of 35% were prepared for the different treatments, viz. THR0, THR1, THR5, TNS0, TNS1 and TNS5. Stress significantly reduced the weight gain and feeding of 0.1% nutraceutical improved it in both stress-exposed and non-exposed groups from 3 months onwards. Increased malate dehydrogenase (MDH) and glucose-6-phosphate dehydrogenase (G6PDH) activity was recorded in the 1st and 2nd months stress-exposed groups. However, THR1 group exhibited a reduced MDH and G6PDH activity compared to THR0 group in 1st and 2nd months. Hence, it can be concluded that the duration of multiple stress exposure adversely affected the digestive and metabolic enzymes activities and feeding of 0.1% nutraceutical conglomerates could restore the activities of digestive and metabolic enzymes in rohu fingerlings.
This study aimed to evaluate the performance of a preclinical PET insert in three configurations as a stand-alone unit outside the MRI bore, inside the bore of a cryogen-free 3T MRI and, finally, while performing simultaneous PET/MRI studies.
The PET insert consists of two rings of six detectors, each detector comprising 8 × 12 SiPMs reading out dual offset layers of pixelated LYSO crystals with a 1.4-mm pitch. The inner diameter is 60mm, transaxial field of view (FoV) 40mm and axial FoV 98mm. Evaluation was based on NEMA NU 4-2008 guidelines with appropriate modifications. Spatial resolution and sensitivity were measured inside and outside the MR bore. Image quality, count rate and quantitative performance were measured in all three configurations. Gliomas represent the most frequent central nervous system (CNS) tumors in children and adolescents. However, therapeutic strategies for these patients, based on tumor molecular profile, are still limited compared to the wide range of treatment options for the adult population. We investigated molecular alterations, with a potential prognostic marker and therapeutic target in gliomas of childhood and adolescence using the next-generation sequencing (NGS) strategy.
We selected 95 samples with initial diagnosis of glioma from patients treated at Pediatric Oncology Institute-GRAACC/UNIFESP. All samples were categorized according to the 2021 World Health Organization Classification of Tumors of the CNS, which included 39 low-grade gliomas (LGGs) and 56 high-grade gliomas (HGGs). Nonetheless, there is limited information on the effect of pooling on test performance, and research is required to determine the appropriate number of samples which can be pooled. Therefore, this study aimed to evaluate the use of pooled serum samples as a herd-level surveillance tool for infectious production-limiting diseases bovine viral diarrhoea (BVD), infectious bovine rhinotracheitis (IBR), enzootic bovine leukosis (EBL) and Neospora caninum (NC), by investigating the maximum number of samples one can pool to identify one positive animal, using commercial antibody-detection ELISAs. Four positive field standards (PFS), one for each disease, were prepared by pooling highly positive herd-level samples diagnosed using commercially available ELISA tests. These PFS were used to simulate 18 pooled samples ranging from undiluted PFS to a dilution representing 1 positive in 1,000 animals using phosphate-buffered saline as diluent. A 110 dilution of the PFS resulted in positive results for IBR, BVD and EBL. Moreover, for IBR and BVD, results were still positive at 1100 and 130 dilutions, respectively. However, for NC, a lower dilution (810) was required for a seropositive result. This study indicates that, at herd-level, the use of pooled serum is a useful strategy for monitoring infectious diseases (BVD, IBR and EBL) but not NC, using readily available diagnostic assays.Search to a new alternative to control bovine ticks (Rhipicephalus microplus), the present study aimed to evaluate the acaricidal activity of organo-modified siloxane alone and in association with different commercial products or with piperonyl butoxide (BPO). Engorged females were subjected to an in vitro immersion test and 10 groups were used control, 0.5% siloxane, 1% siloxane, 2% siloxane, 0.5% siloxane + 5% BPO, 1% siloxane + 5% BPO, 2% siloxane + 5% BPO, commercial product, 0.5% siloxane + commercial product, and 1% siloxane + commercial product. After immersion, engorged females were incubated for 14 days for oviposition and hatchability tests. Another immersion test was performed with 5% siloxane and 2.5% siloxane + 10% BPO to evaluate the histopathological changes. Then, engorged females were incubated for 0, 2, 4, 8, 12, 24, 36, 48, 60, 72, 84, 96, 120, 144, and 168 h and immersed in 10% formaldehyde for later analyses. The unassociated siloxane showed an acaricide efficacy of 93.88% at 2% concentration, and when associated with 5% BPO, it reached 100% efficacy at all tested concentrations.
